1. INTRODUCTION

TPG/UTS Kent, through implementation of an advanced Health, Safety and


Environmental Protection management system in line with GASCO HSE Policy (refer
Appendix 5 for list of GASCO Procedures and their location) strives to establish a risk
free, safety oriented working environment, protecting the health of its work force and
keeping the environment, pollution free to its best efforts and protecting the
equipment.

This system shall be enforced through self-implementation by allocating clear and


dedicated individual responsibilities, setting performance challenges and by
implementing day to day advanced techniques and monitoring its effectiveness.

This HSE management plan is directly applicable to every TPG / UTS Kent employee
assigned to work on GASCO DCS project. It will also be binding on any
subcontractor employed by TPG / UTS Kent on this project. Finally our vision is
simple i.e., within the area of TPG / UTS Kent’s operation or wherever we have
influence, we will try ourselves and contribute to others our best, to achieve

♦ no accidents

♦ no harm to people and equipment

♦ no harm to the environment

Starting from detail design and engineering, mobilisation, site office and fabrication
shop establishments, transportation to project execution, the system shall ensure that
safety rules and procedures are in place, communicated, implemented, documented
and enforced for site related activities including vehicle operation.

The following are the primary items which are covered under this HSE Management
Plan and Procedures.

During Design / Engineering phase the HSE aspects shall also be considered through
the following :

1. Hazard and Operability study (for process control modifications)

2. Internal safety review of documents to ensure suitable HSE considerations have


been given.

Hazard and Operability (HAZOP) study is a formal systematic examination of the


process and engineering intentions for facilities to assess potential of mal-operation
or malfunctioning individual items of equipment and the consequential effects on the
facility as a whole.

The procedure for conducting Hazard and Operability HAZOP study shall be issued
separately for this project.

2. LOCAL LEGISLATION AND INTERNATIONAL STANDARDS

TPG / UTS KENT and its activities fully conform with the following United Arab
Emirates Laws and Regulations :

Abu Dhabi Emirates Law No. 8 of the year 1978.

Ministry of Labour and Social Affairs “Federal Law No. 8 of the year 1980”

Ministry of Labour and Social Affairs “Ministerial Order No.(32) of the Year 1982”

TPG / UTS KENT in its Planning, Designing, Procurement, Installation and Operation
adopts the following international standards: -

ISO 9000 standards

BS 5750 - The British version of the ISO 9000.

ISM Code - International Safety Management Code.

API RP 75 - Recommended Practice for Development of Safety and Environmental


Management Program.

Other Recognised International HSE Management Systems.

3. PRELIMINARY REQUIREMENTS

3.1 SITE ENTRY

TPG/ UTS Kent shall obtain written permission from GASCO before the work starts
on site and before the material deliveries prior to commencement of work.

The TPG/ UTS Kent employees (which covers all contractor / sub-contractor
employees engaged for this project) shall possess valid security pass at all times
while they are on GASCO premises. TPG / UTS Kent shall ensure that employee’s
access is limited to project Work Areas. For this purpose the GASCO-Ruwais Areas
classification map as annexed in Appendix-4 shall be followed.

Only vehicles that possess a valid security pass shall be allowed to enter and
transport TPG/ UTS Kent employees on to GASCO premises and concession area.

The TPG/ UTS Kent employees and its sub contractors’ employees shall at all times
act in a courteous manner when dealing with site security and shall co-operate fully
with any instructions given to them.

3.2 PRELIMINARY SAFETY INDUCTION BY GASCO

All TPG/ UTS Kent employees and its sub contractors’ employees shall undergo the
Safety induction imparted by GASCO Safety Department. TPG/UTS Kent Safety
Officer will maintain a record of the same. The induction enables the employees to
be familiar with the classification of plant areas, general rules and regulations to be
followed, emergency telephone nos. to be contacted on an unusual event, minimum
requirement of PPEs (Personal Protective Equipment) to be used, GASCO Ruwais
Emergency Procedure and briefing of GASCO Permit to work system. (Refer
Appendix 5 for list of GASCO Procedures and their location).

4. SITE OFFICE SAFETY PROCEDURE

TPG / UTS Kent management shall insist and implement certain safety guidelines in
accordance with GASCO safety rules and regulations in mobilising, setup and
maintaining of its site office and fabrication facilities.

4.1 MOBILISATION AND ERECTION OF TPG / UTS KENT OFFICE AND


FABRICATION WORKSHOPS

Prior to mobilisation, approval will be obtained from GASCO authorities regarding


location and permission for setting up the facilities. TPG / UTS Kent will implement all
safe method of loading, transportation, unloading and setting up of its office facilities
and fabrication shops as detailed in section 4 & 7.24.

4.2 SAFETY DEVICES

The TPG / UTS Kent office will be furnished with the following Safety devices.

♦ Battery back up smoke detectors in each room and locations as per area
specification.

♦ Common alarm system with manual call point at two exit doors (normal and
emergency) locations with battery back up.

♦ One emergency exit door for easy evacuation of inmates in case of an


emergency.

♦ Emergency exit door signs with battery back up. to be pictorial type.

♦ CO2 fire extinguishers for emergency use in the office

♦ CO2 and ABC Dry powder extinguishers in the workshop area.

♦ Main incoming supply will be controlled by an ELCB with suitable RCD.

4.3 GENERAL INFORMATION

TPG / UTS Kent will display the following :

♦ GASCO HSE Policy Statement.

♦ TPG / UTS Kent HSE Policy Statement.

♦ Office set up plan and escape route single line diagram.

♦ Emergency contact numbers ( Main Control Room (MCR), Fire Station, Hospital,
Ambulance, GASCO Project Manager)

4.4 FIRST AID

Necessary first aid boxes will be available with site safety officer. An updated record
will be maintained and periodical replenishing of the items will be done by him.

4.5 TOILET AND DRINKING WATER FACILITIES

TPG / UTS Kent will provide sufficient toilet and drinking water facilities to its
employees and will be responsible for hygiene.

4.6 EMERGENCY EVACUATION EXERCISES

TPG / UTS Kent Project Manager with Safety Officer will conduct uninformed
periodical emergency evacuation exercises to keep the inmates of the office
employees familiar about the system. GASCO Safety Superintendent (ORS or
ORS/I) shall be informed in advance.

4.7 OFFICE SETUP PLAN

An updated drawing of the office setup will be provided to GASCO for reference and
approval.

5. LEADERSHIP AND RESPONSIBILITIES

5.1 LEADERSHIP

5.1.1 LINE MANAGEMENT RESPONSIBILITIES

TPG / UTS KENT Management recognises that the responsibility for safety is not only
with the safety department but also that it lies with Line Management from General
Manger to every level of the operation. TPG / UTS KENT will clearly define all
responsibilities and accountability of all management personnel in their job
descriptions. The Management Organisation encourages a two way flow of
information and ideas so that all personnel understand that their input is necessary.

Management and Supervisors will demonstrate personal commitment and proactive


approach to Health, Safety and Environmental Protection Program by good example
and by reacting promptly and effectively to :

♦ Poor HSE performance / excellent HSE performance


♦ Lack of standards of HSE practices
♦ Lack of adequate HSE program, plans and objectives
♦ Unsafe working conditions and practices

♦ Inadequate training or instructions

5.2 RESPONSIBILITIES

Every manager, supervisor, employee, subcontractor employee, etc. of TPG / UTS


KENT has a responsibility to implement the HSE Policy within his own area of
responsibility. The following are the prime HSE responsibilities for various positions
within the Company organization :

5.2.1 GENERAL MANAGER RESPONSIBILITY

The General Manager is ultimately responsible for the establishment of the Company
HSE Policy, HSE Manual and Project Health, Safety and Environmental Protection
Plans.

The General Manager shall :

♦ Ensure that there is an effective HSE Protection Policy within the Company.
♦ Periodically appraise the effectiveness of the HSE Management System and
ensure that any necessary changes are made.

♦ Through the Project Manager and support services ensure that project specific
Health, Safety & Environmental Protection Programs are established,
implemented and maintained.
♦ Ensure that tenders are adequate to cover sound and safe methods of Health,
Safety and Environmental Policy.

5.2.2 FINANCIAL CONTROLLER RESPONSIBILITY

The Financial Controller shall :

♦ Review absences due to accident, injury or ill health and ensure that occurrences
are notified to the relevant authorities.
♦ Arrange for all insurable risks throughout the organisation to be assessed in
relation to the Health and Safety of its employees, loss or damage to company
property and risks to the public and the environment through its activities.

♦ Ensure that adequate arrangements have been made to cover financial liabilities
on HSE matters.
♦ Review insurance and claims record periodically and make any necessary
changes.

♦ Provide a financial review on Safety, Health and Environmental Protection to be


included in the annual account.

