DEED OF WAIVER OF RIGHTS WITH

SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, GENESA P. PARAGADOS, of legal age, Filipino citizen, a resident
of Bio-os, Amlan, Negros Oriental, hereby declare:
1. That I am the owner of St. Peter Life Plan # __________ of
which I have fully paid for burial and funeral services to St. Peter
Life Plan, Inc.;
2. That I am the first cousin of the late ROENA G. REGALADO
who died due to a car accident in Muscat, Oman on December 3,
2017;
3. That I have fully assigned all the benefits of the said Life Plan to
the deceased which was used for her burial and funeral last
February 9, 2018.
4. That, on the basis of the foregoing premises, I hereby waive all
my rights, interests and benefits with respect to the burial and
funeral expenses in favor of CHRISTIAN G. REGALADO, of
legal age, married, and who affixed his signature herein below as
manifested and his acceptance of the same;
4. That, by these presents, I do hereby name, designate and
constitute CHRISTIAN G. REGALADO as my Attorney-in-
Fact, for me and in my name, place and stead and for my own
use and benefit to demand, collect and receive from any and all
government or private agencies any amount as burial assistance,
and to do all things/acts required within the authority granted,
hereby ratifying and confirming all that he shall do or cause to
be done under and by virtue of these presents.
