Following are her Duties and Responsibilities

 Take down and monitor the vital signs of the patients and checks for any irregularities in
condition.
 Maintains and processes patient’s records and update physicians in case there is
negative symptom.
 Helps patients be prepared in any test and examinations.
 Educates patients in the outpatient department; responds to consultations made by the
physicians.
 Performs patient’s histories and physical examinations under physician’s supervisor;
evaluates results and monitor treatment plan; maintains medical records.
 Attends and assist during minor surgical procedures and normal deliveries.
 Provides education and information’s for patients and/ or their careers in the
management of pant, promoting their understanding and their ability to evolve
strategies of self-management.
 Empowers patients to make informed choice regarding drug treatment and lifestyle
changes acting as the patient’s advocate.
 Ensure that the health education needs of patients and careers are met and health
promotion activities are pursued.
 Performs other job related duties as assigned.

This certification is issued as recognition of her ability and for whatever legal purpose or intent
