Feed Hopper :

 Man falling from hopper


 Man hit by loader
 Loader plunging into hopper or off the side of the ramp

Trommel

 Man being caught and pulled under trommel


 Falling from trommel platform
 Objects getting into trammel i.e tools, bars etc

Vibrating Screen

 Clothing caught in screen


 Pinch points
 Falling from conveyor platform

Jigs

 Falling from ladders or jig platforms


 Hit by falling objects from above
 Clothing caught in machinery
Sluice Box

 Falling on sluice box riffles while cleaning


 Water starting while cleaning sluice box

Mill

 Clothing caught in machinery


 Pinch points
 Electrocution
 Fire
 Carrying and dropping heavy objects
STANDARD OPERATING PROCEDURES

Plant
All times - Everywhere

Wear PPE’S
No clowning around
No throwing objects to each other
No unauthorized persons on site - anywhere

Stage 1: Feed Hopper

 Only experienced drivers for the loaders – no trainees, no mobile


phones or headphones
 Loader must sound horn before loading and clear everyone from the
hopper
 Only essential personnel on site : 2 washers and one supervisor as
maximum – no idle spectators can be present
 The washer does not go on to the hopper when washing
 A handrail must be built around the hopper edge
 The second man watches the tubing from the pump to make sure it
does not get tangled in feet.
 Wear PPE’s at all times – boots, hard hat, visibility vest, safety
glasses

Stage 2: Trommel

 No-one goes out on the hopper platform when it is moving


 Anyone on the platform must wear a clip on safety harness
 No attempts to repair or move the trommel when the power is on –
someone must close the switch and guard it.
 Keep all foreign objects away from the trammel opening e.g spades,
crowbars etc and never try to clear blockages of the feed chute with
them
 Do not wear any loose clothing around the moving parts
 Never attempt to clean the machinery when it is moving or power is
on
 Wear PPE’S at all times
Stage 2: Vibrating screen

 Wear PPE’s
 Wear no loose clothing
 Watch for pinch points
 Do not climb on moving machinery

Stage 4: Jigs

 Wear hard hats – falling objects


 No loose clothing
 Wear proper footwear – ladders can be slippery
 Do not try to clean moving machinery
 Do not climb on moving machinery

Stage 5: Sluice box

 Do not climb on sluice box when the water is running


 Do not attempt to sample or clean out sluice box when water running
 Always inform people at the top when cleaning the sluice and make
them wait for all clear to open the pump again
 Wear proper boots, hard hat, visisbility vest and safety glasses

Stage 6: Mill

 Watch for pinch points


 No loose clothing
 Check electrical system and wiring each day – replace worn or
damaged parts instantly
 Have fire extinguishers and buckets close at hand
 Wear PPE’s – especially boots when carrying heavy objects, hard
hats, safety glasses, viz vests
 Switch off all machinery when working on it
 Do not climb on machinery
RISK ASSESSMENT - MINING

1. People being run over by trucks, loaders, dozers


2. People being hit by swinging excavator buckets and booms
3. Vehicle collisions and crashes
4. People falling from the mining equipment
5. Trucks, loaders, dozers running off cliffs and ramps
6. Fires in vehicles
7. Runaway equipment
8. Trucks dumping loads on people and equipment

STANDARD OPERATING PROCEDURES

1. No unauthorized person allowed on site


Do not walk or stand behind moving vehicles or reversing
vehicles
Always wear visibility vests
Drive slowly and steadily
2. Stay outside an excavators swing
Never climb on moving excavators
Always have a spotter with each excavator
Keep all unauthorized persons off site
3. Drive slowly and steadily – never too fast
Look out for other vehicles
Take care when reversing
Follow signals from spotters, excavators and loaders
4. Never climb on moving equipment
Never ride on outside of moving vehicles
5. Always have spotters when working close to ridges, ramps, slopes or
faces
Take greatest care when reversing
6. Have fire extinguishers close at hand
Train people to use them
7. Use hand breaks and leave vehicles in gear when parked
Do not get out of vehicles with engines running
No unauthorized drivers or persons in the cabs
8. Use spotters when dumping in areas where people are working
9. General

Always wear PPE’s especially viz vests, hard hats, boots


Never operate machinery when intoxicated
Operators must not use mobile phones or headphones
No unauthorized people on site
Use spotters with machines
Use only qualified machine operators
