I, RODEL VIVAS ACUNIN, of legal age, Filipino, residing at P-3 Del Carmen
Talisay, Cam. Norte, declare under oath that:

1. I am a duly licensed driver in accordance with pertinent Land

Transportation laws, rules and regulations, and was issued a
corresponding Driver’s License.

2. I used to put the said driver’s license inside my wallet.

3. Sometime in December 2018, after I rode a bus, I notice that my wallet

was missing which unfortunately all my identification card including my
driver’s license was kept.

4. Despite earnest and vigorous efforts exerted on my part to find the said
driver’s license, the same remains to be missing and can no longer be
found.

5. I am executing this affidavit to attest to the truth of all the foregoing and
for whatever legal intents and purposes this may serve.

AFFIANT FURTHER SAYETH NAUGHT.
