– Certificate of Completion (Original Copy) and
RCTLCL plus two photocopies of the said
documents certified by the Department Head,
the instructor and the Student Assistance
Services Office

Course Requirements
• Video Presentation
– A 30-minute presentation (maximum) must be prepared
by a student summarizing the nature of his/her on the
job training, actual demonstrations of work/duties,
reflections about on the job training and supervisor’s
evaluation. Presentation must be submitted in a CD in an
acceptable and widely used video format. Copies of the
video must be given to SASO, the department/college
concerned and to the organization/institution where on
the job training was held. The University will only accept
authorized videos and if found cheating, you will get a
grade of 5.0 for the entire course. FAILURE TO SUBMIT
WILL AUTOMATICALLY MERIT YOU A GRADE OF INC.

Related Course Policies


• Submit the course requirements on time!
• Don’t hesitate to see your instructor anytime whenever
you needed advice about you OJT and other relevant
activities about the course.
• Maintain regular coordination with your instructor,
department and Student Assistance Services Office and
report for any problems you will encounter during the
course.
• Follow the institutional/company rules where you are
engaged with; observe punctuality and do your best at all
times.
• Always keep in mind your University Core Values and
apply it in every activity you are assigned in your OJT.

Related Course Policies


• Never ever submit a copy-paste document or
any plagiarized materials during the entire
course. Failure to abide will automatically merit
a grade of 5.0 and can be grounds for expulsion.
• Failure to submit the final hard bound copy and
electronic format of your portfolio will
automatically merit you a grade of INC. Your
instructor will closely monitor your portfolio and
in any given date, he will look at it and have it as
his basis for your portfolio grade.

Related Course Policies


• Your industry immersion report will be graded
according to the content and results of your
study. The format of the industry immersion
report will be the same as to a research paper,
except that the scope of the study is only good
for a single term. THE SUBJECT OF YOUR
INDUSTRY IMMERSION REPORT MUST ANSWER
CERTAIN ISSUES OF THE COMPANY/INSTITUTION
WHERE YOU ARE HAVING YOUR OJT AND MUST
BE HIGHLY RELEVANT TO THE PROGRAM YOU
ARE TAKING. FAILURE TO SUBMIT WILL
AUTOMATICALLY MERIT YOU A GRADE OF INC.

Frequently Asked Questions


o What should I do to at least get an INC grade
in the course in case/s I failed to finish my OJT
within the semester I enrolled?
o Submit one of the three course requirements
mentioned or;
o Present and submit a certification signed by the
company where OJT was conducted attesting
that at least you’ve completed two hundred ten
(210) on the job training hours

o Yes but it will be dependent on the availability of