5.2.3 PROJECT MANAGER RESPONSIBILITY

The Project Manager shall :

♦ Ensure that all Employees are fully aware of and briefed on the TPG/UTS Kent
HSE Management Plan and Procedures and ensure that the Procedure is issued
to the Employees.

♦ Acquire a full and accurate knowledge and understanding of the Company HSE
Policy and ensure that all sub –ordinates appreciate their responsibilities.

♦ Be involved at the planning stage with work practices / methods and ensure that
tenders make adequate allowances to comply with the HSE Policy.

♦ Delegate a clear responsibility for Safety, Health & Environmental Protection to


all, including subcontractors, through liaison, planning and co-ordination.

♦ Review and implement the HSE recommendations following site inspection,


accident investigation and environmental hazard inspection to be satisfied that
they are correct and appropriate.

♦ Make him or any sub–ordinate available to accompany the Safety Officer on site
safety inspections or accident investigations.

♦ Ensure compliance with statutory regulations that all site registers and accident
investigation reports are kept up to date and available at all times.

♦ Decide in conjunction with operations how best to deal with existing, temporary
and future safety services.

♦ Liaise with Safety Officer to organise basic fire precaution, prevention and
emergency procedure.

♦ Inform the Safety Officer of all new or additional work so as to anticipate hazards,
recommend precautions and instruct all in relevant procedures.

♦ Make arrangements to ensure that all visitors to site are aware of and comply with
the HSE standards.

♦ Ensure that all subcontractors and their employees are made aware of and
comply with the organisation’s HSE policy by giving a copy of the Company HSE
Management System to each and obtain a signature, indicating acceptance.

5.2.4 CONSTRUCTION MANAGER RESPONSIBILITY

See Responsibilities as indicated for Project Manager.

5.2.5 SAFETY ENGINEER / SAFETY OFFICER RESPONSIBILITY

The Safety Engineer / Safety Officer is responsible for the implementation and
direction of the HSE Program reporting directly to the Project Manager.

He shall be approved ‘’Safety Authorised Signatory’’ by GASCO Safety Suptd. after


thorough familarisation of site safety procedures and practices and shall closely liase
with GASCO Safety Dept.

He shall be responsible to advice the Project Manager on :

♦ Prevention of injury to personnel and damage to plant and equipment.


♦ Further improvements in existing working methods.
♦ Applications of TPG / UTS KENT and Client HSE Procedures and Regulations.

♦ Suitability from a safety viewpoint of new / hired plant & equipment and the
validity of all appropriate test certificates.

♦ Suitability of Personnel Protective Equipment.


♦ Awareness of potential hazards on new projects or work activities before work
commences.
♦ Provide the site / work area HSE organisation and fire precautions required.
♦ Carry out surveys and inspections on a regular basis to ensure that safe working
practices are in operation.

♦ Supervise the recording and analysis of information on injuries, damage and


production loss, assess accident trends and review overall safety performances.
♦ Assist in the training of employees to promote awareness of HSE policy.
♦ Ensure strict adherence to Work Permit Procedures .and adequate safety
coverage.

♦ Ensure compliance with HSE Policy.


♦ Co-ordinate incident / accident reporting including first aid, rescue, evacuation,
investigation and reporting.
♦ Liaise with the site supervisors in hazard identification and the establishment of
preventive measures.

♦ Maintain and audit fire protection and fire fighting equipment.

5.2.6 ENGINEER / SUPERVISOR RESPONSIBILITY

The Engineer / Supervisor shall be responsible to :

♦ Organize the site and work places so as to ensure that work is carried out to the
required standard, with minimum risk to men, equipment and materials and to
ensure that the employees under his responsibility do not engage in unsafe work
practices or take unnecessary risks.

♦ Know the requirements of relevant HSE regulations and COMPANY specific


requirements.

♦ Give all employees precise instructions on their responsibilities for correct working
methods.

♦ Arrange delivery and stacking of materials to avoid increasing the risks of double
handling; position plant effectively; ensure that electrical supplies are safely
maintained.

♦ Plan and maintain a tidy site / work area, segregate waste for the removal of
debris daily.

♦ Implement arrangements with sub-contractors and others to avoid any confusion


about areas of responsibility.

♦ Check that all machinery and plant are maintained in good condition and report
any defects immediately.

♦ Ensure that adequate PPE is issued to each person and that equipment is
properly used.

♦ Ensure that all persons under his control know what to do in the event of an
accident, fire or other emergency.

♦ Arrange to investigate all accidents promptly to discover their cause and eliminate
recurrence. Accident Reports shall be completed by the Supervisor, in conjunction
with the Safety Officer.

♦ Liaise with the Project Managers on matters of safety and, where necessary,
consult the Safety Officer.

♦ Continually develop safe practices in his section to ensure maximum safety for all
under his supervision.

♦ Ensure strict adherence to Work Permit Procedures.

♦ Ensure that every employee is fully informed of job hazards he may expect to
encounter.

♦ Ensure that all injured employees receive immediate medical treatment.

♦ Conduct weekly tool box meetings.

5.2.7 FOREMAN / CHARGE HAND RESPONSIBILITY

The Foreman / Charge Hand carries the same HSE responsibilities as the supervisor
for the employees under his direct control. In addition the Foreman / Charge Hand
shall be responsible to :

♦ Incorporate safety instruction in routine order and to see that they are obeyed.

♦ Prevent men from taking unnecessary risks.

♦ Ensure that all new employees understand the TPG / UTS KENT and Company
HSE Regulations and standards for work.

♦ Discourage horseplay and reprimand those who fail to consider their own well
being and that of others around them.

♦ Report defects in plant and equipment promptly to his Supervisor.

♦ Ensure strict adherence to Work Permit Procedures.

♦ Ensure that tasks are allocated to workers only in accordance with the worker’s
capabilities and qualifications.

5.2.8 ALL EMPLOYEES RESPONSIBILITY

Each employee shall be responsible to:

♦ Use the correct materials, tools, equipment and methods for the work he is
carrying out.

♦ Ensure that all PPE provided is used and maintained in good condition.

♦ Report all defects in plant and equipment immediately to his foreman or


supervisor.

♦ Develop a personal concern for himself and others around him.

♦ Avoid improvisation, which entails unnecessary risk.

♦ Refrain from horseplay and the abuse of facilities.

♦ Observe all HSE rules at all times.

♦ Report all unsafe conditions to the foreman or supervisor.

♦ Caution any fellow employee whom he observes to be working in an unsafe


manner.

♦ Promptly report any injury, accident or incident to his foreman, supervisor or the
Safety Officer.

♦ Be familiar with emergency procedures.

6. TRANSPORT SAFETY PROCEDURE

6.1 INTRODUCTION

The following rules and conditions are designed so as to ensure that vehicles that are
used by TPG / UTS Kent are fit for purpose and maintained to the highest standards,
and work practices are so designed as to ensure the safety of all vehicle drivers and
passengers. GASCO Transportation Policy for Contractors shall be followed in
totality. (Refer Appendix 5 for list of GASCO Procedures and their location).

6.2 OPERATION

The TPG / UTS Kent management shall ensure that all operators/drivers under their
control shall comply with the following :

♦ U. A. E. Traffic Regulations and GASCO safe driving rules shall be observed at


all times such as valid U. A. E. driving/operator license, security pass etc.

♦ Personnel will not drive when unfit to do so for medical reasons or as a result of
fatigue or intoxication.

♦ All posted road signs including speed restriction and temporary signs relating to
works or diversions will be respected and complied with.

♦ GASCO policy with respect to seat belts shall be complied with.

♦ Passengers will only be carried in the passenger compartment of vehicle.

♦ The number of persons carried in the passenger compartment of vehicle will not
exceed the maximum number stated on the vehicle registration document, and
side of the vehicle.

♦ “No Passenger” signs will be fixed on vehicles where there is no provisions for
passengers as stated on the vehicle registration document.

♦ All drivers will wear coveralls, safety footwear and as and when indicated by the
GASCO representative other such safety apparel which shall comply with British
Standard Specification or equivalent.

♦ Personnel will not remain on a vehicle that is being loaded.

♦ Floors of vehicles/cabs shall be kept tidy, free from oil and trip hazards.

♦ Personnel will not mount or dismount from a moving vehicle, and will use the
proper hand/foot holds.

♦ Where adjustments are required to be made to the vehicle by the driver, these
shall not be made with the engine running, unless unavoidable, e.g. tick-over
adjustment.

♦ Unattended vehicles will be left on firm level ground, in neutral, handbrake


engaged, engine switched off and key in place. Vehicles should be parked in
designated parking area where available and in safe location on the plant roads

♦ Before reversing, a check will be made that the rear of the vehicle is clear. If the
rear view is obstructed, the driver will only continue with the aid of a
banksman/flagman.

outside the plant.

