To the Participant:

You are requested to be one of the key informants that will help materialize this
phenomenological study. You are chosen as participant of this study because we firmly
believe that you can give valid and accurate information about the concern of my
endeavor.

Purpose of the Study:

The main purpose of this study is to know the Views of the Grade 10 Students
on the Different Senior High School Track that is offered

Research Objective:

1. To know the Grade 10 students views on the different Senior High

School Track offerings.

Procedures:
 If you decide to be part of this study, you will also agree on the following
procedures:

I will tape record our interview and when you have ample time to say what you
would like to share about the topic, you are free to say it. The purpose of the tape
recording is to secure accurate information that is useful for this study. I will share a list
of the questions with you prior to the interview. Additional questions may arise during
the interview. The recorded information and answers will be kept privately and be
treated with utmost confidentiality. To protect your privacy, you will be referred to as an
acronym (P1 or P2 or P3… which means Participant 1, or 2, or 3).

In addition, I will also take photos from our encounter and your identity will be
protected by using a reference of (P1 or P2 or P3) which helps us indicate Photo 1,
Photo 2 or Photo 3).

Statement of Consent:

I have read and understand the explanation provided to me. I have had all my
questions to my satisfaction, and voluntarily agree to participate in this study.
