JUDICIAL AFFIDAVIT OF RUPERTO NG PADUHILAO

(Examination Conducted by Atty. Don V. Sala of Mag-ata,
Tomas Oppus, Southern Leyte)

PURPOSES:

1. The testimony of this witness is offered to prove that

he is the representative of the petitioner in this case;
2. He will prove the allegations in the Petition;
3. He will identify the documents mentioned in the
Affidavit;
4. He will testify on related matters.

I, Ruperto ng Paduhilao, the representative of the

petitioner in this case, married, Filipino and resident of Brgy.
Looc, Tomas Oppus, Southern Leyte, after having been sworn
to in accordance with law depose and state:

(Do you have any proof that it should be January 5,
1958?)

A: O sir. Ang ija Baptismal Certificate, Elementary School

Cumulative Record, and Secondary Permanent Record.
(Yes sir. Her Baptismal Certificate, Elementary School
Cumulative Record, and Secondary Permanent Record.)

Q11: Naa pa ba lain parte sa Birth Certificate nga sajop or wa
nabutangi og entry?
(Is there any other part in the Birth Certificate that is
erroneous or lacking any enrty?)

A: O, sir. Wa nakabutang sa petsa og lugar sa pagminju sa ija

ginikanan.
(Yes, Sir. There is no record on the date and place of
marriage of her parents. )

Q12: Unsa man ang petsa og lugar sa pagminju sa ija

ginikanan?
(What was the date and place of marriage of her
parents?)

A: October 14, 1954 sa Malitbog, Leyte, Philippines ang

nakabutang sa ila Marriage Certificate.
(October 14, 1954 at Malitbog, Leyte, Philippines as
reflected in their Marriage Certificate.)

Q13: Unsa mn imo i-ampo sa halangdon nga Korte?

(What do you pray of this Honorable Court?)

A: Nga kani ahu nga petition matugotan.