Standards

TPG/UTS Kent will maintain the following general standards and ensure that all
vehicles comply fully with the same.

♦ All vehicles will be of adequate capacity and of a design suitable for the work to
which they are allocated.

♦ All vehicles will comply with the GCC construction standards and TPG/UTS Kent
will ensure that all vehicles under TPG/UTS Kent control are fully insured and
currently registered in accordance with the U. A. E. Traffic Regulations.

♦ All vehicles will be fitted with 3 point interia reel seatbelts in the front and rear
outer seats and waist seat belts in the middle, for the maximum capacity of
personnel carried.

♦ All light vehicles and buses will be fitted with high intensity rear lights.

♦ Appropriate tyres will be fitted to all light and medium vehicles. All tyres will be of
the same size, ply rating tread pattern, profile and in good condition with a
minimum of 2mm tread depth.

♦ All semi-trailer vehicle will be fitted with side restraints for containing long loads,
e.g. casings and tubulars.

♦ All rigid type vehicle with a box body with drop sides and tail gate, hinges and
securing pins will be fitted.

♦ A dry Powder fire extinguisher will be fitted in each vehicle.

♦ TPG/UTS Kent will provide fully equipped first aid boxes in each vehicle.

♦ All TPG/UTS Kent vehicles will be identified by sticking the company stickers
probably on both front doors and the passenger carrying capacity as per
registration will be marked.

♦ All vehicle will carry a reflective safety triangle to be used is the event of a
breakdown or accident.

♦ A spare tyre, jack and tool box for the particular vehicle will be provided in each
vehicle.

♦ Heavy duty vehicles will be fitted with audible reversing alarms.

♦ Spark arresters shall be fitted on the exhaust system and auxiliary equipment,
such as pumps powered by diesel or gasoline, of vehicles entering GASCO
Restricted Areas.

♦ All diesel engines to be fitted with auto shut-off at air intake.

Inspection/Maintenance

♦ TPG/UTS Kent will ensure that all vehicles under their control are serviced and
maintained in accordance with the manufactures recommendations.

♦ TPG/UTS Kent shall provide copies of insurance, service and maintenance


records to the GASCO REPRESENTATIVE and keep the original documents for
inspection as and when requested by the GASCO REPRESENTATIVE.

Safety Check

On a weekly basis, the TPG/UTS Kent operators / drivers will undertake the following
safety check for items as appropriate.

♦ Water

♦ Oil

♦ Tyre pressures and condition

♦ Wheel nuts ( Check for loose wheel nuts)

♦ Efficiency of brakes and steering

♦ Efficiency of lights / horn/ reversing light/ reversing alarm/ direction indicator /


flashing beacons/ windscreen wipers/ washers, etc. ( as appropriate).

♦ Batteries – secureness, cleanliness, corrosion free, and electrolyte condition.

♦ Defects, damage, or other conditions considered by the driver/operator as being


likely to make operation of the vehicle unsafe, will be reported by the
driver/operator to his immediate Supervisor who will arrange the necessary
repairs etc., prior to use.

♦ All daily / weekly inspection reports will be maintained for inspection / auditing by
GASCO authorities.

6.3 TRANSPORTATION OF PERSONNEL AND MATERIAL

TPG/UTS Kent will follow strictly the present ‘ LOCAL TRANSPORT RULES AND
REGULATIONS’ and GASCO transport safety regulations in transporting Personnel
and Material such as;

♦ Use of safety belt will be strictly implemented.

♦ No. of passengers travelling will not exceed the rated capacity.


♦ No fuels will be carried in the vehicles meant for transporting passengers.

♦ All loads to its rated load will be properly secured to load carrying vehicles as per
Safety Regulations.

♦ Pilot vehicles or banksman will be engaged to transport heavy vehicles especially


inside plant to avoid accidents.

♦ No personnel will be transported along with loads.

Vehicle Loading

♦ Vehicle loads will not exceed the weight limitations indicated on the vehicle
registration document.

♦ Any load which extends beyond the rear of the vehicle in which it is being carried
will be marked with a red flag or warning system approved by GASCO.
♦ All vehicle loads will be fully secured. Loose tools or equipment will not be carried
inside the passenger compartment of any vehicle. Such items will be secured in
position, placed behind a protective screen or secured in the load bed of the
vehicle.

♦ Rollbars will not be used to secure loads.


♦ Loads will not be carried suspended from the ‘A’ frames of trucks.

♦ Fuel will not be transported inside the passenger compartment of any vehicle.
♦ Fuel containers will be purpose built and secured in designated holders mounted
on the outside of the vehicle or inside the load bed of a pick-up ( not one designed
to carry passengers).

Towing

♦ Towing of plant equipment will only be conducted by vehicles fitted with


manufacturer approved towing fitments.

♦ Any towed equipment which exceeds the width of the prime mover will be fitted
with indicators and brake lights powered by the prime mover.

♦ The weight of the equipment being towed will not exceed the weight of the towing
vehicle.

Training

♦ TPG/UTS Kent will ensure that all drivers/operators under their control hold a
current full U.A.E. driving / operators license appropriate to the vehicle being
driven/ operated.

♦ TPG/UTS Kent will provide copies of the drivers/ operators driving license/training
certificates to the GASCO REPRESENTATIVE and keep the original documents
for inspection by the GASCO REPRESENTATIVE as and when requested.

♦ Periodical and specific induction and training will be given to drivers / operators in
driving, operation, emergency evacuation procedures and especially how to
behave inside GASCO plant as per TPG/UTS Kent and GASCO Safety rules and
regulations.

6.4 PROHIBITION OF UNSAFE PRACTICES

Any wilful violations of statutory or GASCO Safe Driving rules by the TPG/UTS Kent
personnel whilst on GASCO premises or on GASCO related business will result in the
immediate removal of any negligent driver/ operator and the replacement thereof with
another person acceptable to GASCO.

7. SITE SAFETY PROCEDURE

7.1 HSE QUALIFIED STAFF

TPG / UTS KENT employs qualified and trained staff who will monitor, supervise and
assess the implementation of Health, Safety and Environmental Protection Policy in
all the company activities. In most of the cases this task will be carried out by the
Safety Officer with the help of selected Supervisors and Technicians under the direct
control of the Project Manager. The team will be in full co-ordination with GASCO
HSE Management to gather information, communicate among company employees,
assess the performance of Company Health, Safety and Environmental Protection
Policy Management System and make periodical corrections.

7.2 MOTIVATION AND COMMUNICATION

The key element that ensures implementation of the Health, Safety and
Environmental Protection Policy is the effective and organised communication system
from the General Manager to the employees. TPG / UTS KENT considers the key
elements for communication is the introduction of the Policy from the Management
level to the Company task force, induction and training, group meetings, tool box
talks, risk analyzing, outcome of investigations, lessons learned from accidents,
incidents etc.

7.3 WELFARE STANDARDS

TPG / UTS KENT will take maximum care in maintaining local regulations / GASCO
regulation guide lines in providing the following welfare steps for its employees.

7.3.1 Employees shall be provided with GASCO approved accommodation.

7.3.2 Transportation of employees as per transport safety regulations

7.4 SUBCONTRACTORS SAFETY

TPG / UTS KENT’s subcontractors shall be contractually bound to comply with all the
HSE requirements as mentioned in this HSE Plan. The TPG / UTS KENT Project
Manager has the responsibility to enforce the subcontractor’s compliance with the
HSE requirements and the Safety Officer is responsible to monitor the
subcontractor’s compliance and HSE performance. All HSE inspections shall include
the inspection of subcontractor’s works, facilities etc., as applied to TPG / UTS
KENT’s own activities.

Any portion of work to be subcontracted shall be specified prior to commencement of


the works.

7.5.1 SELECTION OF PERSONNEL

TPG / UTS KENT in selecting its personnel for the designated projects will take
cognisance of the following elements.

7.5.1.1 QUALIFICATION

Employees shall be holders of recognised and attested licenses, trade certificates or


engineering degrees as applicable and appropriate.

7.5.1.2 EXPERIENCE

Employees should have a minimum period of experience in the respective trade.

7.5.2 SELECTION OF MATERIALS AND EQUIPMENT

TPG / UTS KENT will conform to GASCO approved standards, where available, in
selection of materials and equipment for the project.

7.6 AUDITING

TPG / UTS KENT Management has formulated auditing systems on following.

7.6.1 INTERNAL AUDITING

This will be done by TPG / UTS KENT Senior Management with the assistance of
Safety Engineer / Officer. Audits will be documented in the approved standard format.
Corrective actions will be carried out as soon as reasonably practical without delay
unless impractical depending upon the risk involved.

