JOB SUMMARY
The Medical Records Clerk assembles patients’ health information, make sure that patients’
initial medical charts are complete, all forms are completed and properly identified and signed,
and that all necessary information is in the computer.

SIGNIFICANT DUTIES AND FUNCTIONS


Under supervision maintains patient files and responds to requests for medical records; performs
all daily, weekly, monthly, bi-annual and annul tasks/duties as outlined in the standard operating
procedure manual.
Ø Maintains patient files and deliver charts for scheduled appointments
Ø Pulls charts, places out-guide, delivers to providers and to other departments as requested,
retrieve charts at the end of day and re-files the chart in appropriate sequence.
Ø Files all patient data upon receipt of information
Ø Maintains patient/client confidentiality
Ø Abides with all HIPAA rules and regulations.
Ø Initiates/set-up medical chart for new patients with all required forms and creates computer
index
Ø Prepares file labels
Ø Perform required EMR data entry
Ø Maintains filing statistics
Ø Audits filing sequence.
Ø Responds to requests for medical records; processes letters and reports
Ø Answers and directs telephone calls.
Ø Retrieves patient schedules from computer system
Ø Release information to persons and agencies according to regulations
Ø Responds to correspondence requests.
Ø Perform 35 random chart audits monthly
Ø Photocopies records and documents for billing and/or legal services
Ø Sends and receives information via facsimile machine.
Ø Keeps supervisor informed of problems or issues
Ø Monitors and replenishes supplies and prints medical record forms as needed.
Ø Performs other duties as assigned inclusive of travel from one CCCHC site to another to
perform duties/tasks when requested by COO, CEO, Medical Director or supervisor

JOB  REQUIREMENTS  
KNOWLEDGE/SPECIAL  REQUIREMENTS    
Ø Minimum  2  years  previous  experience  and/or  training  in  medical  records.  
Ø Must  present  proof  of  highest  academic  level  of  education  
Ø Must  have  Two  forms  of  identification  to  meet  the  Employment  Eligibility  Verification,    i.e.,  
drivers  license,  social  security  card,  etc.  
Ø Must  be  willing/agreeable  to  work  weekends  if  needed.  
Ø Able  to  travel  from  one  CCCHCC  site  to  another  to  perform  duties/tasks  and/or  distribute  
documents,  as  deemed  necessary.  
Ø Must  be  able  work  at  both  CCCHC  locations,  if  needed.  
Ø Familiar  with  medical  terminology    
Ø Proficient  in  computer  applications  and  data  security    
Ø Knowledge  of  modern  office  equipment    
Ø Ability  to:    
o type  at  least  45WPM    
o organize;  high  level  of  initiative(self-­‐starter)    
o maintain  confidentiality    
o maintain  records  and  files    
 
WORKING  CONDITIONS    
Ø Work  a  40-­‐hour  week,  some  overtime  may  be  required.      
Ø Pleasant  comfortable  environment  
Ø Some  direct  patient  interactions/contact  
Ø Handle  some  work  related  stress  
Ø Sit  for  prolonged  periods  of  time  and  withstand  a  moderate  amount  of  walking.  
Ø Must  be  able  work  at  both  CCCHC  locations,  if  needed.  
Ø Must  be  willing/agreeable  to  work  weekends,  if  needed.  
EDUCATION    
High  School  Diploma  or  GED  
WORKFORCE  PREPARATION  
Employees  in  these  occupations  need  anywhere  from  a  few  months  to  one  year  of  working  with    
experienced  employees.
