JOINT AFFIDAVIT OF SUPPORT AND GUARANTEE

We, _____ and ____, both of legal age, Filipinos, married and with
residence and postal address at _____, after having been duly sworn in
accordance with law, hereby depose and state that:

1. We are the legitimate parents of ______ (hereinafter referred to as “___”), a

minor who is presently seventeen (17) years old, Filipino, single and with
residence and postal address at ______;

2. Our daughter will be accompanied by her legitimate aunt ______

(hereinafter referred to as “______”) in a forthcoming travel outside of the
Philippines;

3. We are giving our full consent to our said minor child to travel to
Singapore sometime in the year 2018;

4. During her stay in said country, ______ will have temporary custody and
will exercise authority over her, ensuring her safety from any kind of
harm while they are outside the Philippines;

5. ______ will likewise be financially responsible for them during such

occasions ensuring that all her needs and her wants are met. ______ will
shoulder all her travel, board and lodging expenses including but not
limited to application fees, airline tickets, terminal fees, taxes, pocket
money and such other expenses that she may incur. She will neither be
a burden to the Philippine Government nor the government of the place
of her destination at any time during her stay abroad;

6. We assume full responsibility for the issuance of the travel clearance and
for giving her permission to travel outside of the country. We likewise
assume any and all obligations consequent thereto; and

7. We are executing this Affidavit to attest to the truth of the foregoing

statements in connection with the requirements for the issuance of a
travel clearance for ______and for all legal intents and purposes.
