TREATMENT PLAN

Patient: Ms J
Age: 45
Diagnosis: Schizoaffective Disorder with Depression

OCCUPATION REASON FOR DIFFICULTY


1. Home Management Struggling to keep up with household tasks

Observation: Associated with Ms J’s diagnosis


of Schizoaffective Disorder and
Depression

2. Personal Hygiene and Grooming Ms J. reportedly been arriving at the group session
with her hair uncombed, yet clean

Observation: Associated with Ms J’s diagnosis of


Depression. Withdrawn.

3. Social Interaction and Community Ms. J appears to be distracted by her own thoughts
more than the environment

Observation: Associated with Ms J’s diagnosis of


Schizoaffective Disorder and
Depression

4. Leisure Exploration Given directions to create a collage, when Ms. J


was struggling to get the glue bottle to dispense
glue, after only a few of tries she stops and sits
quietly, possibly discouraged

Observation: Deficit in engaging in complex


activity, can only engage in simple
one step activities that involves use of
few materials or tools. Impatient

ASSETS BEHAVIORAL JUSTIFICATON


Hard Worker Ms. J complete all her course work for a Bachelors
in Special Education, but after being hospitalized,
she never completed her teacher certification;
however she persevered and completed her degree
at a local college and became a librarian

Responsible and Reasonable After a family discussion ensued, Ms J identified


that
due to her psychological issues her son would be
better off living with her parents while he was
young
Involved with Community Ms. J is an active member of the church community
and
her mother reports that this is a good source of
social support for Ms. J

PROBLEMS BEHAVIORAL JUSTIFICATON


1- Suicidal Ms. J was yelling and crying and yelling saying she
wanted to die while waving a knife

2- Lack of Optimism, defeatist Ms J was unable to state a goal for her treatment
saying, “What’s the point anyway” with tears in her
eyes

3- Social Interaction Deficit Ms. J rarely initiates interaction in group.

4- Distracted and Attention Deficit Several questions needed to be repeated to Ms J….


She attends to a task 5 minutes before becoming
distracted and stopping her participation in the task.

5- Sleep Deprivation She reportedly been missing doses of her


medication and is not sleeping well.

GOALS / OBJECTIVES

1 - Long Term Goal: Help Ms. J regain her self-esteem, values and life role in her life

Short Term Goal


 Reinforce and Build Ms J’s confidence.
 Encourage a positive mood
 Improve social skills and social interaction
 Decrease episodes of anxiety and restlessness caused by depression.

Group: Ms. J will participate in Parallel Group 2 times per week for 30 minutes

Activities to be used
 Create an autobiographical and chronological photo album diary
 Ping Pong Tournament

Environment
 Organize and arrange a homestyle setting
 Maintain a nostalgic playlist of “soft rock” and have play in the background
 Placement of plants throughout to encourage selfless responsibility

Therapist Interventions
 Communicate with individual using a soft inviting voice
 Speak slowly and speak clear allowing time for Ms J to react
 Develop Parallel Group activities
2 - Long Term Goal: Help Ms. J retain a stable emotional health.

Short Term Goal


 Decrease the number of emotional outbursts
 Assist in improving keeping up with taking medication on schedule

Group: Ms. J will participate in Ego-Centric Cooperative Group 2 times per week for 30 minutes

Activities to be used
 Watering Plants encourages responsibility
 Paint by Numbers Art Exhibits will reinforce independent expression

Environment
 Large Windows and wide space to invite sunlight
 Peaceful background music
 Soft homey furniture (not clinical)

Therapist Interventions
 Therapist anticipates minimal verbal or physical response by someone like Ms. J.
 Therapist will be advised not to become discouraged at Ms J’s inactivity; do not show
approval or disapproval of any behaviors exhibited by her.
 Accepting that Ms J as she is providing just enough support to earn trust.
 Interventions should be solo, short term, simple and structured activities initially until she
opens up more and trust more.
 Ms. J’s efforts to avoid any implication of pleasure is her way of communicating
depression and her feelings. She must cooperate, but at a pace comfortable for her.

3 - Long Term Goal: Re-enter workforce

Short Term Goal


 Improve home management
 Improve sleep preparation

Group: Ms. J will participate in Cooperative Group 3 times per week for 30 minutes

4 - Long Term Goal: Reestablish role in community

Short Term Goal


 Attend bible study
 Reestablish routine of going to church service

Group: Ms. J will participate in Cooperative Group 3 times per week for 30 minutes
