This certificate is to certify that Ms. Jonah Fae Raquel, female, and a resident of P7, Brgy. San
Luis Cauayan City Isabela, was a full-time Staff Nurse in our beloved institution, Demano’s Medical and
Pediatric Clinic, from October 15, 2013 to June 10, 2015. Ms. Raquel was assigned at various wards
(Medical, Surgical, Pediatric, OPD, and Deliver Room) and was able to successfully fulfill her duties as a
nurse.

As a full-time Staff Nurse, Ms. Raquel worked a total of 48 hours a week plus applicable
overtime as required. Her duties include but not limited to:

o Perform detailed assessments of patients conditions to diagnose appropriate nursing

interventions
o Collaborate with physicians and other members of the multidisciplinary team to promote
continuity of patient care
o Provide pre and post-operative nursing care for patients in care
o Administers prescribed medications safely (via oral, intravenous, intramuscular, subcutaneous
routes)
o Monitor, document and report patients’ conditions for potential side effects of medications and
other medical intervention
o Monitor and maintain patient’s charts and confidential files
o Operate various medical instruments and equipments (e.g. blood pressure apparatus, portable
suction apparatus, cardiac monitor/telemetry, portable ECG machine, laryngoscope, etc.)
o Perform sterile procedures including catheterization and dressing changes to maintain aseptic
technique and infection prevention and control
o Educate patients and their families on disease process or condition, medical surgical procedures
or health care needs, options available, and other aspects of therapeutic regimens including
medication and pain management techniques.

This certification is issued to Ms. Jonah Fae Raquel for whatever legal purpose it may serve.
