    Sir:

    Greetings!

    Through the continuous joint efforts of DepEd and GSIS, the latter agency has opened a financial
    assistance loan to all members in DepEd. This program is the GSIS Financial Assistance Loan (GFAL),
    which aims to lend a maximum amount of Php500,000 to DepEd borrowers to pay outstanding balance
    of our loan from accredited private lending institutions (PLIs) at a lower percent per annum on a longer
    term basis.

    Sir, in this connection I come on bended knees to plead for your kind-hearted approval as to the
    issuance of the copy of my Statement of Account (SOA) from MTMAS, Ilagan Branch, Ilagan City, Isabela
    so I could apply for the above mentioned GFAL. This would, be of great help to me and to my family
    since it will increase my net take home pay and can somehow augment the basic needs of my family.

    I am hoping for your generous and immediate response regarding this request.
