LOCKER RENTALS FOR TERM 1, A.Y.

2018-2019

Thank you for showing interest in renting a locker through our ​Pahiram Locker services! To complete the
process of your application for this service, please continue reading the following terms of agreement:

1. Present your own padlock and key to any representative of the Student Services team of the
Office of the Vice President of Internal Affairs (OVPIA). You will be given the official sticker for
locker rental services. Please ensure that the sticker is evident at all times throughout the term.
Lockers seen with padlocks without the official sticker will be considered invalid and will be
forcefully broken with or without prior notice to the owner.
2. Please be responsible of your belongings. The University Student Government (USG) and the
Student Discipline and Formation Office (SDFO) will ​NOT be held liable for any losses and/or
damages to the items inside the locker, hence, the requirement of a padlock to reduce the risk
of theft.
3. Lockers must always be kept clean and tidy. Storage of any object which may be spoiled is
strictly prohibited. Any student that have such objects in the borrowed locker may have their
padlocks forcefully broken with or without prior notice to the owner and be given a warning slip
by the SDFO.
4. In the case of lost keys or forgotten padlock codes, you may request the locker to be forcefully
opened for a fine of Php 80. In case of such, please contact either Alicia Alon (09985713135) or
Cj Castro (09175976861).
5. People caught using an unauthorized locker will have their padlocks be forcefully broken and be
given a slip from the SDFO as a minor offense.
6. All lockers must be emptied on or before Grade Consultation Day (December 18, 2018). Failure
to comply will result to a fine of Php 200. Once your locker has been forcefully opened due to
expiration of the rental period, your ID will be marked by the SDFO as the student is to be held
responsible of updating him/herself of the deadlines
7. The USG reserves the right to refuse rentals.

By paying for the rental and receiving the sticker for the padlock & an official receipt, you agree to abide
to all the terms and conditions above in exchange of availing to the ​Pahiram Locker​ services.
