1.0 SCOPE
This procedure is to be used at all times when it is necessary to work on equipment that is found
to be faulty or unsafe and where the operation or use of that equipment would cause further
damage or create hazards.

2.0 DEFINITIONS
None

3.0 RESPONSIBILITIES
All persons working near or on equipment with Out of Service or Dangers tags must obey the
following procedure. No one is permitted to use a piece of equipment which has a Out of
Service or Danger tag. Only the person who placed the tag or their supervisor are permitted to
remove safety tag.

Out Of Service Tags

Whenever equipment is found to be faulty or unsafe and operation or use of that equipment
would cause further damage or create hazards, then the equipment must be tagged “OUT OF
SERVICE”

It is the responsibility of an employee who becomes aware of faulty or unsafe equipment to place
the Out of Service tag on it, fill in his or her name, number, equipment, date and brief description
of the fault. Before any maintenance or work is undertaken, place your Out of Service and/or
Danger tag on every isolating switch or valve. Out of Service tags must be securely fastened in a
prominent position on the equipment. Generally the isolation point is preferred although ignition
point of the machinery is acceptable.

An Out of Service tag is only to be removed from a piece of equipment by the person effecting
repairs or replacement, or by a supervisor. Removal should only occur when that person or
supervisor is satisfied that the equipment is in a useable and safe condition.

Out of Service tags are not to be used in place of Personal Danger tags. The Out of Service
tags are designed for the removal of equipment from service pending repair or replacement.

Where work on equipment bearing an Out of Service tag is incomplete at the end of the shift, the
tag is to be left in place until the work is completed.

Danger Tags

A Personal Danger Tag must be attached to the main isolating switch of equipment, whenever
danger to persons could arise from the operation of the machinery and/or use of faulty

equipment.

Prior to starting any job where the operation of a switch, valve, machine etc, will put you in
danger, you must place your Personal Danger Tag on every main isolating switch; valve etc.
making sure that the switch or valve etc is in the correct position. Ensure that the correct switch
has been tagged and verify that the isolation is effective before commencing work. Talk to your
supervisor if you have doubts about tagging correctly.

All Personal Danger Tags must be in place prior to starting a job and must contain your name,
date, equipment, valve etc on each tag, printed in ink. Do not write on Personal Danger Tags in
pencil.

When two or more persons are working on the same job, each person must sign and place a
Personal Danger Tag on each machine, main isolating switch, valve etc. Fasten Personal
Danger Tags securely so that they do not become detached.

Personal Danger Tags must be removed whenever you leave the job (Out of Service Tags may
remain in place). If you are not on the job, neither is your tag.

The only person permitted to remove a Personal Danger Tag is the person whose name is in the
tag

Should a person fail to remove a Personal Danger tag prior to leaving the worksite and the
equipment to which it is attached is required for use, the person immediately in charge of the
plant and the work group must, if unable to locate or contact that person, verify beyond doubt
that the person is not in, on or about the plant, prior to authorising reactivating of the equipment

Where work on equipment is not completed by the end of a working shift and the equipment is
required to remain isolated, arrangements must be made for Out of Service tags to be placed on
each isolating point prior to the removal of Personal Danger tags
