ADMISSION OF A PATIENT

HOSPITAL SERVICES

1. Purpose :
1.1 To receive a patient in an appropriately prepared and equipped area.
1.2 To meet the immediate physical and psychological need of the patient.
1.3 To the patient to the hospital environment and policies those are applicable to patient and
families
2. Slope :
2.1. All inpatient and outpatient area from where patient admission process take a place
3. Responsibility :
3.1. On study doctor, team leader and coordinator in charge
4. Process :
4.1. The consultant will make judgment regarding need for admission based upon through clinical
evaluation including physiological parameters, detail history and physical examination
4.2. Bed management will be done with the coordination with the billing department, nursing in
charge and duty dr.
4.3. Billing office will process the admission upon assigning of bed by package or service avail by
attendant
4.4. ER on call dr will inform the unit in charge ICU, CCU, HDU, gynae ward, NICU staff prior to
seding patient to the unit
4.5. ER in charge nurse inform the admission office in case bed arrangement issue
4.6. The assigned nurse/ nurse incharge take into consideration clinical need of the potential such
as isolation close observation
5. Preparation of area :
5.1. Prepare the bed according to patient condition
5.2. Patient bed and equipment should be clean an adjusted
5.3. Provide appropriate line i.e bed sheet towel blanket and patient clothes
5.4. Make sure O2 suction and side nail are available or in working condition

Note :

For isolation cases apply yellow tag on patient and bed file

6. Receiving patient from ER/OPD


6.1. The admission procedure on the nursing unit begins when the patient and patient relative
arrive. In case of admission from ED patient occupied by emergency staff in elective /direct
admission if needed then accompany by opd staff nurse or office boy
6.2. The unit inchrge or team leader/senior staff nurse will inform medical officer and attending
physician regarding patient admission
6.3. Assigned nursing staff will perform following activities on the arrival of the patient I the unit
6.3.1. on first counter will greet the patient and relative and introduce self and accompany the
patient on the bed
6.3.2. check and confirm patient identification i.e the patient name and medical record (HIMS)
numbers at the patient identification band with that listed on admission form
6.3.3. helps patient to change in hospital clothing
6.3.4. the assigned staff will provide full orientation of following
6.3.4.1. light switching
6.3.4.2. call bell system

6.3.4.3. bathroom facilities


6.3.4.4. meal timing
6.3.4.5. consultant visit
6.3.4.6. reinforce the following hospital policies
6.3.4.6.1. visiting timings
6.3.4.6.2. no smoking policy
6.3.4.6.3. valuable (money & valuables)
6.3.4.6.4. no food or fruit to be brought in
7. Nursing staff will do following assessment
7.1. check vital signs
7.1.1. TRR
7.1.2.Blood pressure
7.1.3.Cardiac rythem(of monitor attached or ECG advice by dr)
7.1.4.Weight (if not possible, inquire patient for an estimate weight)
7.1.5.Height
7.1.6.Others as appropriate
7.2. Assessment of the risk of fall with in an of the admission
7.2.1.Conduct head to toe assessment
7.2.2.Complete initial assessment form with in the same shift
7.2.3.Will notify the medical officer patient condition indicates
7.2.4.Nursing staff allows relative to visit patient after being settled in bed
7.2.5.Adjust patient bed to low level and lock the wheel
7.2.6.Place call bell water tissue and other items as required with easy reach of the patient (if
the patient has a physical arrange appropriate side of the bed)
7.2.7.Review history and physician order
7.2.8.Initiate the nursing can place
7.2.9.Coordinate of unit or team leader or assign staff nurse will inform nutritionist and food
service about patient admission and dietary requirement( nutritionist and cafeteria
supervisor)
