The first experience for a medical student in a hospital is very transcendental,
because it gives us the opportunity to empathize with patients of different conditions
and social classes, acquire knowledge and experience to provide a quality service
when we will be professionals. As a medical student the first time you attend a
hospital nobody tells you what to do, you start learning by observation. It is difficult
to learn by this method and it is not the most adequate but in a hospitable
environment there are not many opportunities to teach properly. There are three
things that we had to overcome: empathize with our patients, shut up when
necessary and apply the whole theory in practice. Being these, the only pearls that
we deduce are necessary for a medical student.

The doctors need to have empathy with their patients, this is defined as the intention
to understand feelings and emotions, trying to experience in an objective and rational
way what another individual feels that is applied in medical practice asking questions
about how was their day, how they feel and how they are considered in your position,
in such a way that the relationship is not only professional, it must also involve our
values. For example, empathy towards the patient can be demonstrated with simple
actions such as: saying hello, introducing oneself and asking the patient what is
happening to them, showing calmness and listen carefully, try to resolve your
concerns explaining in an intelligible way, without technicalities and show a sense of
responsibility1. Finally, apply these rules: communication, understanding and trust
will help us to arrive in a better way at a correct clinical diagnosis.

It is important to silence when necessary, this means that in different situations it is

better for the doctor to refrain from saying the comments that affect the patient, the
doctor must measure his words so as not to damage the patient's susceptibility. It
also refers to keeping silent about the confidential information provided by the
patient. For example, one should speak about the time of life before a serious illness,
based on statistical data but telling the patient that his case could be particular or
different from other people, nothing can be guaranteed because medicine is very
unpredictable and regarding the confidentiality of the information, this is an obligation
of the doctor stipulated in Article 4 of the rules of medical conduct: The doctor must
keep secret from third parties about what he or she had known explicitly or implicitly,
directly or indirectly, about of the illness, private life or privacy of those who would
attend or examine in the exercise of their profession and keep silent about it at all
times, even after the death of the patient2. This confirms the importance of keeping
silence when necessary.

Apply the whole theory in practice is to apply the knowledge mostly theoretical
acquired by the long years of university study and postgraduate in hospital practice;
with the patients and, thus, systematically develop an empirical disciplinary
knowledge and increase our experience3. It is necessary to think differently in
relation to a problem, to look for different solutions. For example, the
pathophysiology of the disease studied in detail in the theoretical form must be
correlated in practice by looking for determining parameters such as laboratory
values, the diagnostic process learned theoretically to be applied to patients in our
country must be modified in a practical way so that it adapts correctly. The treatment
learned theoretically must be molded to the economic condition; the accessibility of
our patients, that is to say in practice, their application is modified according to
different conditions. All these examples detailed to us that the problems of a patient
are particularly to be taken into account of the specific characteristics of their case
to satisfactorily implement the theory in practice.

In conclusion, the experiences within the hospital environment help us in our training
as students and future health professionals. Every professional must develop good
observation and empathy. The first experiences in the hospital for a doctor are very
valuable because they are the basis of a good health professional, the experiences
help to be better every day.
