USE OF PRP THERAPY IN SKIN and HAIR FOLLICLE REJUVENATION
Platelet rich plasma (PRP) application is a successful
catalyst in the healing process for wide varieties of
conditions. PRP was first developed in the 1970s and
first used in an open heart surgery procedure. PRP
therapy began gaining popularity in the mid 1990s.
Methods of harvesting and using platelet rich plasma are
becoming widely practiced in the fields of sports
medicine, orthopedics, traumatology, general surgery,
dentistry, dermatology and cosmetology.
Their main function is hemostasis by attaching to any
breach in vessel where platelets aggregate, activate and
release granules. The platelets organelle zone is rich in
platelet granules. Alpha granules contain clotting
mediators such as factor V, factor VIII, fibrinogen,
fibronectin, platelet-derived growth factors, and
chemotactic agents. Delta granules, or dense bodies,
contain ADP, calcium, serotonin, which are platelet-
activating mediators. Activated platelets release
granules by exocytosis which contain not only clotting
factors but more than sixty different biologically active
substances that are involved in processes of tissue
regeneration, angiogenesis, epithelialization,
proliferation and differentiation of osteoblasts, and in
synthesis of collagen and extracellular matrix of
connective tissue.
Use of PRP in skin Rejuvenation :
Skin aging is a unpreventable, irreversible process which
is influenced by both intrinsic and extrinsic factors. ROS
damage, fragmentation of collagen and elastin,
degradation of extracellular matrix are few of the
intrinsic factors. Different growth factors in PRP
including PGDF, TGF, VEGF and IGF stimulate dermal
fibroblasts thus leading to deposition of new collagen.
PRP works better than many other therapies based on
inducing mild inflammatory response.

Use in Androgenetic Alopecia and Alopecia
Areata:
EGF and FGF in PRP activate the proliferation and
trans-differentiation of hair follicle stem cells and
product new follicular units or rejuvenate dormant
follicles. PRP has mitogenic as well as antiapototic
effect and prolong survival of dermal papillae.
PRP as standalone or as an adjunct to surgical
procedures (along with follicular transplantation)
holds promising results. PRP is injected as inter-
follicular region as 0.1 ml/sq cm area. Follicles
ready for transplantation can be dipped for 15 min
before transplantation to improve survival and take
rate.
PRP Product to be used:
Concentrated PRP preparations (typically 7-10
times concentration of platelets) are used in these
conditions without any added stimulant. Quantity
required varies depending on condition and 1-3 ml
PRP may be needed for injection.
What to expect:
Initially after injection slight discomfort or increase
in pain is expected for first 1-3 days as injected
platelets give some inflammatory response. It
subsides rapidly and may require mild analgesics
initially.
The injected platelets release growth factors over
next one week and rapid reversal of original
symptoms.
