COLS..Hundred And Seventy

On 10th May 2004 his mother Venutai then 72 yrs old had sudden cardiac arrest in a car which he was driving.
This happened at a very busy intersection at Amravati. On realising the catastrophe his wife who is a
paediatrician raised alarm. Dr.Rajendra immediately stopped the car and gave Chest compressions in the car.
She revived successfully. She lived a healthy life till her death on 14th September 2016. This incidence inspired
him & teaching Bystander CPR to lay people became his passion. For this purpose he travesl a lot .
The first such workshop was conducted on his succeeding birthday 27th June 2004.
Since then 170 such COLS hands on workshop he trained more than 18000 lay people from various walks of
life, youngest being 8 yr old child & oldest 82 yr old grandfather.
He carries his laptop, projector, mannequins, sound system & single handedly train people in COLS till he is
satisfied.

MISA NEWS APRIL 2019 ISSUE

COLS but for Unique Audience....Dr.Tasneem Dhansura Mumbai

COLS CPR for the Visually Challenged- An Experience.

Medical emergency situations can occur anywhere and at anytime. There is a growing consensus that
persons with disabilities should be included in mainstream society. Inclusive education recognises that
teaching methods which can make curriculums accessible to persons with disabilities can help build their
futures.
India is home to the worlds largest number of blind, of the 37 million blind in the world 15 million are in
India.India as a country has one of the largest visually impaired/blind population in the world (approximately
8million blind, 54 million visually impaired people in India).
Without any references being available to teach any kind of life saving skills to them , we decided to make
minor modifications to COLS CPR framework to include training the visually impaired as it could promote
development of self-confidence, self-esteem and social inclusiveness among them.
After visiting the National Association For The Blind centres in Mumbai where lifestyle rehabilitation training is
being given to visually impaired people so as to make them accustomed to 3As- Accept Blindness/Visual
Impairment, Adjust to Blindness/Visual Impairment and Achieve despite being blind/Visually Impaired, we
modified and designed the COLS activity as per their needs.
Students at these centres are given intensive training for confidence building to lead a normal and productive
life through activities like physical training, participation in sporting and cultural activities, visits to public
places, familiarisation with public transport etc. They are also given vocational guidance, exposure to work
avenues open to them over a four month training period.
We mainly focussed on, Understanding each other, making them understand why is it important to
learn COLS, how to perform CPR (verbal lecture), hands on training and experience on mannequins
MISA NEWS APRIL 2019 ISSUE

by touch and feel technique. A step by step guideline was formulated and taught to them by making slight
modifications in COLS guidelines formulated by ISA.
Guidelines: In case they feel or realise somebody has collapsed in front of or near them
1.Check for response (verbally/ feeling for breath at nostrils/feeling for chest expansion
2.To ensure scene safety/their own safety first
3. Call for help, Dial 108
4. Move the collapsed person to a safe place and start Chest Compressions
5. For chest compressions feel for midline of the body from Head→ Forehead→ Nose→ Mouth→ Neck→
Breast Bone→Feel for end of the bone →Star ng of so part of abdomen
6. Go back 2 fingers start chest compressions at that area 5 cycles of 30 compressions each at the rate of
100-120 compressions per minute for an approximate depth of 5-6cms ( Tactile aids in the form of boxes with
an approximate height of 5cms, index and middle fingers ar on an average 5-6cm in length, was used and
given to them to feel it to have an approximate sense of depth perception)
7. After 5 cycles of 30 compressions each check for response in form of coughing or vocalizing
8. If responsive shift them in Recovery Position (sideways position) and early transfer to medical facility
9. If no response continue COLS as long as possible or till medical help arrives.
The Xavier’s Resource Centre for the Visually Challenged (www.XRCVC.org) provides assistance to VI students
across many schools and colleges and has a comprehensive e-library. The COLS flowchart was creared into a
word document , which can be printed into Braille using a Braille embosser, this was later incorporated into
our training programmes.
We also created a playlist of popular Bollywood songs with a 120-130bpm rhythm which enabled the VI to
give effective chest compressions which was verified in one session where we used the QCPR mannequins.
The future has so much more to be done, incorporating COLS within the framework of the rehabilitation by
the NAB and othre NGOs that are involved with the VI, incorporating BCLS into the physiotherapy curriculum
for the VI, creating Audiobooks for the same curriculum since many of the VI who are not blindsince childhood
cannot read Braille.
A truly humbling experience, and what it can teach the rest of the population is “ If they can, anyone can” .
I would like to acknowledgemy colleagues( Dr Swati Kudalkar, Dr Tarana Shaikh, Dr Nabila Shaik), my students
(Dr Aastha Khurana, Dr Abhishek, Dr Sanchita Mahajan, Dr Shweta, Dr Yash Upadhya,, NirbhaGhurye and
Radhika), friends from the NAB , special thanks to Dr Sam Taraporewala and Krishna from XRCVC, without
whose love and constant support this would not have been possible