7.6.2 AUDITING BY GASCO

A structured monitoring, audit and review programme will be agreed with GASCO.

The audit will be designed to assess the key elements of TPG / UTS KENT HSE
Management system.

TPG / UTS KENT and GASCO will satisfy themselves that adequate systems are in
place to implement corrective actions identified by monitoring /auditing activities.

Scope and Responsibility : Supervisory role of TPG / UTS KENT & GASCO Job
Officer.

Auditing philosophy : Frequency and standards required.

Review scope and responsibility : Various stages of the contract to be considered.

Follow-up on corrective actions by allocated responsible personnel for the respective


action.

Both parties will agree on :

Type of audit.

Designation of authorities and audit frequency.

7.7 PERMIT TO WORK

To obtain the maximum safety of personnel, equipment and environmental protection,


TPG / UTS KENT will in its activities comply with all the GASCO Permit to work
system. (Refer Appendix 5 for list of GASCO Procedures and their location). Every
activity of TPG / UTS KENT, irrespective of its nature will be covered with a Cold
Work, Hot Work, Entry and Excavation permit system. TPG / UTS KENT will
nominate qualified supervisors for work permit signatory training. Each operation or
task will be covered by a suitable permit which will be enclosed in permit boxes and
demonstrated near the work area. The TPG / UTS KENT Safety Officer / Area
Supervisor will make periodical checks on permits to ensure its activities are covered
by suitable valid permits.

All Permits and procedures shall be strictly in accordance with GASCO Permit to
Work Procedure and GASCO Plant Area Classification and Permit to Work
Requirements. The procedure is based upon Main Permits and Complimentary
Permits. Complimentary Permits will not allow work to be carried out without a Main
Permit.

Those Procedures and this document are designed to ensure the safety of operating
locations and employees. The Permit to Work System objectives are to :

♦ Provide formal records of handover of plant/equipment between authorities and to


record permissions to carry out specific work in specific areas.

♦ To emphasize the hazards involved and to define necessary precautions

♦ To communicate the scheduling of work to responsible and authorised individuals


and to allow monitoring and control of safe working for the integrity of installations
and safety of personnel

♦ To record the completion of work and return of responsibility

♦ To identify Restricted and Safe Areas of the Plant.

7.7.1 WORK CLEARANCE

Certain specified activities may be carried out in a Restricted Area without a PTW.
Any such work can only be carried out with the prior approval of GASCO duty Shift
Controller or Shift Supervisor and after logging in the Activity log book with signature
from the Shift Controller or Shift Supervisor and the Person In Charge of Work.

7.7.2 COLD WORK PERMIT

Activities using handtools within Restricted Areas that do not involve a source or
potential source of ignition will require this Permit, unless the activity is listed as an
exclusion within GASCO Plant Area Classification and Permit to Work Requirements.
In such cases the procedure to be followed as described in paragraph 7.7.1 above.

7.7.3 EXCAVATION COMPLEMENTARY PERMITS

All excavation works whether in restricted or safe areas require this permit. This
permit does not allow work to be performed. It will only be issued together with a
Main Permit. Any Special precautions will be detailed on the Permit.

7.7.4 HOT WORK PERMITS

7.7.5 ELECTRICAL ISOLATION COMPLEMENTARY PERMITS

This Permit will be required when it is necessary to work on plant or equipment


connected to a source of electrical power. This permit does not by itself allow work to
be performed. It will only be issued together with a Main Permit. The application and
use of this type of Permit must be in conjuction with GASCO Electrical Regulations.

7.8 RISK MANAGEMENT / RISK ASSESSMENT

Before commencing any activity, the line management of TPG / UTS KENT shall
encourage and appreciate that every employee must understand and be aware of
any risk and related safety, health and environmental hazards. All such risks and
hazards will be identified and rectified before starting the job. TPG / UTS KENT will
introduce the job hazard identification system so that the same can be utilised for
assessment and evaluation for future projects. Changes to working practices, use of
new and modified plant, equipment and Personnel Protective Equipment, proper
Permit to Work system and adherence to same will prevent most of the risks involved
in any specific activity.

7.8.1 ACTIVITY RISK ASSESSMENT SYSTEM

Using the attached format, all hazards and risks involved in an activity will be
assessed. All relevant precautions will be taken to avoid accidents, incidents, near
misses and environmental hazards.

7.9 EMERGENCY PREPAREDNESS

TPG / UTS KENT operates its system in line with GASCO procedures to create
effective response to emergency situations by specific planning and training.
Emergency procedures will be tested by exercises and evacuation drills with the aid
and supervision of GASCO safety department or within TPG / UTS KENT itself. Such
procedures will be planned and agreed with GASCO. All drills and exercises will be
recorded and monitored by the Project Manager and Safety Officer.

Relevant telephone Nos. (Fire Control Station, Control Rooms, Hospital / Ambulance
etc.) will be distributed and made familiar to all employees by induction in tool box
talk, displaying in safety notice boards at designated locations of work sites,
telephone locations etc.

In emergency only the Control Room number to be used.

Emergency life saving equipment is obtained and kept ready. Before starting any job,
all employees are trained regarding the consequences and use of special equipment
and systems. Depending on the seriousness of the situation a special task team may
be formed to meet any specific requirement.

7.10 PERSONNEL PROTECTIVE EQUIPMENT

7.10.1 INTRODUCTION

Besides the proper accident prevention methods implemented to protect the injury
and possibilities of accidents, personnel protective equipment provides a barrier
between the person at risk and the potential instrument of injury.

7.10.2 GENERAL STANDARDS

TPG / UTS KENT shall adopt generally the following industrial standards of PPE
during the execution of its activity which is subject to the approval of GASCO
standards.

For eyes, ear and face protection, foot wear, hand protection, head protection /

maintain the use of PPE as per the attached guide in Annexure - 3.

Other than these general items, special PPE’s will be organised for tasks of a more
hazardous nature like :

Confined space entry


Tank cleaning
H2S contamination
Work over water etc.
Handling of asbestos type of material.

7.10.4 TRAINING

♦ TPG / UTS KENT shall ensure that their employees are trained in the proper use
of PPE, how to correctly fit and wear it and what its limitations are.

♦ Ensure employees involved in maintaining, repairing and testing the equipment


and in its selection for use are also trained.

♦ Ensure that in addition to initial training, users of PPE and others involved with the
equipment, shall be given refresher training.

♦ Keep records of all training undertaken and submit copies to GASCO when
requested.

Training shall include elements of theory as well as practice in using the equipment,
and shall be carried out in accordance with the recommendations and instructions
supplied by the PPE manufacturer and GASCO safety management.

7.10.5 THEORETICAL TRAINING

♦ An explanation of the risks present and why PPE is needed.

♦ The operation, performance and limitations of the equipment.


♦ Instructions on the selection, use and storage of PPE related to the intended use.
Written operating procedures such as permits to work involving PPE shall be
explained.
♦ Factors which can effect the protection provided by the PPE such as other
protective equipment; personal factors, working conditions, inadequate fitting,
defects, damage and wear.

♦ Recognising defects in PPE and arrangements for reporting loss or defects.

7.10.6 PRACTICAL TRAINING

♦ Practice in putting on, wearing and removing the equipment.

♦ Practice and instruction on inspection and where appropriate, testing of PPE


before use.

♦ Practice and instruction on the maintenance which can be done by the user, such
as cleaning and the replacement of certain components.

♦ Instruction on the safe storage of equipment.

7.10.7 MAINTENANCE OF PPE

TPG / UTS KENT shall :

♦ Establish a “PPE Maintenance Programme” to ensure that the equipment


continues to provide the degree of protection for which it was designed, this shall
be made available together with records of tests and examinations to the GASCO
Representative.

♦ Clearly identify the responsibility for carrying out maintenance together with the
details of procedures to be followed and their frequency.

♦ Make arrangements for cleaning and disinfecting if necessary before the PPE is
re-issued.

7.11 CERTIFICATION OF EQUIPMENT

All equipment which will be used inside GASCO restricted area will be tested and
certified according to the GASCO safety rules and regulations. The procedures will be
continued as per the duration limitations of the GASCO HSE guidelines.

The following equipment will be under a valid test certificate up to the end of the
projects :

♦ PPE’s which require a test certificate like safety belts and safety harness etc.

♦ All lifting equipment and gears

Updated records will be maintained which is subjected to the periodical inspection of


GASCO safety department.

7.12 SAFETY MEETINGS

7.12.1 INTERNAL AND GROUP MEETINGS

Monthly group meetings will be planned and conducted. All safety aspects of the
activity covering installation / operation will be discussed and all relevant points will
be recorded and reported to the client at the end of the monthly safety reports. The
main topics of discussion will be reviewed on accidents, incidents, near misses,
standard of house keeping, risk involved in the activity, emergency response /
procedures on the ongoing job.

