Preparation as Requested

Social Case Study Report


ABOUT THE SERVICES: Social Case Study Reports are required by
charitable institutions, government hospitals and non-
government organization that provide services to clients and
patients.

Step 1 >> 5 minutes
Write your name and purpose of the visit on a client logbook.
Submit requirement. Submit yourself to an interview and give
all the necessary information and cooperation during the
interview.

MSWDO Staff instructs client to fill-in the logbook. Advised


schedule of interview.

Step 2 >> 5 minutes


Fill-out the General Intake Sheet

MSWDO Staff accomplishes the general Intake Sheet

Step 3 >> 30 minutes


Submit to an interview and provide the necessary and pertinent
information.
Social Welfare Officer II conducts interview and determines if
the case requires a home visit for further validation.

Step 4 >> 3 hours


If home visit is not necessary, wait for the preparation of
the social case study report. Secure a copy of the case study
for presentation to the agency concerned.

If home visit is not necessary, Social Welfare Officer II


prepares the Case Study Report. Provides the client a copy.

Step 5 >> half to 1 day


If home visit is necessary, provide a sketch of the location
of your house and take note of the schedule.

Social Welfare Officer II Schedules the home visit with the


client.

Step 6 >>
Extend the necessary assistance during the home visit.

Social Welfare Officer II visits the residence to check


client’s status and to further determine condition. Advises
the client of the schedule of release of the Social Case Study
Report.

Step 7 >> 10 minutes


Go to the MSWDO on the schedule date of release to secure
social case study report. Sign logbook.

Social Welfare Officer II releases case study.

Provision of Certificate of
Indigency
ABOUT THE SERVICES: A certificate of indigency is required to
avail of the services of charitable institutions, government
and non-government organizations and institutions and for
legal assistance.

CLIENT GROUPS: Indigent individuals/families.


• Barangay certification of Residency
and indigency
REQUIREMENTS: • Certification from the Municipal
Assessor’s Office that the party
concerned doesn’t own real property.
Mondays to Fridays 8:00 AM to 12: NN,
SERVICE SCHEDULE:
1:00 PM to 5:00 PM
TOTAL PROCESSING
40 minutes
TIME:

Submit requirement.

Day Care Worker I briefs client on the service and its


requirements.

Step 2 >> 15 minutes


Submit yourself to an interview. Cooperate and give all the
necessary information.

Day Care Worker I and Social Worker Assistant conducts an


interview.

Step 3 >> 10 minutes


Wait while the MSWDO Staff prepares the Certificate of
Indigency.

Day Care Worker I and Social Worker Assistant prepares


certificate of Indigency.

Step 4 >> 10 minutes


Secure the certificate of indigency and sign logbook. Submit
the same to the agency concerned.

Department Head issues certificate of indigency.

Provision of Disaster Relief


Assistance
ABOUT THE SERVICES: The Municipal Social Welfare and
Development Office is at the foremost of relief assistance
during natural or manmade calamities such as typhoon, fires,
and earthquakes. Among other, it distributes relief goods and
provide housing materials to families whose houses have been
destroyed by calamities. It also provides financial assistance
and referrals to concerned agencies. 4 Camp Management
(Evacuation Centers).

DAY CARE WORKER I briefs client of the service and its


requirements.

Step 2 >> 30 minutes


Submit to an interview. Give the necessary information and
wait as the attending personnel accomplish the AICS form.

DAY CARE WORKER II conducts interview and accomplishes the


AICS form.

Step 3 >> 5 minutes


Sign the AICS form.

secures the other necessary signatures.

Step 4 >> 15 minutes


Receive the necessary assistance referral. Sign logbook.

MSWDO Staff provides assistance:


a. For medical assistance: refers client to the pharmacy.
b. For burial and financial assistance: prepares a
disbursement voucher and refers client to the Municipal
Treasurer’s office for the release of financial assistance.
c. For the transportation assistance: prepares request to
transport companies for the provision of free transportation
services.
d. For referrals prepares the necessary papers and forward to
the individual /agencies concerned for funding and releases of
assistance.
Provision of Solo Parent
Identification Card
ABOUT THE SERVICES: The MSWDO is the lead agency in the
implementation of R.A. 8972 of the Solo Parent Act and is
tasked to issue the Solo Parent ID.
Holders of a solo parent ID Card are entitled to programs and
services offered by various government entities.

CLIENT GROUPS: Solo Parents 18 – 59 years old


• Brgy. Certification of
Residency/Indigency
• Pictures
1. (2×2) – 1 copy
2. (1×1) – 1 copy
• Income Tax Return or any document
that will establish income level of

• Death Certificate of Spouse – in case

home visit/monitoring. Take note of schedule of home visit.

MSWDO Staff conducts an interview and schedules with the


client.

Step 3 >> 1 hour


Extend the necessary assistance during the home visit. Take
note of schedule of release of Solo Parent ID.

MSWDO conducts home visit assess condition. Advises schedule


of release of the Solo Parent ID.

Step 4 >> 10 minutes


Proceed to the MSWDO on the scheduled date of release and
secure the solo parent ID. Sign logbook.

MSWDO Staff releases solo parent ID.

Provisions of Assistance in
Crisis Situation (AICS)
ABOUT THE SERVICES: The Municipal Social Welfare & Development
Office extends emergency financial assistance and referrals to
hospitals, charitable institutions and other agencies to
indigent individuals and families. This service covers burial
and medical assistance. It also covers food allowance for
indigent’s transportation allowance for individuals who are
stranded in the municipal.
CLIENT GROUPS: Indigent individuals and families.
For Burial Assistance
• Photocopy of death certificate
(claimants should be any immediate
member of the family of the deceased.)
• Funeral Contract
• Certificate of Indigency
For Medical Assistance
• Doctor’s prescription
REQUIREMENTS:
• Medical certificate or Medical
Abstract
• Certificate of Indigency
Food and Transportation Assistance
• Police Blotter, in case client is a
victim of pickpockets.
• Certificate of Indigency from the
Barangay, in case of food assistance.
SERVICE SCHEDULE: Monday to Friday 8:00 AM to 5:00 PM
TOTAL PROCESSING TIME: 1 hour
TOTAL FEES/CHARGES: NONE
Step 1 >> 40 minutes
Write your name and purpose of the visit on a client logbook.
Submit requirements. Submit yourself to an interview and give
all the necessary information.

Day Care Worker I, Social Welfare Aide accomplishes the


general Intake Sheet and AICS form to enable client to avail
of the assistance requested.

Step 2 >> 10 minutes


Sign the General Intake Sheet and Certificate of Eligibility
form and wait for the MSWDO staff to secure other signatures
needed.

MSWDO Staff secures other signatures needed to complete the


required form.
Step 3 >> 10 minutes
Receive the assistance and sign logbook.

Treasury/MSWDO provides assistance:


a. For medical assistance: refers client to the pharmacy.
b. For burial and financial assistance: prepares a
