FOR APPROVAL OF SUPERVISOR THE FOLLOWING REQUIREMENT MUST BE FULFILLED:

1 As per CPSP Policy it is mandatory that applicant at least holds the position of Assistant Prof.
to be approved by CPSP as a supervisor for training of the post graduate candidates and to
have 5 years post fellowship experience after acquiring FCPS/FRCS/FRCP/MRCP/MS/MD or
equivalent qualifications recognized by CPSP to be approved as supervisor
2 Form “B” i.e Supervisor Data duly filled form.
3 The application has to be properly recommended and forwarded by the Principal/Head of
Institution.
4 Appointment letter for the present position in the institute/hospital also mention the
unit/department you are working and how many previously approved supervisors working in
the same unit.

6 Complete Curriculum Vitae (CV)
7 Photocopy of Valid PMDC registration certificate.
8 Photocopy of workshops/Courses
a. Education planning & Evaluation
b. Assessment of Competence.
c. Supervisory Skills
d. Research Methodology, Biostatistics & Medical Writing.
e. Advance Skill Course (According to Specialty ) I.e BLS, ACLS,ATLS, ALSO, PLAS)
9 Good Standing in Fellow directory . (www. cpsp.edu.pk).
10 25 MCQ’s & SEQ’s (as per format) typed with CD ,
