St., Brgy. Caniogan, Malolos, Bulacan, after having been duly sworn into law
hereby depose and state that:

1. That I am the owner of a motor vehicle by virtue of with the following

details more particularly described as follows, more particularly described

2. Said unit is still under the possession of Malolos City Police Station,
Station Drug Enforcement Unit, Malolos City, Bulacan;

3. That I hereby undertake to bring the said unit to the Honorable Court, and
further undertake that I will submit the same or to the Court whenever
required, including myself;

4. I will not sell, encumber, or alienate the vehicle to any third person until the
termination of the case, which involves the same;

5. I am executing this affidavit to attest to the truthfulness of the foregoing