A Line Management Team including Safety Officer and Supervisors will be attending
the group meeting. Minutes of the meeting will be taken and follow up actions /
responsible persons identification will be done. The completed actions of last month
meeting will be reviewed.

7.12.2 TOOL BOX TALK MEETING

Each Foreman and Supervisor shall every two weeks and when circumstances
dictate hold a tool box talk with his workforce. Additional tool box talks must be held
prior to commencing any new or hazardous operation to ensure the workforce are
familiarised. A standard format will be used for registering the names and signature of
the attendees, person conducting the talk, main topics discussed with follow up
actions and responsible persons involved. A representative from GASCO may be
invited to witness the talk. To promote understanding visual aids and material display
may be used.

7.12.3 MEETING BY GASCO

TPG / UTS KENT will participate in all safety review meetings of GASCO. All
accidents, incidents and near misses will be reviewed. A follow up / corrective action
procedure will be set up with a target of elimination of any unsafe practices or unsafe
conditions. Reports will be maintained.

7.13 INDUCTION, TRAINING, INFORMATION

All TPG / UTS KENT personnel engaged in the project activities will be adequately
inducted, trained and informed in Health, Safety and Environmental issues. All
personnel shall be issued with Company Safety HSE guide hand book. An updated
log will be kept for inspection and auditing.

7.13.1 INDUCTION

Prior to entry to an installation all personnel will be briefed regarding the Health,
Safety and Environmental situation and general restrictions.

7.13.2 TRAINING

All personnel will be trained as per the following requirements :

♦ General safety - Introduction to the plant, smoking, driving restrictions,

♦ Permit to work system, use of PPE etc.


♦ Breathing Apparatus

♦ Basic Fire fighting

♦ First Aid
♦ Work permit
♦ Fire Warden (stand by fire man)

All available informatory safety posters will be displayed, in yards, work shop areas,
installation sites, to keep the personnel informed regarding every field of safety

7.13.3.2 FLASH BACKS, LESSONS LEARNED

Day to day display of flash backs and lessons learned from the investigation of
accidents/ incidents and near misses will be advertised in the Safety Notice Boards.

7.13.3.3 CATALOGUES

All available catalogues, publications and information regarding HSE will be made
available and circulated.

The HSE Management Plan and procedure document shall be available with Site
Safety Officer of TPG / UTS KENT for easy reference. Also the relevant GASCO
Procedures / HSE Documents will be located at the same place.

7.13.3.4 VIDEO FACILITIES

Regular video facilities for usage of PPE, fire fighting and other safety clippings /
information will be made available to the work force in co-ordination of the client.

The following events in TPG / UTS KENT area of activity will be reported immediately
to GASCO MCR and GASCO MANAGEMENT after every occurrence. Project
Manager shall be informed immediately regarding all accidents and incidents
occurring at site. (Refer 9.3 for contact telephone numbers)

♦ Incidents - Accidents but not LTI

♦ Near Misses (To be reported in GASCO standard Simplified Near Miss Form)

Incidents will be reported in accordance with GASCO Guide to Accident and Injury

7.15 INVESTIGATION AND CORRECTION ACTION

7.15.1 ACCIDENTS – LTI (LOSS TIME INCIDENTS)

All accidents in the category of LTI will be investigated thoroughly by TPG / UTS
KENT Safety Officer / Area Supervisor in co-ordination with GASCO investigation
team and the basic reasons behind the happening will be established. The basic
causes as to who, what, why, when, where and how will be the line of investigation.
All relevant measures will be established to prevent the root causes of the accident
through :

7.15.2 INCIDENTS

Incidents will be treated with the same importance as that of LTI.

7.15.3 NEAR MISSES

Negligence of near misses is in many cases the cause of an accident or incident.


GASCO safety department encourages people to report and study all near misses
that happen during work activities. The detailed study on near misses can virtually
prevent most accidents and incidents. TPG / UTS KENT will, through its Line
Management, encourage its personnel to report all near misses. TPG / UTS KENT
will utilise the GASCO near miss reporting format.

7.15.4 OFF THE JOB ACCIDENTS

All off the job accidents will be investigated, reported and recorded.

7.15.5 VEHICLE ACCIDENTS

To prevent accidents TPG / UTS KENT will take all necessary measures including
periodical inspections, replacements, employing competent and licensed personnel
and strict disciplinary actions.

Reporting and investigation of vehicle accidents will be in compliance with the local
rules and regulations and GASCO vehicle accident reporting and investigation
procedure. In all cases regulations laid by the Police Department will be properly
observed.

7.17.2 TRAINING

TPG / UTS KENT safety officers, through tool box talks and other suitable means of

As part of the training uniformed evacuation exercises will be planned with co-
ordination of GASCO Operations and Safety department to evaluate the performance
of the TPG / UTS KENT personnel on an emergency situation. A scenario will be

7.18.1 DAILY INSPECTION

The Safety officers will conduct daily inspections of all work areas and the records will

On the event of any conditions, practices or hazards mentioned above the activity will
be stopped immediately, line management will review the situation and only after
adopting necessary measures will the activity be started again.

7.18.2 WEEKLY INSPECTIONS

All mobile plant, generators, vehicles etc. will be inspected weekly by a certified
mechanic. Weekly reports will be maintained for inspection and auditing. All mobile
plants will be fitted with spark arrestors. TPG / UTS KENT Safety Officer will produce
all electrical tools and extension leads to the GASCO electrical inspection department
to obtain a Safety Tag and Numbers before its use inside the plants. TPG / UTS
KENT Electrical Supervisor will inspect all electrical tools and extension leads on a
weekly basis. The inspection reports / follow up actions/ maintenance records will be
maintained for GASCO inspection and auditing.

TPG / UTS KENT Mechanical Supervisor will inspect all the mechanical tools weekly
and the reports / follow up actions/ maintenance records will be maintained for
GASCO inspection and auditing.

The Site Manager will conduct weekly inspection of the workshop facilities along with
the workshop Supervisor and Safety Officer. All inconsistencies will be discussed and
follow up action will be implemented strictly as per the corrective action strategy.

7.18.3 MONTHLY INSPECTION

Monthly inspections will be conducted on the following :

♦ Electrical, Mechanical and instrument tool boxes.

TPG / UTS KENT Safety Officer along with Electrical / Mechanical /


Instrumentation Supervisor / Foreman will conduct inspection. Assessment will
be done as to how technicians are maintaining their tools. Reports will be
maintained for follow up actions and auditing.

♦ Site Office Set up and Yard Facilities

The Project Manager together with the Safety Officer will conduct the inspections.
This will cover areas of site establishments, office set ups, work shop facilities,
stores etc. All items will be reviewed and these items will be discussed at the
months safety group meeting. Reports will be maintained for follow up actions
and auditing.

♦ PPE Stock and Quality.

The Project Manager together with the Safety Officer will make an assessment of
the quality and stock of PPE’s. Follow up actions will be made to replenish if any
items are out of stock. All records will be maintained.

♦ Fire fighting equipment inspection

♦ First aid box inspection and replenishment.

♦ House Keeping, Waste segregation and Disposal.

The Project manager together with the Safety Officer will inspect TPG / UTS
KENT yard facilities / work site to assess the quality of house keeping, waste
segregation and disposal methods. House Keeping rating forms will be used for
assessment. The reports will be included in the monthly safety returns. All
records will be maintained for auditing.

7.18.4 SUDDEN INSPECTION

Apart form routine inspection as above, sudden checks shall be conducted as desired
by TPG / UTS KENT, or company representatives with respect to adherence to HSE
policy. Security Personnel shall have right to search for ignition sources / flammable
materials at gate as well as project site.

7.19 WORKING AT HEIGHTS

Any activity of TPG / UTS KENT over the height of 2 mts. or more will be done in a
safe manner as discussed below.

7.19.1 ACCESS / EGRESS

TPG / UTS KENT ensure that :

♦ All access / egress to working places will be kept clear and free of obstruction.

♦ Access / egress will be installed in such a way that it will not create additional
hazards.

♦ Clear defining and marking out of access / egress will be done wherever
necessary.

7.19.2 PORTABLE LADDERS

Ladders shall be used at places when working at heights where scaffolding is not
necessary. Daily inspection and regulations of use will be implemented.

Maximum height for working is approximately 2.0 M.

7.19.2.1 INSPECTION OF LADDERS

Daily inspection on ladders will be done on the following :

♦ Missing or damaged rungs or damaged strings.

♦ Wooden ladders for cracks

♦ Hinges and locking system of split ladders

♦ Bottom rubber bushes of ladder legs missing.

♦ Ladders to be numbered for identification. Ladder register to be kept.

