Chapter 1: Introduction

Purpose

This manual provides requirements for the Duo 2™ Multi Mode Mattress System
normal operation and maintenance. It also includes a parts list (in chapter 5) for
ordering replacement components.

Audience
This manual is intended to be used by facility authorised maintenance personnel

only. Failure to observe this restriction can result in severe injury to people and
serious damage to equipment.

This service manual contains seven chapters. 3
Chapter 1: Introduction
In addition to a brief description of this service manual, chapter 1 also provides a
product overview.

Chapter 2: Troubleshooting Procedures


Repair analysis procedures are contained in this chapter. These procedures are
used to gather information, identify the maintenance need, and verify the

Since the development of Clinitron® Air Fluidised Therapy in the 1960’s,
Hill-Rom has been developing and manufacturing a range of products for the
prevention and treatment of pressure ulcers and other complications associated
with immobility.
Duo 2™ Multi Mode Mattress System is the latest innovation to be introduced.
5 For the first time two clinically accepted therapies are combined in a clinically
proven system.
At the touch of a button, you can select either alternating low pressure therapy or
continuous low pressure therapy to suit the changing clinical needs of your patient.
Whichever mode you select, you can be confident that the therapeutic mattress is
automatically regulated at the lowest possible support pressure. This is due to the

6 patented Deteq™ intelligent pressure control system, which constantly senses and
adapts cushion pressures to each individual patient. So whatever the size, weight
or bed position of your patients, you are giving them the most comfortable and
effective therapy available, automatically.
Our prevalence survey experience with over 50,000 patients across Europe shows
that the patient’s heels are particularly at risk of skin breakdown (31% of patients).
Duo 2™ Multi Mode Mattress System incorporates RealHeel™, an ultra low

7 pressure heel zone in both continuous and alternating therapy modes, designed to
treat as well as prevent skin damage in this extremely vulnerable area.

Introduction

All of these advanced functions are controlled by the most compact, lightweight
and easy to use control pendant. Also incorporated are maximum inflation and

transport modes, alarm silence, and CPR rapid deflation, all without the need for
obtrusive air hose attachments.
Duo 2™ Multi Mode Mattress System offers a double layer multi mode support
surface which incorporates a special low pressure zone (the last 8 cushions) for the
patient’s heels. The double layer support surface is 25 cm deep and has a fully
enclosed cover which is moisture vapour permeable. The material possesses a two
way stretch ability helping towards the reduction of "hammocking" and can be

safely disinfected between patients.
Deteq™ technology also provides an inbuilt alarm that indicates when the patient
is close to touching the sub-mattress. Should this happen, the patient may be too
heavy for example (greater than 150 kg), the backrest should be lowered until the
alarm stops.
Beneath the mattress is an under mattress composed of 90 mm (4") layer of foam. 3

Enclosed Cover
The enclosed cover is made of Protex™ fabric which has low friction, stretches
two ways, and is breathable.
3 The cover contains an additive that is bacteriostatic, fungistatic and antimicrobial.
It can be wiped down and washed.

1 SHOCK HAZARD:
Unplug the unit from its power source. Failure to do so could result in
personal injury or equipment damage.

2 SHOCK HAZARD:
Do not expose the unit to excessive moisture that would allow for liquid
pooling. Personal injury or equipment damage could occur.

CAUTION:
Following prolonged storage of the mattress, it is advisable to select
maximum inflation mode to allow the mattress to expand its cushions fully.

3 Allow the mattress to run in maximum inflation mode for two minutes then
select the therapy mode you wish. Failure to do so could result in
incomplete inflation of the mattress.

CAUTION:
The air filter door will not close if the lever is not returned to the horizontal
position. Damage to the door or its clasps may occur if it is forced.

4 CAUTION:
Do not use harsh cleansers/detergents such as scouring pads and heavy
duty grease removers or solvents such as acetone. Equipment damage
could occur.

5 CAUTION:
The control pendant is not protected against ingress of water. Disconnect it
before cleaning. Failure to do so could result in equipment damage.

