Preface
The proposal is based on meeting held between Mr Hasib Baig (ECO QMS.9000) and Ms
Saba Nasir (DD Medical Screening Facility, Punjab Food Authority) & Mr Zahid (AD Medial
screening) dated 16 March 2018.
This proposal covers both technical requirements and financial brief relating to customer
requirements.
Some of the terms and terminologies expressed in this document are being defined below for
better clarity of our proposed plan:

I. ABOUT QMS.9000

II. TECHNICAL ASPECTS

A. Customer Requirements
Punjab Food Authority Medical Screening Facility Lab (Customer) is committed to
demonstrate their competence to generate technically valid Lab results in order to provide
the best quality services and to provide confidence in the validity of laboratory's output.
For this they intend to develop its Lab Management System that governs its operations by
complying with all requirements of ISO/ IEC 15189 and get their Lab accredited from
Pakistan National Accreditation Council (PNAC) to gain international recognition on testing.
B. Scope of the Lab Accreditation Project

Quality Management Systems.9000 (QMS.9000) will provide Consulting, Training and
Internal System Auditing of Laboratory Management System based on ISO 15189 to:
“Punjab Food Authority Medical Screening Facility Lab”
Consulting services by QMS.9000 will include Review and Gap Analysis, System Development,
In House Trainings, System Implementation and Auditing based on international standards for
Medical Laboratories ISO/IEC-15189
Training by QMS.9000 will include trainings on Laboratory Management System and Internal
auditing of Laboratory Management System. QMS.9000 will develop strong teams for
implementation of this standard through trainings, briefings and awareness sessions at all levels
within the Laboratories
Internal Auditing by QMS.9000 will include comprehensive laboratory wide internal audits in
order to ensure that the system is fully implemented and effective before the final
accreditation. All non-conformances will be highlighted so that corrective actions are taken.
Internal audits by QMS.9000 teams also include desktop audits in addition to compliance /
conformance audits.

of Success” will be awarded to participants securing 70% and above and rest will get “Certificate of
Attendance”. Moreover, these trainings will be held collectively at common facility of the client.
Lab System Internal Auditing by QMS.9000 will include comprehensive laboratory wide
internal audits in order to ensure that the system is fully implemented and effective
before the final accreditation. All non-conformances will be highlighted so that

E. Project Execution Plan
 Consultants Team:
Quality Management Systems.9000 will nominate a team of consultants for this

project.

Punjab Food Authority- Medical Screening Facility Lab will nominate a team for
successful and timely development and implementation of this project.

 Working space:

Client is to ensure that QMS.9000 consultants are provided separate office

space and a computer/ printer preferably with operator for preparation of
necessary documentation required for the project.
 Project Reporting:
The consultants of QMS.9000 will send phase wise project plan, describing the
activities to be conducted that phase to the management and a Progress Report at
the end of each phase, which will help the Top management and the project
implementation team to monitor the progress of the project.
 Trainings:
The Chief Consultant/ Project Team Leader will conduct the training sessions. The
client will be responsible to make administrative arrangements for getting the
participants from all labs/ sections, seating plan and multimedia projector.
 Internal Audit/ Auditors Team:
QMS.9000 audit team will conduct comprehensive internal audit in order to ensure
all clauses/requirements of ISO 15189 are addressed. On the basis of performance
of the members of implementation teams, internal auditors will be selected. Chief
Consultant/ Project Team Leader will conduct training on internal auditing to the
selected team of internal auditors.
F. Consultants (QMS.9000) Responsibilities
Chief Consultant
He is responsible to:
a. Execute entire project plan through QMS.9000 Consultants

b. Closely monitor progress of work in order to ensure that the project is handled
properly
c. Report progress of the project to the Client’s management i.e. Chief Coordinator on
regular basis.
d. Provide guidance to the QMS.9000 Auditors, Consultants and Coordinator on
matters related to specific areas
e. Provide all trainings as per the training plan.

a. Provide consulting to the specified area as per the work plan.

b. Ensure that the targets regarding project execution / implementation are met
c. Ensure development of the system in accordance with the project plan
d. Communicate the progress to the Chief Consultant
e. Keep close liaison with the Chief Consultant
f. Immediately report to Chief Consultant about any abnormal activity i.e. Lack of
cooperation from the client, work delays, resource constraints etc.
g. Cooperate with the auditors and assist them during audits
h. Ensure corrective actions on NC’s and decide follow-up date
i. Guide Quality/ Technical Manger regarding documentation and recording
Auditor
a. Prepare Audit Check-list and required auditing material with the help of Lead
Auditor
b. Liaise with Project Coordinator regarding audit schedule and other arrangements
c. Conduct the Audit of the department according to the project plan
d. Identify the progress, project performance and NC’s to the Consultant
e. Report the Progress and NC’s to the Chief Consultant on prescribed documents
f. Pay follow-up audit and report the implementation to the Lead Auditor/ Chief
Consultant.
G. Client’s Responsibilities
In order to implement the effective and efficient role the client team has the following
responsibilities:
Project Coordinator
The Client will appoint ISO 15189 Project Coordinator from the management.
a. This member of the team will speak as a liaison between the organization and the
consultant
b. He/ She will be responsible for overall administration of the Project

c. Project Coordinator will create an action team comprising of Quality /

Technical Managers and Deputy Quality / Technical Managers from each
laboratory, who will be responsible for developing and implementing
laboratory management system with the help of consultants
d. He/ She will ensure that laboratory management system requirements are fulfilled in
accordance with the standard
e. He/ She will ensure that the said system is implemented and maintained
f. He/ She will be responsible to facilitate consultants and coordinate with them for
visits etc.
g. Ensure admin arrangements for consultant’s visits and trainings
h. He/ She will keep the top management informed about the progress on Laboratory
Management System project and its implementation
i. Co-ordinate with the Accreditation Body for final accreditation audit
Quality / Technical Manager
He/ She will:
a. Act as Laboratory Representative for ISO 15189 systems

b. Co-ordinate with the Consultants (QMS 9000) for accreditation process of ISO 15189
c. Pass on instructions of the Consultants to the team members for compliance as per
standards
d. Be responsible for obtaining approval of the project works required for the purpose.
e. Ensure that the said system is implemented and maintained
f. Keep the Project Coordinator informed about the progress on LMS project and its
implementation
g. Take up any other assignments related to laboratory management systems given by
senior management
Member Implementation Team
He/ She will:
a. Provide assistance and support to Manager Quality and Technical in implementing

various steps required during project implementation phase
b. Highlight and identify processes for improvement
c. Provide technical information to concerned where required for the implementation
and execution of project

d. Ensure implementation of LMS and educate the laboratory personnel under

their chain of command
e. Keep him/ her updated on all laboratory issues
f. Take up any other assignments related to laboratory management systems given by
senior management