17.19.2.2 CARE IN USING LADDERS

Following safety regulations will be implemented and inductions will be given to


personnel who will be utilising ladders for working at heights.

♦ Safety inspection will be done by the user before the work commences.

♦ Ladders will be erected at an angle of approximately 76 deg. to horizontal in 4 feet


in height : 1 foot out of base ratio.

♦ Ladders will be placed on firm and leveled surface.

♦ Ladders will be secured properly on the top side.

♦ If securing is not possible another person will be holding and footing it at the base
level.

♦ One meter extension will be at least provided on the top side above the stepping
point.

♦ Ladders will not be used for carrying materials or tools.

♦ Only one person will be allowed to work on a ladder.

♦ Ladders with the limpet system can only be used to support a working platform.

♦ Extension ladders will have the rated overlap of rungs as rated by the safety rules.

♦ No painting will be done on wooden ladders.

7.19.3 SAFETY HARNESS

When a working platform cannot be provided fitted with guard rails and toe boards
TPG / UTS KENT will ensure that safety harness will be used where persons can fall
more than 2 mts.

All harness will be in good condition and inspected prior to use and comply with BS
1397 or equivalent standards, tested annually and inspected every 6 months.

Fixing points will be established such that persons have free movement without the
need to constantly unclip the harness. A running line will be fixed where free
movement is required.

7.20 SCAFFOLDING

7.20.1 STANDARD

TPG / UTS KENT will utilise only the following approved standard materials for its
scaffolding works.

♦ British Standard Specification No. BS 1139 : Metal Scaffolding

♦ British Standard Specification No. BS 2482 : Specification for timber scaffold


boards.

♦ British Standard Specification No. BS 5973 : Code of practice for access and
working scaffolds and special scaffold structures in steel.

7.20.2 ERECTION

All scaffolding towers will be erected by an experienced, certified and competent


scaffolder. Standard scaffolding erection rules and regulations as per the GASCO
safety guide lines will be implemented strictly.

7.20.3 INSPECTION

GASCO Scaffolding Certificate to be used.

TPG / UTS KENT Safety Officer will inspect on a daily basis all the scaffolding and
daily signing off tags will be brought into effect. Before work commences on any
scaffolding, inspection and test approval from GASCO safety department will be
obtained. ‘Ready to Use’ and ‘Not to Use’ symbols will be displayed on safe and
unsafe scaffolds respectively signed by Contractor Personnel authorised by GASCO.

7.20.4 ALTERATIONS

Any alterations to the existing approved scaffolding will only be done by a certified
scaffolder.

Inspection procedure (7.20.3) will be repeated for each alteration activity before work
on scaffolding starts.

7.20.5 SCAFFOLD ACCESS

Properly secured approved ladders will be utilised for access to scaffold working
platforms. Minimum 1m of ladder will be kept extended to the top of working platform.

7.20.6 SCAFFOLD TAGGING DETAILS

TPG / UTS KENT scaffold inspection tags will contain the following details.

♦ Date erected with name and initials of Scaffolder.

♦ Maximum loading kN/m2

♦ Date inspected with name and initials of Safety Officer.

to the GASCO Representative and keep the original documents for inspection as and
when requested by the GASCO Representative.

7.21.2.1 SAFETY CHECK

At the start of each day / shift, the operators shall undertake the following safety
check for items as appropriate.

♦ Efficiency of lights / horn / reversing alarm / direction, indicator / flashing beacons,

Defects, damage or other conditions considered by the operator as being likely to


make operation of his vehicle unsafe, shall be reported by the operator to his
immediate Supervisor who shall arrange the necessary repairs etc.

7.21.3 MACHINERY

TPG / UTS KENT shall implement the following :

♦ All dangerous parts of machinery, including non-powered machinery shall be


securely guarded or fenced.

♦ Where a fixed or interlocked guard is not possible, an automatic safety device,


which prevents the operator from coming into contact with the dangerous part of
the machine will be fixed.

♦ All guards shall be substantially constructed, properly maintained and kept in


position whilst the dangerous part is in motion or use.

♦ The machine controls shall be positioned for safe and easy access by the
operator and clearly identified.

♦ Handles, hand wheels and levers shall be positioned and spaced for safe
handling with ample clearance from other parts of the machine.

♦ Braking systems, where provided, shall be maintained in good working order.

7.21.3.1 OPERATION TRAINING

Persons operating machinery shall be trained to cover the following :

♦ for good housekeeping around the machine

♦ The use of any necessary ventilation equipment

♦ The need for adequate lighting

♦ Any electrical, pneumatic or hydraulic hazards which may exist and the
precautions to be taken.

♦ How to assess equipment condition prior to routine use.

7.22 RIGGING PROCEDURE

7.22.1 INSPECTION, TESTS AND EXAMINATIONS

TPG / UTS KENT shall ensure that the following inspections, tests and examinations
of cranes, lifting appliances, lifting gear are undertaken by a GASCO approved Third
Party Inspection Authority.

♦ All cranes shall be tested and thoroughly examined for satisfactory operation
within the previous 2 months or since the last substantial alteration or repair.

♦ After each erection of crane or lifting appliance or after adjustment of the


arrangements, the anchorage or ballasting arrangements shall be tested.

♦ The automatic safe load indicator shall be tested and examined before the crane
is put into use after erection or alteration.

♦ Pulley blocks, gin wheels etc. shall be tested and thoroughly examined before first
use and thereafter thoroughly examined every 12 months.

♦ All wire ropes, chains and lifting gear shall be tested and examined before first
use. Test and examination shall be repeated for all chains and lifting gear which
have been altered and or repaired. Additionally all ropes and lifting gear shall be
thoroughly examined every six months.

♦ All hoists shall be tested and thoroughly examined before first use and after
substantial alteration or repair, they shall also be thoroughly examined every six
months.

TPG / UTS KENT shall provide copies of all records, tests and examinations in
connection with the above to the GASCO Representative prior to lifting operations
taking place.

7.22.2 OPERATION OF CRANES

7.22.2.1 GENERAL PRECAUTIONS

For lifting procedures the following precautions will be implemented.

♦ During lifting operations no person will be positioned beneath the load, hoist or jib.
Similarly, loads will not be slewed over the heads of personnel.

♦ No lifting appliances will be left unattended with a suspended load.

♦ After completion of a lift, all slings will be hooked back to the hook to avoid
catching obstructions.

♦ If the automatic safe load indicator (ASLI) is activated during lifting operations, the
operator should not carry on with the lift and will return the load to its original
position.

♦ Where a lifting appliance has a travelling or slewing motion, a clearing of at least


1m will be provided between the lifting appliance and any adjacent fixture. If this is
not possible, the access will be barricaded off to prevent personnel access.

♦ The hoisting mechanism on a crane will only be used for direct raising or lowering
operations.

♦ Crane jips shall not be worked at a radius greater than specified on the test
certificate.

♦ All cranes will be marked clearly with their maximum safe working loads.

♦ If the lifting radius can be varied, a sign will be displayed inside the cab showing
the safe load at each radius.

♦ Except for testing purposes, lifting appliances will not be used for loads greater
than the specified safe working load.

♦ All lifting gears will be colour coded after approval by third party inspection and
testing. Any lifting gear that does not carry the agreed safety colour code will not
be used.

♦ Offloading locations shall be inspected by Person In Charge of Work (PICW) prior


to commencement of work regarding safe place for crane stationing and material
placement.

7.22.3 SIGNALING

TPG / UTS KENT will implement the following in signaling such as :

♦ A signal shall be given to crane drivers only by authorized persons i.e the
banksman who may also be known as the slinger or signaller.

♦ Hand signals shall be given strictly in accordance with the well established
operation code.

♦ The banksman shall wear a high visibility waist coat to enable the driver both to
see the signals clearly and be reassured that the person giving them is
authorised.

♦ If radio’s are to be used as a form of communication between banksman and


driver, they shall be maintained in first class condition and the person using them
shall speak clearly, distinctly and in a common agreed upon language.

♦ Bad language shall be strictly avoided when using radio’s.

♦ Once a banksman has been appointed, strict discipline shall be imposed to


ensure that no other person gives signals to the driver for any move other than
the emergency stop.

All mechanical erections related to instrumentation and electrical systems (instrument

7.23.2 WEIGHT LIFTING AND HANDLING PROCEDURE

Induction and training will be given to personnel who deal with mechanical erection
ensuring proper and safe handling.

TPG / UTS KENT will implement proper setting up of project workshop as per
GASCO guidelines.

7.24.1 DESIGNING

Before setting up the workshop, it will be properly designed to have the following

7.25 WELDING SAFETY

7.25.1 COMPETENT PERSONNEL

TPG / UTS KENT only employ certified experienced welders for its activities.

