AFFIDAVIT OF CIVIL STATUS

I, MARCIAL A. DE LIRA, JR., of legal age, married, Filipino and a resident of Blk 1,
Lot 4, Ph 3, V and G Subd., Tacloban City, Leyte, after having been duly sworn to in accordance
with law, do hereby depose and state:

1. That I am the Buyer of a parcel of land covered by Original Certificate of Title under
Title No. P-32919 and Tax Declaration No. 08-41035-00049, situated at Brgy. Tinghub, Villaba,
Leyte, containing an area of Thirty Eight Thousand Ninety-Five (38,095) square meters, more or
less, as evidenced by a duly executed Extrajudicial Settlement of the Estate of the Pascuala
Gaviola with Deed of Absolute Sale dated January 28, 2019 entered in the Notarial Registry of
Atty. Gerentstein T. Banzon, as Doc No. 105; Page No. 22; Book No. LXXIII, Series of 2019
(the, “Deed”);

2. That in said Deed, my civil status does not appear;

3. That I hereby execute this Affidavit to attest to the truth that my civil status, at the time I
executed said Deed and at present, is MARRIED;

4. That the name of my wife is NOREEN T. DE LIRA;

5. That this affidavit is being executed to attest to the foregoing facts for the specific
purpose of transferring said property to my name with the Office of the Registry of Deeds,
Tacloban City, Leyte.

IN WITNESS WHEREOF, I have hereunto set my hand this 4 th day of April, 2019 at
Villaba, Leyte, Philippines.
