Description with Timeline Sequence:

(1) Truck no. RDA-2701 entered at site on 11:45 AM with an empty ISO Tank. All the security
measure including the physical inspection was carried out before for entering the site.
(2) The Truck with ISO Tank was parked at ISO yard. At 02:30 PM the truck was parked at loading
point T-210 in order to load the Pyrolysis Fuel Oil (PFO) from the Top filling point in ISO Tank.
(3) Weldon Pump (Pneumatic Pump) was used for the loading of PFO into the ISO Tank.
(4) Operator connected the grounding cable to the truck and went to the Top Loading Point to
open the lid of the ISO Tank and to put the Hose from T-210 in order to load the PFO. He fixes
the hose into the lid, and tell the other operator to open the valve of the Tank. After opening
the valve and confirmed to each other the operator at the Top set aside from the ISO Tank
and the other operator was reaching to him on stairs.
(5) Nearly two minutes passed the oil was loading from T-210 to ISO Tank from top, suddenly a
ball of fire came with loud sound and spread immediately. The operator at the Top point fell
down with the pressure of sound and fire. He received some burning injuries on his back, arms
and face. He was hospitalized by his direct employer (GEMS contractor)
(6) Operator on the stair close the air valve immediately and tried to close the oil hose valve but
was unable to close due to high intensity of fire and escaped from the area.
(7) The fire spread immediately all around the vehicle and was getting more intense as the oil
supply valve was open and continuously burning.
(8) The flames were getting higher due to the continuous supply of oil from the open valve of the
T-210

Immediate Action taken:

(1) On hearing the blast all staff came outside from the admin building.
(2) Emergency alarm was activated from firefighting control panel.
(3) All Alarms were activated at site and all people evacuate safely.
(4) GEMS personnel tried to extinguish the fire with our existing fire-fighting system but were
unable to handle it because of its capacity and intensity.
(5) Immediately Civil Defense was called.
(6) After 20 minutes the civil Defense arrived at the site and took over the charge to extinguish
the fire which they couldn’t.
(7) Site Manager arrived at the site at 03:30PM, he arranged the personnel for the closing of T-
210 supply Valve including Ram Pukar (Operation Foreman), Moemin (Security Supervisor)
and Hassan Ali (Civil defense). It took almost 30 minutes to arrange the crew and to locate
the Valve for the Tank and to accomplish it by taking all safety measure for the crew. Ram
Pukar with Hassan Ali went to the Tank Farm inside and locate the valve and close it. It took
them 15 minutes to close the valve. After closing the valve, the supply of the fuel ended and
with in 20 minutes the fire was extinguished completely.

Possible immediate Causes:

(1) Possible Iso-thermic reaction of PFO with possible residue contents of ISO Tank (more accurate
immediate cause will be identified after thorough investigation)
(2) Escalating Factors and Root Causes will be Identified and clearly stated after thorough investigation
