DEED OF RELEASE WAIVER AND QUITCLAIM

I, ________________________the Complainant in
_____________________________entitled: In the matter of the Request for Conciliation
Filed by _____________________vs._____________________________________, Filipino and
of legal age, a resident of _________________________________________, under oath, hereby
depose and state:

1. I am the Complainant in the foregoing case for actual illegal dismissal with
prayer for payment of unpaid salary, 13th month pay, and service charge pay, with
claim for moral and exemplary damages and attorney’s fees against
______________________________, represented by Mr/Ms. _____________________

2. I, _________________________, with the assistance of SEAD Officer

________________________, decided to amicably settle my case in the total amount of
Thirty Five Thousand Three Hundred Twenty- Six and Eighty Eight Centavos
(Php 35,326.88), received from the Company, and thereby waive and forever
abandon my claim for actual illegal dismissal with prayer for payment of unpaid
salary, 13th month pay, and service charge pay, with claim for moral and exemplary
damages and attorney’s fees.

3. I now hereby release, remise and forever discharge

________________________________________________, their subsidiaries, affiliates,
stockholders, officers, directors, employees or agents from any action for sum of
money, damages, claims and demands whatsoever, which in law or in equity I ever
had, now have, or which I, my heirs, successors and assigns hereafter may have upon
or by reason of any matter, cause or thing whatsoever, the intention being to
completely and absolutely release___________________________________________,
their subsidiaries, affiliates, stockholders, officers, directors, employees or agents
from all liabilities arising wholly or partially from my employment with
________________.

4. I further warrant that I will institute no further action against

stockholders, officers, directors, employees or agents. I also manifest that the
payment made by ________________________________________________________ of
the foregoing sum of money shall not be taken by me, my heirs or assigns as
confession and/or admission of liability on the part

subsidiaries, affiliates, stockholders, officers, directors, employees or agents for any
matter, cause, demand or damages I may have against any or all of them.
 5. I acknowledge that I have received all amounts and benefits that are due

acknowledged that during the entire period of my employment

received and was paid all wages and salaries, compensations, commission, benefits
and privileges to which I was entitled under all laws, policies of and agreements with

of such employment; and if I hereafter be found in any manner to have been entitled
to any amount, the above consideration is a full and final satisfaction of any and all
such undisclosed claims.

6. I further undertake and agree that I will not publish or circulate any malicious

subsidiaries, affiliates, stockholders, officers, directors, employees or agents or other
management personnel.

7. I finally declare that I have read this document and have fully understood its
contents, which has been fully explained to me in English and in Filipino by
Honorable Labor. I further declare that I voluntarily and willingly executed this Deed
of Release, Waiver and Quitclaim with full knowledge of my rights under the law.
