AFFIDAVIT OF RESIDENCY

I, JARDELYN VANZUELA CANTOS, Filipino, of legal age and a

resident of Brgy. Alangilan, Batangas City, after having been sworn to in
accordance with law, hereby depose and say that:

1. I have a daughter named ALTHEA V. CANTOS and nieces

named AYESSA KYLE BUAN and HAZEL ANNE DUEÑAS;

2. The classmates of the mentioned children are DAISY

ANDRADE, PRECIOUS MARIE ATIENZA, MAXINE JOYCE SANTOS,
XANDYR VILLANO and CHARLES SYDRICK DE TORRES;

3. Said children will be participating, as the Power Kids Junior,

the upcoming “Pakitang Gilas sa Makabagong Sayaw 2019” of the City
Council for Youth Affairs (CCYA);

4. In relation with the same, I have secured copies of

Certification from the Office of the Barangay Chairman of Alangilan,
Batangas City proving that the mentioned children are bonafide residents
of Brgy. Alangilan, Batangas City, possessing good moral character and no
derogatory record in the community;

5. I hereby attest based on my personal knowledge coupled with

the mentioned facts provided in the aforementioned Certification that
ALTHEA V. CANTOS, AYESSA KYLE BUAN, HAZEL ANNE DUEÑAS,
DAISY ANDRADE, PRECIOUS MARIE ATIENZA, MAXINE JOYCE
SANTOS, XANDYR VILLANO and CHARLES SYDRICK DE TORRES are
residents of Brgy. Alangilan, Batangas City;

6. I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal intent and purpose it may serve;