Fire watch (stand by Firemen) shall be present during all (e.g. welding) Hotwork
Category 1.

7.25.2 7.25.2 PPE PROTECTION

For Hot work Category 1, the area of work shall be covered on the sides and below
with suitable fire blankets to contain sparks, hot slugs and drops.

7.25.34 DIESEL POWERED WELDING GENERATORS

7.25 GAS WELDING

TPG / UTS KENT will implement the following safety measures while doing gas
welding such as :

♦ Ensure that gas cylinders being used will be identified.

♦ Oxygen cylinders will be stored at least 3m away from any source of fuel.

♦ Only oxygen cylinders may be stacked horizontally, fuel gases i.e. acetylene,
propane must be stored vertically (upright)

♦ Cylinders will be stored away from direct sunlight to avoid a build up of excess
internal pressure which may lead to gas leaks,

♦ Ensure checking hoses for leaks daily and that they are free from cuts, cracks and
burns or worn out patches.

♦ Hoses will be used for one type of gas only and colour coded for identification.

♦ Cylinders must be closed and hoses de-pressurised when not in use.

♦ Cylinder keys must have keys attached by chain

7.26 HAZARDOUS MATERIALS AND CHEMICALS

TPG / UTS KENT will not be handling or using hazardous materials and chemicals on
this project. If an activity arises of this nature the HSE Management of TPG / UTS
KENT will implement strictly the safety recommendation of the manufacturer after
consulting with GASCO HSE department and in accordance with GASCO Waste
Handling and Disposal Procedure. The hazardous materials may include Asbestos,
Fiber glass in the activity area.

7.27 GAS CYLINDERS

TPG / UTS KENT will implement the following rules and regulations in handling,
storage and usage of compressed gas and empty cylinders as follows:

7.27.1 COLOUR CODING

TPG / UTS KENT ensure that all compressed gas cylinders shall be of colour coding
as per BS 349C such as :

♦ Oxygen - Black

♦ Acetylene - Maroon

♦ Argan - Blue

All filled cylinders shall be examined on arrival to ensure that they are correctly colour
coded in accordance with the above BS specification, tested and date stamped within
the past two years.

If any valve is found defective on arrival it will be verified labeled defective and
returned to the supplier.

7.27.2 SAFE TRANSPORTATION

♦ Gas cylinders will be carried / transported on open vehicles from site to site and
on purpose built trolleys from location to location kept in the vertical position and
properly secured.

♦ No passengers will be allowed to travel on vehicle compartments where cylinders


are stored for transportation.

♦ Cylinders will be secured, vertical, valve shut and caps fitted.

♦ Suitable fire extinguishers will be kept on vehicles for emergency use.

♦ Cylinders will not be transported by rolling on the ground.

♦ Cylinders will not be lifted on their valves with slings or hooks.

♦ No smoking allowed on vehicles where cylinders are loaded.

♦ Drivers will be properly instructed with the safe handling, loading and transport of
cylinders.

7.27.3 STORAGE

TPG / UTS KENT will arrange separate cylinder store adjacent to the fabrication yard
and main stores in a safe area approved by GASCO safety department and the
following safe methods will be adopted.

♦ Purpose built sheds will be an open structure with shades fixed on the top side.

♦ All cylinders will be stored separately as per their colour code.

♦ Full and empty cylinders will be stored separately and indicated with ‘Full’ and
‘Empty’ notices.

♦ All cylinders will be stored vertically with a chain securing system to avoid falling.

♦ Sufficient warning signs and suitable fire extinguishers will be provided on storage
area.

7.27.4 HOSES AND TORCHES

TPG / UTS KENT will insist the following standards in using and maintaining hoses
and torches such as :

♦ Hoses of BS 5120 can only be used to connect the torch to the cylinder with a
colour coding set out as per BS 3212 which will be adopted as follows :

Blue - Oxygen

Red - Acetylene

Orange - Propane

♦ Defective connections shall on no account be used.

♦ Lengths of hoses shall be kept as short as possible.

♦ Non-return check valves will be fitted to each hose at the torch.

♦ Flash back arresters will be fitted to both fuel gas and oxygen regulators

♦ Flash back arresters will be tested and replaced in accordance with the
manufacturers instructions.

overrun by vehicles.

♦ Daily inspections prior to work start will be strictly implemented and defected
items will be replaced, not repaired.

♦ Cutting torch activity location will be a safe distance away from cylinder storage.

♦ No torches should be left burning or unattended.

♦ All valves, regulators and torches will be operated and maintained as per the strict
safe method recommended by the manufacturer.

7.28 PRESSURE TESTING

This procedure is mainly required during the commissioning stage of instrument


impulse / air lines and associated controls. TPG / UTS KENT will adopt the maximum
safe approved methods to conduct all its pressure testing procedures using
hydro(water) system, gas system and air systems (pneumatic).

7.28.1 GENERAL INFORMATION

TPG / UTS KENT will adopt the following general safe guidelines throughout the
pressure testing procedure.

♦ Testing will only be performed under the direct control of the TPG / UTS KENT
site commissioning supervisor.

♦ Only competent, qualified and experienced technicians will be engaged in the


pressure test and associated activity who will be trained and familiarised with the
hazards associated with the activity.

♦ A test procedure will be written up before commencing any pressure test activity.

♦ All test equipment will be calibrated before the pressure test procedure and the
correct rated equipment will be used for the suitable test.

♦ Calibrated pressure gauges will be fitted to equipment under test to ensure over
pressurising does not take place and de-pressurising is complete prior to opening
up.

♦ Prior to pressurising equipment to be tested it will be thoroughly examined for


isolation, drains closed, vents open / shut etc. as required.

♦ Any other activity will not be permitted to the lines or equipment under pressure
test.

♦ The maximum temperatures permitted during the test will be determined.

♦ The area of the pressure test will be safety isolated by warning tapes, barriers and
warning signs and physical watching / guarding of the area will be done to prevent
unexpected entry of non-concerned personnel.

♦ Prior to pressure testing a mechanical completion certificate will be provided so as


to be sure that all hot work activities are completed, checked and inspected as per
specification.

♦ The pressure will be applied gradually or increased in steps until the required test
pressure is reached.

♦ Air pressure will not be applied (pneumatic test) on lines where hydrocarbons are
present.

7.29 ELECTRICAL SAFETY

TPG/UTS KENT will install and maintain all its temporary electrical set ups as
follows:

7.29.1 OFFICE SET UP

Incoming supply will be controlled by suitable RCD tripping system.

♦ Sufficient sockets will be provided for computers and other equipments to avoid
use of multiple sockets.

♦ All wiring and electrical work within the Construction Sub-contractor’s compound
shall be in compliance with the IEE Wiring Regulations, 16th Edition.

♦ Monthly inspection will be planned to check the Air Conditioning units and exhaust
fans to avoid electrical fire hazard.

♦ All circuits will be properly earthed.

7.29.2 FABRICATION SHOPS AND STORES.

♦ All electrical systems will be conduit wired.

♦ All sockets and plugs will be of GASCO approved type.

♦ Monthly inspections will be carried out to check the adequacy of the systems.

♦ All circuits will be properly earthed.

♦ Each working sockets will be individually protected by 30 mA RCD.

7.29.3 EXTENSION LEADS

♦ Flexible cords, 300/500 Volt grade, rubber insulated, screened, PLP sheathed,
Colour black suiting BS 6500 – 1969 will be used for preparing extension leads.

♦ Each extension lead unit will be protected by 30 mA RCD fixed separately near to
socket side in an enclosed weather proof box.

7.29.4 INDUSTRIAL PLUGS AND SOCKETS

♦ 240V Plugs & switched socket outlets within the office areas shall be commercial
type to BS 1363. Within the workshop, industrial grade switched socket outlets of
high impact plastic construction, shall be used, to BS EN 60309.

Where mobile or portable equipment may be used in the process plant (and
consequently in hazardous areas), all such equipment (inclusive of associated
power plugs, switchgear and cables) shall be certified for use in the hazardous
area, in accordance with IEC 79.

Certified and non-certified equipment shall be clearly identified (labelled) as such,


wherever there is any danger of confusion between the two.

7.29.5 SMALL ELECTRICAL EQUIPMENT

♦ All electrical equipment like drill machines, grinding machines, cutting machines
will be of double insulation type and as per electrical standards of GASCO.

7.29.6 INSPECTION AND APPROVAL

♦ Before commencing activity or entry into site all electrical tools and equipment will
be brought to the GASCO electrical inspection department. Only inspected,
tested and approved apparatus will be used.

7.29.7 IMPORTANT NOTICE

♦ Electrical activities will only be handled by qualified and certified technicians


under the direct supervision of the Electrical Engineer / Electrical Supervisor.

