A randomized controlled trial comparing the effect of fortification of human

milk with an infant formula powder versus unfortified human milk on the
growth of preterm very low birth weight infants

Sir,
The original research titled “A randomized controlled trial
comparing the effect of fortification of human milk with
an infant formula powder versus unfortified human milk
on the growth of preterm very low birth weight infants”
published recently in your very well read journal was
indeed a very pertinent study and a commendable
effort [1]. I congratulate the authors for publishing this
Randomized Controlled Trial in an esteemed journal as
that of yours.
The authors have mentioned that the WHO guidelines
for low birth weight infant feeding does not recommend
the routine use of human milk fortifiers (HMF) in very
low-birth weight (VLBW) neonates [2]. However, it is men-
tioned in the same guidelines that though routine use of
HMF is not recommended, HMF use should be restricted
to VLBW neonates who do not gain weight adequately,
despite reaching full enteral feeds even in resource lim-
ited settings [2]. Did the feeds of the VLBW neonates in
the control arm who were not supplemented with forti-
fiers and who did not gain weight adequately were sup-
plemented with HMF or preterm human milk powder?