CAUTION:
Avoid the use of cleaning solutions containing Dimethyl formamide,
Tetrahydrofuran, Phenol, Cresol or concentrated inorganic acids, as these

6 chemicals will damage the cushion fabric.

CAUTION:
Avoid highly alkaline and strong organic based detergents and
disinfectants, as these chemicals may attack the cushion fabric dye,
causing transfer of colour.

CAUTION:

Do not use hypochlorite (bleach) above a concentration of 500 ppm as this
will discolour and weaken the fabric. Failure to do so could result in
equipment damage.

CAUTION:
The mattress is protected against a jet of water from a standard nozzle of
12.5 mm diameter at a distance 2.5 m to 3.0 m, with a delivery rate 100

l/min, spraying the mattress from all practical directions. Check the
capability of the cleaning equipment prior to using it, otherwise damage to
the mattress may occur.

CAUTION:
Do not use phenolics on the Protex™ cover. This could result in damage to
equipment and loss of waterproof qualities.

CAUTION:

Mattress connections under the therapeutic mattress must be sealed (see
“Tool and Supply Requirements” on page 6-15) prior to laundry. Failure to

Getting Started

WARNING:
Only facility-authorised maintenance personnel should troubleshoot the
Duo 2™ Multi Mode Mattress System. Troubleshooting by unauthorised
personnel could result in personal injury or equipment damage.

Begin each procedure in this chapter with step 1. Follow the sequence outlined

(each step assumes the previous step has been completed). In each step, the normal
operation of the product can be confirmed by answering Yes or No to the
statement. Your response will lead to another step in the procedure, a repair
analysis procedure (RAP), or a component replacement. If more than one
component is listed, replace them in the given order. If the problem still exists,
replace the next component listed, and try to operate the system again.

Start with Initial Actions to begin gathering information about the problem.

Perform the Function Checks to isolate or identify a problem and to verify the
repair after completing each corrective action (replacing or adjusting a part,
seating a connector, etc.).

When reading the voltage, the first test point is for the RED (+) digital multi-meter
(DMM) lead, and the second entry is for the BLACK (-) DMM lead. If no second
entry exists, the BLACK DMM lead must be connected to any good power supply

ground or zero volt test point, TP 3, on the logic P.C. board (see figure 3-3 on page
3-5).

All AC and DC voltage measurements have a 10% tolerance unless otherwise


specified.

Perform the Final Actions after the Function Checks to verify the repair. 5

This analysis assumes that the system functions correctly but the alarm does not
sound or the LED does not illuminate when the audible alarm is silenced.

1. Perform the following:


a. Switch the system off.
b. Place the pendant on a protective sheet face down.
c. Using a small phillips head screwdriver, remove the four screws from the
corners of the back (A) (see Figure 2-8 on page 2-20).

d. Separate the two halves of the pendant.

Subsystem Theory

The operation of the Duo 2™ Multi Mode Mattress System can be divided into
three categories or subsystems:
• Mechanical
• Electrical
• Pneumatic 2
The combination of functions in these three subsystems provides the operational
capabilities of the system. To perform all operational functions, these three
subsystems interact with each other.

Each of these subsystems are described independently in the following sections.


Basic diagrams of the electrical and pneumatic subsystems are provided along
with a functional description. 3

1 Mechanical
The mechanical subsystem is composed of the vacuum mode valve.

Vacuum Mode Valve

The handle of the vacuum mode valve is located behind the air filter door. It is

2 used to quickly deflate the cushions to store the system. The handle opens the
mechanical valve inside the air supply enclosure to let the air pass from the
manifold directly to the blower by the venturi effect. When the mattress is empty,
the handle must be returned to the normal operating position because the air filter
door is designed to remain open if the handle is not in the normal operating
position.

1 Power Supply P.C. Board

All major systems are protected by fuses.