7.30 EXCAVATIONS

♦ All excavations under TPG / UTS KENT responsibility will be done with a valid
excavation permit and depending on the depth and width of the trench, stepping,
shoring and shuttering would be provided. Ladders will be provided every 15 mts.
for emergency escape. Proper barriers or warning tapes will be fixed and night
flash lights (suitably rated for the hazardous area classification, minimum zone-2)
will be displayed. Access platforms will be provided over trenches. No trenches
shall be kept unnecessarily opened after the activity is completed. The Safety
Officer will make periodical checks that proper danger warning systems are
maintained.

♦ Prior to excavation work start, the excavation permit issued shall be attached with
a detailed sketch of activity area. Concerned GASCO Engineers / Supervisors
shall be consulted in this regard.

♦ During excavation if any underground existing services like cables / pipelines are
encountered which are not shown on the drawings, the excavation work will be
suspended and these items will be brought to the attention of GASCO authorities
immediately and the work will only continue as per the instruction of GASCO.

♦ All excavations that shall block access or roads and any temporary supporting for
existing services will be carried out only after discussions and through a method
statement which will be acceptable to TPG / UTS KENT and GASCO.

♦ All excavations will be carried with proper supervision.

♦ In case of excavation by mechanical means all equipment shall be under the


control and use of fully trained, qualified and licensed operators.

♦ The method and location of surplus excavated spoil will be with prior approval of
GASCO Project Manager

♦ Any mechanical equipment used in the course of work which requires washdown
to comply with any GASCO Site Regulations will be washed in an approved

♦ All excavated soil causing inconvenience to existing plant activity shall be


removed and stored in a suitable location approved by the GASCO authorized
Civil Engineer.

7.31 PAINTING

♦ All painting activities will be Subcontracted to a GASCO approved Contractor who


will be contractually obliged to maintain the HSE regulations of TPG / UTS KENT
and the respective GASCO regulations as mentioned here.

♦ TPG / UTS KENT Project Manager with the assistance of the Safety Officer will
monitor on a regular basis the Safety standards, Environmental hazards,
Occupational health hazards of the Subcontractor employees. Corrective actions
will be implemented to suit the activity as per GASCO Safety standards

7.32 OCCUPATIONAL HEALTH AND HYGIENE

The TPG/UTS KENT Policy dictates specific guidelines in selection of personnel in all
our activities. Contact to health hazard substance, radiation etc. will be controlled by
the company health & hygiene policy by using risk evaluation methods. Pre
examination and checks will be carried out on such materials by trained responsible
persons. Complete data, regarding such substances, will be made available to the
work force. All relevant Personnel Protective Equipment to suit such situations will be
made available. Accommodation, food quality, water supply and waste handling will
be maintained under strict hygiene standards.

7.33 RADIATION

All radiation activities related to TPG/UTS KENT Projects will be strictly implemented.

♦ All radiography activities shall be sub-contracted to GASCO approved


contractors.

7.34 NOISE CONTROL

7.34.1 TPG/UTS KENT will take necessary steps to reduce and control noise, starting from
proper design, arrangements and introducing PPEs. Necessary notification shall be
issued to Company in advance regarding details of noisy works to be conducted at
site.

7.34.2 DESIGNING

♦ Less noise emitting machinery will be used as much as possible.

♦ Possibility of introducing noise barriers in fabrication shops etc. will be planned.

7.34.3 ARRANGEMENTS

♦ Diesel Generators, grinding activities and cutting activities will be arranged in


such a way that the noise produced will affect personnel to a minimum. Activities
of high noise will be staggered.

7.34.4 PPE

♦ All relevant PPE like ear muffs, ear plugs etc. will be supplied and worn by
personnel working in high noise level areas.

7.35 ALCOHOL DRUGS AND SMOKING RESTRICTIONS

Consumption or carrying of alcohol and drugs are strictly prohibited in the GASCO /
TPG / UTS-KENT area of activity. Any prescribed medicines to be discussed with the
Site Safety Officer before consumption. Strict disciplinary actions are to be taken for
violation.

Smoking other than the permitted areas are strictly prohibited. Carrying of matches,
lighters or any kind of ignition source are prohibited, other than those required for site
activity and permitted through proper hot work permit.

7.36 PHOTOGRAPHY RESTRICTIONS

Photography is not permitted at project site or in GASCO plants. Necessary


permission is to be obtained from TPG / UTS KENT site supervisor and GASCO
management before a photograph is taken.

7.37 ACCESS RESTRICTIONS

The TPG / UTS KENT employees shall move only in designated project sites and as
mentioned in the permit. Access without proper permission to places other than the
allocated ones is strictly prohibited.

8. ENVIRONMENTAL PROTECTION POLICY

TPG / UTS KENT in its activities will comply with the Environmental Protection Policy
of GASCO.

The possibility if handling hazardous chemicals or materials in this project by TPG /


UTS KENT is small. However, very strict rules and regulations will be laid to protect
the environment in all respects. The company Safety Officer will be monitoring all
aspects related to Environmental hazards. Negative points will be noted and reported
to the TPG / UTS KENT line management and GASCO Environmental Protection
Department for immediate action.

TPG / UTS KENT shall pay particular attention to maintain the environmental
standards through its activity by training its personnel and insisting upon the safe
handling of hazardous material and chemicals, following proper waste disposal
programmes, reducing generation of waste and encourage reusing and recycling of
waste.

8.1 HOUSE KEEPING, WASTE SEGREGATION AND DISPOSAL

The Project Manager together with the Safety Officer will inspect TPG / UTS KENT
yard facilities / work sites to assess the quality of house keeping, waste segregation
and disposal methods on a weekly basis. House keeping rating forms will be used for
assessment. The reports will be included in the monthly safety returns. All records
will be maintained for auditing.

8.1.1 HOUSE KEEPING

TPG / UTS KENT will be maintaining a scheduled house keeping procedure as per
GASCO procedures. Daily, Weekly and Monthly schedules will be implemented.

8.1.2 WASTE SEGREGATION

Separate containers/drums will be used as per GASCO procedures to collect and


segregate waste produced (refer Appendix 5 for list of GASCO Procedures and their
location). Following colour coding will be introduced for drums/containers.

Site messing facilities for TPG/UTS Ket employees is not being envisaged. However,
if it can not be avoided, TPG / UTS KENT employees shall be allowed to have food
only in designated mess at site. The waste generated shall be suitably disposed.

8.1.5 TRAINING

A house keeping/ waste segregation & collection waste disposal team will be formed
at the beginning of the Project and necessary training will be given to the team to
maintain the standard as per GASCO requirements.

9. GENERAL INFORMATION

Zones

Hazardous areas are classified into zones according to the degree of risk.

Zone 0 - where an explosive atmosphere is continuously present for long periods.

Zone 1 - where an explosive atmosphere is likely to occur in normal operation.

and if it occurs it is only present for a short period.

9.2 HYDROGEN SULPHIDE ( H2S )

9.2.1 GENERAL DESCRIPTION

Hydrogen Sulphide is a highly toxic, flammable and colourless gas which, at low
concentrations may be accompanied by a “rotten egg” odour and a sweet taste.
Hydrogen Sulphide odour may be detected by smell at concentrations as low as 0.13
ppm. A sudden exposure to high concentrations may retard or arrest the ability to
detect the odours. The presence of smell is not a reliable detection method.

The Threshold Limit Value (TLV) for Hydrogen Sulphide is 10 ppm on a 8 h time
weighed average.

Hydrogen Sulphide is soluble in water, alcohol, petroleum solvents and crude


petroleum.

9.2.6 DETECTION OF H2S

Fixed Detection Systems

Fixed detection systems are located in different locations of project site for continuous
monitoring of H2S concentrations expected, due to leaks, ruptures etc. The sensors
shall be collectively monitored at a central point.

Portable Detection Equipment.

Personnel working in areas not monitored by fixed detection and alarm systems,
where there is a probability of encountering H2S are to be provided with personal
electronic detectors which will sound an alarm at or above 10 ppm.

Audible alarm will sound at 10 ppm and visual alarm at 20 ppm.

9.2.7 FIRST AID

Persons overcome by H2S shall be moved at once to fresh air by rescuers wearing
breathing apparatus; oxygen may also be given to help dissipate the effects of
exposure to H2S.

An unconscious person who is not breathing shall be given artificial respiration,


without interruption, until normal breathing is restored. Ensure that artificial
respiration is carried out in a gas free atmosphere and that the victim is referred for
proper medical treatment.

Note:-

Every Tuesday 10 A.M. GASCO will test the sirens.

Occasional security exercise are also conducted by the Oil Field Security Police
within the plant. These tests do not usually interfere with work being carried out.
Project Manager will be informed of dates and time of exercises if these are known.
