Lifting Heavy Objects

The vessel was picking up a heavy parcel of 8 steel plates (Approx. 2X1 mtr.) and numerous steel
pipes from a supply barge at anchorage using the port side crane. The Bosun was operating the
crane and few other crew members were guiding/arranging the supply on the poop deck. When
the crew reached the desired location they tried to adjust the load and off land the steel plates.
The vessel was slightly rolling on beam swells (1.5mtr) at this time and a slight roll made the
heavy load shift towards the bunker save all tray. The 3AE tried to control it by pushing it away
from his left hand, whilst the Wiper tried to pull it away. Eventually the load banged on the save
all tray with the 3AE’s left index finger getting crushed between the plates and save all tray
bulkhead.

DONT’s DO’s

1. Before lifting heavy loads, always

prepare a risk assessment and have
a toolbox meeting
2. Remove the risk of injury to
hands/ fingers by using tag lines
/ropes/ lashing for guidance of
heavy loads and keep distance.
3. Always use a supervisor when
carrying out lifting operations, so
that an overview can be maintained.
4. Use hand signals with crane
operator