The fuses are located:


• At input to mains input filter
• At input of rectifiers for the +12 volt rail
2 • At output of solid state relays for the blower and compressor

A rechargeable battery remembers the last status of the system and provides power
for CPR mode in case of power disconnection.

Logic P.C. Board

3 The heart of the system is the unique control logic circuitry which provides
optimum patient support therapy. The Deteq™ module sensor uses induction
between aluminium and an energised coil. As the aluminium foil is moved closer
or further away from the sensor coil, the impedance of the coil varies. This is used
as the feedback circuit to control the blower/pump output.

By calibrating the equilibrium position of the aluminium foil in relation to the


sensor coil, as well as monitoring the correct air volume in the mattress, the servo

4 control always optimises patient support regardless of patient weight or position


(within maximum weight limitations).

Control Pendant P.C. Board

The control pendant operates as the main control of Duo 2™ Multi Mode Mattress
System while showing feedback to the operator of system status and problems.

5 The control pendant allows the operator to select the following modes:
• Continuous therapy mode
• Alternating therapy mode
• Maximum inflation mode

The CPR/STOP button is fitted and, when activated, electrically opens all solenoid
valves for approximately 45 s to assist in deflating the mattress.
6 The control pendant is designed to allow the user to operate it in two different
ways when power is disconnected: to differentiate between transport mode and
power off.

Transport/Stand By Mode

Transport mode is engaged by unplugging the system from its power source. The
7 mattress remains inflated for approximately two hours, and the 12VDC battery

powers the logic circuit and the control pendant until the system is plugged into an
appropriate power source.

Power Off

Power off is achieved by the following sequence:

1. Press the CPR button.

2. Unplug the system from its power source. 2


Alarms (Audible and Visual)

When the alarm on the control pendant activates, there are a number of different
causes which are shown below.

Sleep Surface Bottoming Alarm Causes:


• Insufficient amount of air is entering mattress.

• Air hose becomes disconnected or broken.
• Large cuts or tears exist in the cushions.

Sleep Surface Deteq™ Module Sensor Out of Regulation Alarm Causes:


• Servo control malfunctions.
• Coil sensor is disconnected. 4
• Aluminium foil is damaged, folded, or destroyed.

Power Status Alarm Causes:


• Power fails.
• Power cable is damaged.
• Mains cable is inadvertently unplugged. 5
> 15 Minutes Time Out Alarm Causes:

If the mattress is not able to achieve correct inflation (some causes are: cushion
leaks, hose disconnections, manifold test port caps not secured, compressor not
operating correctly), the servo control continuously drives the compressor to try to
compensate for these problems. After 15 minutes of continuous compressor
operation, the audible/visual alarm will be activated. In all cases, the alarm may be
silenced for 15 minutes.

Pneumatic 1
Air enters the air supply enclosure (ASE) through the air filter. The air filter
removes all airborne particles greater than 30 microns. The air travels directly into
the blower and, to the compressor inlet (see figure 3-9 on page 3-10).

On exiting the blower, the air travels directly into the main manifold and from the
compressor, through a non return valve, into the main manifold.

At the air output on each manifold, there are pressure test ports for calibration

purposes.

A small axial fan pushes air into the therapeutic mattress encasement to minimize
any buildup in humidity and ensure that air is continuously changed in the
therapeutic mattress.

1 The therapeutic cushions are composed of a series of 13 air cushions, none with air
vents. The top cushion at the head end of the therapeutic mattress is not used in
alternating therapy mode. They are at the same air pressure as the Deteq™
module. Twelve cushions are used for the alternating therapy mode. The heel zone
comprises eight cushions none with air vents. The heel zone air is taken through
the main manifold to the heel manifold, the air in the heel zone is regulated by the
heel manifold solenoid valves.

2 The Deteq™ aluminium foil is located between the therapeutic mattress and the
Deteq™ module, and is held in place by two lateral fabric strips fitted with snap
fasteners.

