Subject : Corrective and Preventive Actions

Dear Sir/Madam,

Good day Mam/ Sir kindly see Corrective and Preventive Actions of Coulomb Energy Technic. Inc for
the followings issues:

ISSUE IMMEDIATE ACTION PREVENTIVE ACTION

Using grinding tools with
unusual tapping of chords
and make it to use, but
unsafe and not compliance
to the Wyeth safety
management.
Using electric drill with an
expire PEE.
Immediately pull-out of tools
that wouldn’t complied with
the safety order.
Immediately turns it over to
safety inspector from Wyeth
together with the authorized
and certified electrician from
our company and check it
before to use.
 Daily inspection of
tools before using it.
 Time-to-time
monitoring of tools during
work and the person
whom using it.
 Weekly inspection of
the expiration of PEE of
electrical and power tools.
 Use a log book and
make it noted or filed
every PEEs of power
tools and make it easily to
check and inspect.

With this we will take the immediate action to avoid untoward incident that may compromise the
safety of our personnel’s.
