A. BACKGROUND
The success of health development in Indonesia are affecting on reduction of birth rate,
morbidity, mortality and at the same time increasing life expectancy. Life expectancy of Indonesia
were 68.6 years in 2004 then become 69.8 years in 2010. In 2015 the life expectancy are growing
to 70.8 years and it projected to keep growing at the future resulting an increase of the number of
senior citizens in the future significantly. Indonesia are become one of the top five countries with
the highest proportion of senior citizens in the world. The population census in 2010 stated that
there were 18.1 million of elderly or 7.6% of the total population and it continue to grow. According
to The Central Bureau of Statistic in 2013 the number of senior citizens are predicted to grow up to
27.1 millions of people in 2020, then become 33.7 milllions in 2025 and keep growing up to 48.2
millions in 2035.
This situation needs an attention from the goverment. The goverment of Indonesia has take
several action to ensure the good health and welfare of senior citizens. It is all began at 1996
where the goverment determine that May 29th is the day for seniors. Furthermore, in 1998 the
goverment issued the 13th Legislation which known as Senior’s Welfare Act as a legal basic where
the seniors were akcnowledge. At 2009 the goverment issued the 36th Legislation about The Act
of Health which stated that the efforts to improve and maintain public health carried out based on
the principle of non discriminatory, participatory and sustainable. The efforts to maintain health
care for elderly are intended to ensure that the elderly remain healthy, independent, active and
productiver socially and economically. To achieve this goal the goverment is obliged to ensure the
availability of health care facilities and facilitating the development of senior’s community.
As the senior citizens grow older, the more likely they are facing problems such as physical,
mental, spiritual, economic and social one. The fundamental problems faced by the seniors are
health problem caused by degenerative proccess which is indicated by disease data pattern of
elderly. Based on basic medical research (Riskesdas) in 2018 the most prevalent diseases in
elderly are non communicable disease such as hypertension, osteoarthritis, dental – mouth
problems, Chronic Obstructive Pulmonary Disease (COPD) and Diabetes Mellitus (DM).
In the other hand, aging process experienced by seniors lead to a condition called geriatric
syndromes. Geriatric syndromes include a number of condition typical of, if not spesific to, aging
such as dementia, depression, delirium, incontinence, vertigo, falls, spontaneous bone fractures,
failure to thrive, and neglect and abuse. Geriatric syndromes are associated with poor quality of life
and reduced life expectancy.
The main problem for the elderly is the fulfillment of health care needs. Therefore it is
necessary to develop health services that prioritize the improvement, prevention, and health care
maintenance in addition to healing and recovery efforts. Long term purpose of this efforts are to
gain better quality of life of the seniors.
Therefore as an efforts to support the success of the National Medium Term Development Plan
(RPJMN) in 2019, the nurse as a part of health worker are play important roles. The nurse should
be aware of this issue. Based on the above mentioned background, Faculty ofHealth Science
University of Muhammadiyah Jember would conduct an International Seminar and Workshop of
Gerontology Nursing with the theme “Resolving Geriatric Syndromes Through Elderly Health
Services”.

“RESOLVING GERIATRIC SYNDROMES THROUGH IMPLEMENTATION OF ELDERLY
SERVICE POLICY”
A. INTRODUCTION
The elderly population is growing rapidly in various part of the world including in
Indonesia. This phenomenon is due to the decrease of birth rate and increase of life span. The
life expectancy in Indonesia is expected to increase from 67.8 years in 2000 – 2005 to 73.6
years in 2020 – 20125. Accoding to World Health Organization (WHO), the proportion of elderly
will rise to 22% of the world population, and in 20150 there will be 2 billion people aged 60
years and older. In 2002 Indonesia was ranked 6th as the most populous elderly people, and
expected to raise to rank 5th in 2025. The increase of elderly people will be followed by the
emergence of various problems in health care and services.
As people aged, they will face changing and declining of organic function due to aging
proccess. Aging proccess will lead to the changes of structure and function of body part, it will
affect the homeostatis of the body so that they become susceptible to having a disease and
increase the morbidity and mortality. Those health problems will affect the elderly physically
and psycologically, then it will cause the declining of functional capacity. Elderly with declining
functional capacity will become dependent to other and susceptible to poor quality of life.
Indonesian goverment has a big responsibility to ensure that the Indonesian seniors
will have good quality of life. One of the effort that has been taken by the goverment is securing
delivery of health service among the elderly by issued some elderly service policy. The
goverment has establish elderly health care as a national program. It means that every sector
of health services, wether in community based or hosptal based, they should guarantee the
availability of this service.
It is important for the health workers to understand about the charateristics of elderly,
the aging proccess, geriatric syndromes as their health problems and all the policy that has
been issued as the guideline to delivery this services. Nurse, as the part of health worker, who
spent most of the time of their work to interact with client should aware and have a new
perspective about elderly and their problem. So that the nurse will expected to delivery best
service to their client, helping the elderly being an independent senior with dignity, achieving
better quality of life for elderly and their family, and helping the elderly to pass away peacefully.

WORKSHOP
“EARLY DETECTION AND TREATMENT OF ACUTE CONFUSIONAL STAGE IN SENIOR HAJJ
PILGRIM OF INDONESIA”
A. INTRODUCTION
The Hajj, pilgrimage to Mecca, is the largest mass migration in the world. Each year 2.5 million
Muslims from over 160 countries travel to the same place. During the hajj, the pilgrims undergo
great physical and emotional strain. Good medical preparation including vaccination is very
important for the pilgrimes who undertake the hajj, in particular for those who are older and
have chronic disease. The chance of transmission of disease is also high and rapid contagion
of Hajj pilgrim cpuld cause a pandemic. It is therefore important that the responsible medical
team is aware of the health risk to the hajj pilgrim and his environment.
The largest Hajj quota is allocated to Indonesia, the workd’s most populous muslim nation.
Performance of the Hajj and it’s rites are physically demanding, together with overcrowding
owing to the large number of pilgrims. Health risk during the Hajj including sun exposure
(depending on the season when the Hajj falls – summer or winter), dehydration, crowding,
steep inclines and traffic congestion. As the Hajj is fairly expensive for Pilgrims in Indonesia, a
middle income developing country, the pilgrims tend to be older with medical comorbidities.
As we know that when people get older, they will face the declining of organ function and
functional capacity. The physiological changes accompanied by the condition of surrounding
environment during the Hajj will make them become succeptible to many health problems. One
of common health problem among the Indonesian senior pilgrimage is acute confusional stage
or delirium. This condition will make the senior pilgrim to be confuse and disoriented. The heat
stroke not only affected their body, but also mentally as well.
The medical team need to pay attention to this condition. They are expected to detect and
delivery appropriate initial treatment for delirium. Based on the above mentioned background
we think that it is important to having a workshop that will enhancing the knowledge and the
skill of the nurse as a part of medical team during the Hajj so that the senior pilgrim who having
delirium or acute confusional stage will get an appropriate treatment.
