CANCELLATION/RELEASE OF DEED OF ASSIGNMENT

KNOW ALL MEN BY THESE PRESENTS:

The Pag-IBIG Fund, a government financial institution duly created and

existing under and by virtue of Republic Act No. 9679, with principal office at the
Petron MegaPlaza, 358 Sen. Gil J. Puyat Avenue, Makati city represented in this

Contract to Sell subject thereof, in view of the default of the buyer in the payment
of his/her monthly amortizations on his/her Pag-IBIG housing loan secured by the
said Deed of Assignment.

That having cancelled the Contract to Sell dated _