Opening the ASE

Tools required: 3 mm Allen™ hex key

1. Working on the patient left of the mattress, unclip the therapeutic mattress flap
from the Deteq™ module.

2. Expose the two quick connect couplings at the foot left side of the heel
module.

3. Press the releases on the quick connect couplings and release the two hose
connections (A) (see figure 4-1 on page 4-3).

4. Unzip the heel module coverlet on the patient left to expose the main six
mattress connections and the two air vent connections.

5. Press the releases on the quick connect couplings, and release the six hose
connections.

6. Fold back the therapeutic mattress and the heel module cover to fully expose
the ASE.

7. Separate the two Velcro® straps that secure the ASE to the base of the heel
module.

SHOCK HAZARD:
Voltage is present on exposed electrical connectors and P.C. board when
power is on. Use care when working close to connections. Failure to do so
could result in personal injury or equipment damage.

8. Using a 3 mm Allen™ hex key, loosen the eight screws (B) securing the lid of
the ASE (see figure 4-2 on page 4-4).

9. Lift away the lid of the ASE, and store safely with the eight screws.

Closing the ASE

6 1. Perform the opening procedure in reverse order.

2. To ensure proper operation of the Duo 2™ Multi Mode Mattress System,


perform “Function Checks” on page 2-5.

2. Expose the two quick connect couplings (A) at the foot left side of the heel
module; press the releases on the quick connect couplings, and release the two
hose connections (see figure 4-20 on page 4-26).

3. Unzip the heel module cover on the patient left to expose the main six mattress

4. Press the releases on the quick connect couplings, and release the six hose

11. Repeat steps 5 to 10 for the remaining 3 parts of the alternating cycle.

12. When complete, remove the manometer and seal the test port and remove the
diagnostics connection and fit the test connector cover.

NOTE:
Dispose of the battery in accordance with local regulations. 2
Perform the Pre-Installation Maintenance (PIM) procedure each time the Duo 2™
Multi Mode Mattress System is removed from a facility. The purpose of the PIM is
to ensure that the system is in optimum condition for the next patient.

1. Follow the steps on the PIM form, filling it out as the PIM is performed.

2. After all steps are complete, sign the form. 3


3. Follow service location policy for recording PIMs.

4. Once the form is completed, signed, and recorded, file it according to current
service location policy.

1 6.2 Routine Maintenance

WARNING:
Adhere to the Infection Control Policies and Procedures from Hill-Rom.
Failure to do so could result in the spread of infection.

At appropriate intervals for the patient’s condition or at the caregiver’s request,


perform the Routine Maintenance (RM) procedure. RM ensures the system is
disinfected and clean during patient use.

1. Contact the customer to advise of your approximate arrival time. Ask that the
customer arrange to remove the patient from the unit during the service time
(approximately 15 minutes).

3 2. Dispose of the customer linen according to the caregiver’s instructions.

3. Wipe down the air cushion, control pendant, and cable with an approved or
hospital-approved disinfectant.

4. Perform “Function Checks” on page 2-5.

5. After you complete all the above steps, follow the service location policy to

4 record completion of the RM procedure.

Transportation
Place the folded Duo 2™ Multi Mode Mattress System in the transport case.

Setup

WARNING:

1. Perform the setup procedures prior to patient placement.

3 2. Remove the regular mattress from the frame if one is present.

3. Place the Duo 2™ Multi Mode Mattress System at the head end of the frame
(A) (see figure 6-1 on page 6-6).

4. Unlock the buckle (B), and with one person on each side holding the handle
(C) for foot end, lift, move, and place at the foot end of the frame
(see figure 6-2 on page 6-7).

7 5. Secure with the two nylon straps (D), ensuring that the strap placement does
not interfere with the operation of the bed functions (head-up or hilow).

6. Secure the three side snaps (E) of the therapeutic mattress (F) to their matching
Deteq™ module (G) snaps and close the zippers (H) on each side of the
Deteq™ module (see figure 6-3 on page 6-7).

1 7. Remove the control pendant from its protective location and attach it to the
foot board of the frame using the universal clamp (I) (see figure 6-4 on page
6-8).

