The Motel 6 is right next door to the Greyhound station. You can walk over there to get there.
Local area has shopping and restaurants within walking distance. McDonalds, Denny's, Starbucks, etc. There is also
a Clarement shopping Center across the street, if you prefer a supermarket.

If you are attendingpermit class/ school, you will catch the second shuttle, and should be in the lobby at 6:30 A.M.
Hotel will provide the shuttle.
$78.00 CA DMV Fee
All students will receive a Urinalysis & Hair Testing. Another Urinalysis will be done once in Orientation as
part of the hiring process.
Thursday or Friday after Permit Class . Must be prepared to take the written test for both the class C license
(your regular license, even though you already have your license,) and the Class A license when testing for
the Class A Permit.
Monday: Drug Test & DOT physical. Monday - Wednesday: Study for CDL A permit. Thursday or Friday Take the
CDL permit test at the DMV.
Swift Transportation will cover the drug test & the DOT physical. Student must be prepared to cover the $73 CA CDL
A Fee.
Once you pass your Drug Test, DOT physical and CDL A permit, beginning Friday of the same week you will continue
the rest of the driving course in Fontana, CA.
Valid CA drivers License, (HARD COPY, paper copy is not acceptable), PROOF OF DOMICHLE AND BIRTH
CERTIFICATE ARE NEEDED ANY TIME A CHANGE IS MADE to your license, KEEP DOCUMENTS WITH YOU.
Non-laminated Social Security Card, Certified copy of Birth Certificate with a State Office of Vital Statistics or
equivalent. Certified documentation for name change, if applicable (marriage/divorce papers), and United States &
CA residency paperwork (see below for details). *****Bring Blue or Black Pen***
The California Department of Motor Vehicles (DMV) wants to inform commercial driver license (CDL) customers about
new requirements and updated acceptable documents. All new commercial driver license applicants will be required
to provide the DMV with proof of California residency as well as proof of U.S. citizenship, lawful permanent
residency, or legal presence. The DMV has required proof of legal presence since 1994, the list of acceptable
documents has been reduced to align with Federal requirements adopted in 2012.
Legal Presence Documents can include; A
valid, unexpired U.S. passport or passport card, Certified
copy of a birth certificate with a State Office of Vital Statistics or equivalent,
A valid, unexpired Permanent Resident Card,
A valid, unexpired employment authorization, document (EAD), A
valid, unexpired foreign passport with an approved I-94 form

Hotel shuttle will pick up at 12:00 PM and 8:30 PM The Academy shuttle will pick up at 4:00 PM. Any student
arriving after 8:30 PM will be responsible for their own transportation to the motel.

If you have completed school and going to Orientation for Swift, you need to be in the hotel lobby at 5:45 A.M. If you
are attending school, you will catch the second shuttle, and should be in the lobby at 6:30 A.M. Hotel will provide the
shuttle.

$300 minimum recommended for food/personal expenses. Battery powered alarm clock, towel/washcloth, personal
hygiene items, 5-8 changes of clothes for warm and cold weather, work gloves, sleeping bag, binder, voided check if
you want direct deposit, flashlight, no open toe shoes, pack using a duffle style bag, no hard suitcases because of
limited space on the trucks. PREPARE TO LEAVE WITH YOUR TRAINER FOR 5-6 WEEKS IMMEDIATELY
FOLLOWING ORIENTATION.
