Today's investigation is in response to a complaint pertaining to mold,


rats and roaches observed in the the Halcyon building.
Upon arrival EHS spoke to Steve Lapham, Facilities Services Manager.

Mr. Lapham stated there is a mold problem in the Halcyon building
and they are awaiting a proposal from Andre Downey, General Manager
of Sandow Construction, Inc.

A copy of the emailed from Mr. Lapham to the staff at WAU, dated
Sept. 18, 2018, pertaining to affected areas on campus to determine
scope of work was forwarded to EHS's email at the time of this
inspection.
Note
Mr. Lapham stated they were not aware of rats inside the dorms. An
ongoing pest control treatment plan is provided by American Pest
Control Company. According to Mr. Lapham, the pest control
representative stated he has never seen rats inside the building but
saw evidence of rats outside the building a year ago.

Roaches and mice were reported on American Pest work orders several
occasions inside Halcyon Hall. For example:

Note: Copies of work orders from American Pest Control were given to
EHS for review. No evidence of vermin in the building at the time of
inspection.

According to Mr. Ellis, of WAU Facility Services, Sandow Construction,


is a mold remediation vendor, whom was in the buildings on Friday,
Sept 21st and is putting together an approach to correct along with
taking air samples to determine levels and possibly species and
submitting a proposal. Vendor is implementing remediation plan of the

problem.
Forward a copy of the results for air quality tests and remediation plan
with a timeline to local health department.
Note
Broken lid on dumpster and doors are ajar, dumpster is on grass, bags
of trash on ground behind dumpster.
1C. The person in charge shall ensure that garbage containers outside the
Maintained building are located on a grease resistant, durable surface; that the
storage areas are kept clean; and garbage containers are vermin-
proof. Make the necessary corrections.
Halcyon Hall observations: (escorted by Aliyah)

1.) Room 103 - black microbial spores observed on new AC vent


2.) room 130 - (unoccupied) black microbial matter observed on ceiling
near window
3.) room 131 - tenant stated there was mold on ceiling and door, but
recently the maintenance staff painted over the mold on the ceiling;
but it is still on the door.

Mildew odors observed throughout the Halcyon Hall rooms and halls.
Water damaged indoor/ outdoor carpet observed in the halls.
De-humidifiers were installed in the corridors of dorm rooms in Halcyon
Hall on 9/25/18.
Damprids were given to residents and observed in the dorm rooms.

High moisture and high condensation observed collecting on wall
mounted HVAC unit in hall; resulting in ambient environment being

saturated with humidity.
Make the necessary corrections to eliminate air quality problems.
Provide professional services to test the quality of air and eliminate the
violation. Submit results of air quality tests to local health department
within one week of this report.