8. Check that the power cord is not damaged.

CAUTION:

5 Following prolonged storage of the mattress, it is advisable to select


maximum inflation mode to allow the mattress to expand its cushions
fully. Allow the mattress to run in maximum inflation mode for two
minutes then select the therapy mode you wish. Failure to do so could
result in incomplete inflation of the mattress.

9. Plug the system into an appropriate power source, and use the Velcro® cable
straps to attach the cable to non-moving parts of the frame, ensuring that it will

6 not become trapped or trail on the floor. The mattress will start to inflate
immediately once power is applied.

10. Initially a small blower will start and, even with the patient lying on the
mattress, rapidly inflates the surface. The patient will be well supported after
only one minute, then the blower will shut down and a small silent air pump
will take over.

11. In all cases, the patient's sacrum should be centred on the mattress adjacent to
the Deteq™ logo (it is printed on each side of the mattress cover).

12. The control pendant will display the status of the system continuously.

1. Place the system into continuous therapy mode on the control pendant.

WARNING:
Ensure the frame brake is on to avoid unplanned frame movements.

3. Open the filter door (A) at the right side of the ASE. Turn the vacuum valve
lever (B) one quarter turn counterclockwise (see figure 6-5 on page 6-9).

4. Press firmly on the heel zone to open the heel manifold solenoid valves.

5. The air escapes from the mattress which will be empty in approximately seven
to ten minutes.

1 6. When the mattress is empty, turn the vacuum valve knob to the horizontal
position.

CAUTION:
The air filter door will not close if the lever is not returned to the horizontal
position. Damage to the door or its clasps may occur if it is forced.

7. Close the air filter door and fasten it into place with the two clasps.

8. Re-snap the door flap.

9. Place the power cord on top of the mattress at the head end.

10. Take the control pendant, and place it between the two layers of the mattress.

3 NOTE:
Whilst the packing procedure can be done by one person, we recommend using
two people.

11. Perform the following:


a. One person should stand on each side of the foot end and take hold of the
handles.

4 b. Lift the foot section up and fold the mattress towards the head in a “Z”
shape.
c. Wrap the packing strap around the mattress, lock the buckles, and tighten
securely together.

WARNING:
Follow the product manufacturer’s instructions. Failure to do so could result
in personal injury or equipment damage.

SHOCK HAZARD:

Unplug the unit from its power source. Failure to do so could result in
personal injury or equipment damage.

SHOCK HAZARD:
Do not expose the unit to excessive moisture that would allow for liquid
pooling. Personal injury or equipment damage could occur. 3
CAUTION:
Do not use harsh cleansers/detergents such as scouring pads and heavy
duty grease removers or solvents such as acetone. Equipment damage
could occur.

CAUTION:

The control pendant is not protected against ingress of water. Disconnect it
before cleaning. Failure to do so could result in equipment damage.

CAUTION:
Avoid the use of cleaning solutions containing Dimethyl formamide,
Tetrahydrofuran, Phenol, Cresol or concentrated inorganic acids, as these 5
chemicals will damage the cushion fabric.

NOTE:
These chemicals are not normally found in standard hospital cleaning solutions.

CAUTION:
Avoid highly alkaline and strong organic based detergents and 6
disinfectants, as these chemicals may attack the cushion fabric dye,
causing transfer of colour.

Clean the unit with a lightly dampened cloth and ordinary disinfectants.

6.5 Cleaning

1 General Cleaning
We recommend that you clean the unit with detergent and warm water. Do not use
excessive liquid or harsh cleansers.

Steam Cleaning
Do not use any steam cleaning device on the unit. Excessive moisture can damage
mechanisms in this unit.

Cleaning Hard to Clean Spots


To remove difficult spots or stains, we recommend that you use standard
household cleansers and a soft bristle brush. To loosen heavy, dried-on soil, you
may first need to saturate the spot.

