COMES NOW, the applicant and unto the Honorable Municipal Mayor most respectfully state
and alleges:

That the applicant is of legal age, married/single, Filipino citizen and residing at

SINARA ALTO, JIMENEZ MISAMIS OCCIDENTAL .

That the applicant desire to operate a passenger Motorela/Motorcab service along the
route hereunto describe as follows:

That the applicant is financially capable of maintaining the operation of the proposed

Motorela/Motorcab service.

That the applicant is willing and ready to comply with the ordinances, resolutions, rules
and regulations imposed by the Municipal Government of Jimenez.

The public necessity and convenience demand the immediate approval of this application.

THEREFORE, IT IS MOST RESPECTFULLY, prayed unto His Honor, the Municipal Mayor that the
aforementioned be issued a PROVISIONAL AUTHORITY to operate a Motorela/Motorcab Service in the
route applied for.
