PURSUANT TO D.O.

16 SERIES OF 2001

GUIDELINES PROCEDURES ON ACCREDITATION OF OCCUPATIONAL SAFETY


AND HEALTH PRACTITIONER/CONSULTANT

1. QUALIFICATIONS
A. Practitioner in Occupational Safety and Health
1. Must have completed the Bureau prescribed 40-hour Basic Occupational
Safety and Health Training Course from DOLE-accredited or recognized
safety training organization or institutions;
2. Must have relevant experience in occupational safety and health (OSH);
a. Three (3) years experience is required if applicant is duly
licensed
b. Four years experience, if graduate of any 4 or 5 year college
course without a license;
c. At least 10 (10) years experience if the applicant is a college
undergraduate.

3. For occupational health nurse, physician, dentist, he/she must possess the
minimum qualifications stated under Rule 1964.01 nos. (1) – (5) of the
OSH Standards, whichever is applicable.
Note: Relevant experience in occupational safety and health means actual
practice on OSHC or actual practice on OSH plus attendance to various
trainings, seminars and other related learning processes. Eighty (80) hours
shall be converted as one (1) year experience. However years of actual
practice must not be less than 2 years.

B. Consultants in Occupational Safety and Health


1. Must be an accredited practitioner in occupational safety and health for at
least five (5) years with experience in at least two (2) fields of
specialization enumerated in Annex “A” of D.O. 16 series of 2001;
2. Must have experience in policy formulation and development, monitoring
and evaluation, and implementation of occupational safety and health
management systems;
3. Must have completed the 80-hour Advanced Occupational Safety and
Training Course as prescribed by the Bureau, and
4. Must have earned an additional 480 hours of training/seminar or related;
earning processes in various fields of occupational safety and health from
accredited organizations or institutions authorized by laws.

II. REQUIREMENTS
The following documents must be submitted according to the following order,
duly labeled and placed in a long folder:
1. Duly signed application letter addressed to the Bureau Director;
2. Two (2) copies of resume (duly signed in all pages) including the
following information:
A. Name and address and contact number of applicant;
B. Personal Data specifying the following information: Sex, Civil
Status, Type of employment, Birth date, Birthplace,
Citizenship, Religion, Height, Weight, Blood type, SSS/GSIS
No. and TIN;
C. Educational background
D. Employment background (Name of company, position, status
of appointment inclusive dates)
E. List of OSH Trainings/ Seminars attended (Time, Date,
Number of hours, Venue, Conducted by)
F. Licenses/ examinations/ eligibilities passed
G. Skills specialization
H. Other qualifications, skills related to OSH.

3. Two (2) copies (1”x1”) of most recent ID picture (red background for SP
and blue background for SC) signed by the applicant at the back;
4. Photocopy of certificate of completion of the BWC prescribed course on
Occupational Safety and Health;
40-hours Basic Occupational Safety and Health Course for SP
applicants
80-hours Advanced Course on OSH for SC applicants;

5. Original certificate of employment (indicating name, present position, date


of appointment and actual job description/ duties and responsibilities, duly
signed by immediate supervisor and employer using official letterhead of
the company;
6. Photocopies of college diploma or Transcript of Records and/or PRC
license/ID (if any);
7. Photocopies of certificates of attendance/ participation in any OSH related
trainings/ seminars/ conventions, etc.;
8. Sample copies of safety reports, program prepared/ implemented/
accomplishments related to safety and health;
9. Other documents as maybe required.

III. WHERE TO FILE


Applications may be submitted directly to the BWC or through Labor Standards
Enforcement Division of the DOLE Regional Offices. Documents may be
authenticated by an authorized LSED staff prior to endorsement to BWC for
evaluation.

Follow-up from applicants shall be entertained five (5) days after submission of
application.

IV. EVALUATION PROCEDURE


1. Documents/ Application shall be evaluated for completeness, veracity and
sufficiency;
2. Result of evaluation shall be reviewed by authorized BWC staff for
appropriate recommendations whether approved or deferred;
3. The applicant shall be notified in writing of the results of evaluation upon
receipt of the documents by the evaluator (whether documents are
complete or there are still deficiencies/ requirements that need to be
complied with or submitted to the Bureau).
4. An applicant may be required to appear personally for an interview and to
present to the Bureau evaluator, original copies of documents submitted
for authentication purposes (if said documents were not yet certified as
true copy by an authorized receiving officer of the Bureau or RO).
Applicant may get an appointment for Interview. Schedule for interview
is from Tuesday to Thursday, a.m. only.
5. Evaluation and approval of documents normally takes 15 days upon
submission if requirements are complete and. Or applicant has appeared
immediately for interview after proper notification by the evaluator.

V. PAYMENT OF FEE(S) AND ISSUANCE OF CERTIFICATE OF


ACCREDITATION AND IDENTIFICATION CARD

Upon approval of application, a certificate of accreditation valid for three (3) years
and an identification card shall be issued to any applicant who has satisfactorily met/
complied with all the requirements specified in this rule, and upon payment of the
prescribed fee as provided for under Rule 1974 of the Standards as amended by D.O.
issued by the DOLE.

Payment Procedure
Step 1. The accredited SP/.SC shall first secure from OHSD an order of payment
form indicting breakdown of fee to be paid for accreditation. A total of P450.00 shall
be paid by an accredited SP, while P750 shall be paid by an accredited SC for a three
year validity period.

Step 2. Proceed to the DOLE Cash Division for payment of required fee. Upon
payment, an official receipt shall be issued.

Step 3. Present O.R. to OHSD for recording of O.R. number and date of payment and
issuance of certificate of accreditation.

Profile form shall be filled-up by the accredited SP/SC.


VI. REQUIREMENTS FOR RENEWAL
Accreditation for practitioners and consultants shall be renewed every three (3) years
on or before original date of accreditation by complying to the following
requirements:

1. Application letter for renewals of accreditation as practitioner or


consultant I occupational safety and health addressed to the Bureau
Director;
2. Submission of an updated bio-data with 2 copies of most recent 1”x1”
picture;
3. Presentation of original and submission of certified true copies of
additional and relevant occupational safety and health training certificates;
4. Summary of individual accomplishments as OSH Practitioner/ Consultant
duly signed by the immediate supervisor and employer using official
company letterhead;
5. Proof(s) or supporting documents to the accomplishments submitted.
6. Payment of renewal/ annual fee. A total of P450.00 shall be paid by an
accredited SP, while P750 shall be paid by an accredited SC for a three
year validity period.