3 Disinfecting

CAUTION:
Do not use hypochlorite (bleach) above a concentration of 500 ppm as this
will discolour and weaken the fabric. Failure to do so could result in
equipment damage.

4 1. When there is visible soilage and also between patient use, we recommend that
you disinfect the unit using a tuberculocidal disinfectant.

CAUTION:
The control pendant is not protected against ingress of water. Disconnect it
before cleaning. Failure to do so could result in equipment damage.

5 CAUTION:
The mattress is protected against a jet of water from a standard nozzle of
12.5 mm diameter at a distance 2.5 m to 3.0 m, with a delivery rate 100
l/min, spraying the mattress from all practical directions. Check the
capability of the cleaning equipment prior to using it, otherwise damage to
the mattress may occur.

6 2. Dilute the disinfectant as specified on the manufacturer's label and follow the
manufacturer’s instructions for “wetting” times for hard surface disinfection.

CAUTION:
Do not use phenolics on the Protex™ cover. This could result in damage to
equipment and loss of waterproof qualities.

7 3. Remove the soiled Protex™ cover from the mattress.

Handle the sodium hypochlorite (bleach) with care. Dispose of the sodium
hypochlorite in accordance with the proper disposal procedure as specified
by the local regulating authority. Failure to do so could result in personal
injury. 3
CAUTION:
Do not use hypochlorite (bleach) above a concentration of 500 ppm as this
will discolour and weaken the fabric.

CAUTION:
Mattress connections under the therapeutic mattress must be sealed (see

“Tool and Supply Requirements” on page 6-15) prior to laundry. Failure to
do so could result in equipment damage.

NOTE:
8 air outlet plugs (F02071) must be used to seal the mattress.

1. Mattress connectors on the underside of the therapeutic mattress must be


connected together during the wash cycle.

2. We recommend that mattress cushions & Protex™ covers be laundered at 67°C
(153°F) for a minimum of 10 minutes.

CAUTION:
Ensure the mattress is completely dry before storage. Failure to do so
could result in equipment damage.

3. Ensure adequate time for the Protex™ cover to completely dry.

CAUTION:
Do not mangle the Protex™ cover. Equipment damage could result.

Do not use heat above 67°C (153°F) as this may damage the urethane
lining and heat seals of the cushion.

4. Air dry or machine dry the Protex™ cover separately at temperatures up to


67°C (153°F).

5. Remove the Protex™ cover promptly from the dryer.

6. Zip the Protex™ cover onto the mattress.

7.1 Seat Cushion
The seat cushion is an option available with the Duo 2™ Multi Mode Mattress
System.

Installation 2
1. Perform the following:
a. Place the seat cushion on the patient's chair, and secure the seat cushion by
using the strap provided (A) (see figure 7-1 on page 7-3).

b. Open the Velcro® flaps (B) on the seat cushion, disconnect and extend the
hose (C).

c. Lift the cover (D) of the unit at the foot left end to reveal the heel zone
access zipper (E).

d. Open the zipper (E).
e. Locate the seat cushion outlet (F) (see figure 7-2 on page 7-4).

f. Insert the seat cushion connector (G); an audible click will confirm

2. Select P.Max on the control pendant, and silence the alarm.

3. When the seat cushion is firmly inflated, perform the following:


a. Press the seat cushion connector release (H) and pull the seat cushion
connector (G) from the seat cushion outlet (F).

5 b. Place the patient on the seat cushion.


c. A hissing sound will be heard; when the sound stops, the seat cushion is
correctly inflated.
d. Connect the seat cushion connector (I) to the seat cushion check valve (J)
(see figure 7-3 on page 7-5).

1. To deflate the seat cushion, pull apart the covers at the Velcro® joint on the
side adjacent to the hose cover.

2. Locate the blue deflate valve (L), and twist the cap (K) a ½ turn counter-
clockwise to open (see figure 7-4 on page 7-6).

3. Fold the cushion from the opposite side to expel the air; then return the cap to
the sealed position, fasten the Velcro® joint, and store.
