Anamnesis

2. FAMILY HISTORY

2.1 Maternal and Paternal Grand Lineage

The client’s maternal grandfather has a known mental illness but

was not diagnosed according to the informant. The grandfather was

showing some signs of depression like staring blankly and not wanting

to eat. The grandfather’s condition lasted only a month and was able to

recover immediately with the help of the family.

2.2 Father

The father is responsible. He supported his three children financially

and they were able to finish college. He was able to provide financially

to the family by trying out numerous jobs. He disciplined his children

through words. The father is a high school graduate. The client’s

parents are separated but the father still supports his children even

until today. The father is an alcoholic and a smoker. He is also gambles

occasionally. Currently, he is residing at the United States of America.

The informant doesn’t know the current job of the father.

2.3 Mother

The mother is understanding and supportive. She worked for 7

years without the support of the father who went to the United States.

The mother worked as a ‘labandera’ in the neighborhood so she was

able to watch her children and afterwards she was able to land a job as

a cashier in a cinema. The mother disciplines her children with verbal

command. The client’s parents are separated. The client’s parents

have currently a casual relationship and they still communicate until

today to support their children.


2.4 Siblings

The client has one older brother and a younger sister. Both the

client’s siblings were able to finish college and now have jobs and a

business. According to the informant, their sibling relationship was

okay. When they were still kids their older brother would take good

care of his two younger sisters especially when crossing down a

pedestrian lane. No history of any juvenile delinquency among the

three siblings.

3. PERSONAL HISTORY

3.1 Prenatal

The mother was very hands on with her pregnancy. She routinely

gets pre-natal checkup. Miss D.T is complete with her immunizations

during her pregnancy. She was happy when she delivered her second

child as well as the others. After having a son, she finally had a

daughter so she was so grateful and excited.

3.2 Birth

The mother delivered the client full term on January 3, 1983. No

unusual incidents during labor. She delivered via spontaneous vaginal

delivery in a hospital in Cavite. The client’s birth was attended by a

doctor.

3.3 Infancy and Childhood Characteristics

The client was initially breastfeed then bottle feed. The client was

weaned at 7 months. The mother cared for her three children without

getting any help from the father after their separation. Client’s first

teething was at the age of 4 months. She begun to talk and walk at the
age of 2 years old. The client was toilet trained at the age of 3 years

old. Potty chair was provided. She was diagnosed with appendicitis

and had an appendectomy at the age of 3 years old.

3.4 Psychosocial History

The client had her first boyfriend in high school. She experienced

her first menstruation when she was in her 2nd year in high school aged

14 years.

3.5 Play Life

Client likes to play Barbie dolls and likes to socialize with both

sexes. She played with children of the same age with her. Client played

outside of their house so that their mother can still watch over them

while doing laundry in the neighborhood.

3.6 School History

The client entered school at the age of 5 years old. She began

learning different things like the alphabet and numbers at the age of 6

when she was in grade 1. She was able to finish grade school on time.

She was an average student and regularly attends school. She had a

good relationship with both her teachers and classmates. In college,

she got pregnant and was forced to transfer to other school. However,

she still finished her bachelor’s degree in nursing.

3.7 Religious and Social Adaptability

The client has many friends from both sexes but she has one

female best friend whom she goes to whenever she has problems. She

is silent and talks calmly. The client took the nursing licensure
examination but failed. According to the informant, the client got

depressed and decided not to take the licensure exam again.

3.8 Occupational History

The client is currently jobless. She worked as a call center agent for

2 months but later on resigned due to lack of motivation. It would take

approximately 2 months for every job the client takes before she

resigned due to lack of motivation.

3.9 Marital History

The client has a former live-in partner. They are not married but

they have 2 children. Their separation was caused by the client’s live-in

partner’s jealousy. The client and her former live-in partner decided to

live together after the client got pregnant during her college days.

3.10 Onset of Present Illness

The client began manifesting symptoms of Bipolar after their

separation with her former partner. According to the informant, the

client would not eat and just stay in her room. She also had difficulty

sleeping. The client lacked the motivation to go to work and would end

up resigning after about 2 months on the job. The client went missing

before and later on she was found on the cemetery sleeping in her

maternal grandmother’s grave. There was one instance in the past

prior to the separation where the former partner locked the client inside

a cabinet and stabbed continuously the door of the cabinet with a knife

due to jealousy. According to the informant, after that incident the client

was never the same anymore.

According to the informant, her father has a mental illness. Her father

was showing some signs of depression like staring blankly and not wanting to

eat. The said mental disorder was not diagnosed that time because they lack

the money for a medical check-up. The client now is diagnosed with Bipolar 1

Disorder with manic episodes. At the age of 12, her parents separated and for

7 years they don’t have any communications with his father. She was locked

inside a cabinet and an almost a near-death experience for her when the

partner stabbed the door of the cabinet with a knife. Her awful experiences in
life might have greatly contributed to her illness. The informant didn’t provide

any vital information about the father of the client and his family history.

5. SOCIOECONOMIC STATUS

The client and his two other siblings were able to finish college with

the help of the father. Prior to the separation of her parents, her mother

was already their breadwinner earning money from doing laundry within

the neighborhood. Her father tried numerous jobs but doesn’t have a

permanent one. Their family also depends on her father’s parents. After

the separation, her father went to the United States because his parents

were there. The three children were left to the care of his wife and they

moved to Davao City from Cavite where the mother originated. For seven

years, they don’t have any communication with her father. Her mother was

their only breadwinner, raising them alone through the money she earns

by doing laundry. After 7 long years, they reconnected and her father

helped them finance their college education and they were able to finish

college. The father still supports his family until now by sending them

money occasionally. Her mother is currently an office worker in a church

here in Davao City. Her sister owns a boutique selling bags and clothes.

The father of his children supports the education and expenses of his

children. The client is currently jobless and depends on her mother.
