Sir:

I am writing to you to request for your approval for a certain proposal regarding
the task that you gave to us in the PE subject specifically, during in our practices. Pointing
out the foremost issue, unfortunately almost one-fourth of the class are not attending the
said practices, and also, majority of the remaining participating students arrive too late.
Hence, we cannot rehearse well due to absences and unpunctuality.

In this regard, I am formally asking, in this signed letter, for your approval around
certain circumstances and equivalent consequences. Firstly, for every 1 minute late
during practices, one must pay 2 pesos. And, for 3 absences without valid and credible
reason/s, one is considered out for the performance. Consequently, those who will not
get involved in our performance will be having a zero grade in this certain area in F2F PE
subject.

Attached here within is the form for those persons with their name/s on the list that
will be nonattending a defined date of practice for important matters.

I would greatly appreciate your understanding and approval in this matter.
