Dear Ms. Bandiola,

We are pleased to inform you that we are accepting your employment application with our company. The terms and conditions of this
acceptance are as follows:

A. General Conditions

Position : Office Staff

Status : Probation for 6 months. Your services may be terminated for just cause or authorized
causes or for failure to comply with standards set by the company hereunder.
Current Salary : P7,500.00 per month inclusive of government mandated wage increase and COLA
Effectivity of new contract : February 12, 2018
Home Base : Davao City

B. Duties and Responsibilities

1. Compile, copy, sort, and file records of office activities, business transactions, and other activities
2. Maintains office supplies inventory by checking stock to determine inventory level; anticipating needed supplies;
placing and expediting orders for supplies; verifying receipt of supplies.
3. Compute, record, and proofread data and other information, such as records or reports.
4. Collect, count, and disburse money, do basic bookkeeping and complete banking transactions.
5. Print and scan paper waivers’ and other forms as needed.
6. Maintain and update filing, inventory, mailing, and database systems, either manually or using a computer.
7. Keeping diaries and arranging appointments; taking minutes.
8. Inventory and order materials, supplies, and services.
9. Process and prepare documents, such as business or government forms and expense reports.
10. Assists in updating/canvassing local materials.
11. Familiarize those products and services we offer.
11. Other duties as may be assigned from time to time.

C. Post-employment conditions

Should you resign or be separated from our company for any reason, you agree not to be employed directly or indirectly by
all our competitors or by any company carrying our products and services within one (1) year from the effectivity of your separation.
The employee acknowledges that the Company shall or may in reliance of this agreement provide you access to trade
secrets, customers and other confidential data. Employee agrees to retain said information strictly confidential and not to use said in
formation on your own behalf or disclose the same to any third party.

D. Non-Disclosure Agreement

The undersigned employee hereby agrees and acknowledges:

1. That during the course of my employment, certain trade secrets of the company may be disclosed to me; said trade secrets, but are
not necessarily limited to the following:

a. Technical Information: Methods, processes, formulae, systems, techniques, computer programs and research projects.

b. Business Information: Customer list, pricing date, sources of supply, financial data and marketing production, or
merchandising techniques and plans.

2. I agree that I shall not during, or at any time after my termination of my employment with the company, use for myself or others, or
disclose or divulge to others including future employees, any trade secrets, confidential information, or any other proprietary
data of the Company in violation of this agreement.
3. That upon termination of my employment from the Company:

a. I agree that I shall return to the Company all documents and properties of the company, including but not limited to:
reports, manuals, customer lists, computer programs, and all materials and all copies thereof relating in any way to the
Company’s business, or in any way obtained by me during the course of employ.

b. I agree that the Company shall have the authority to withhold any amount due to be paid to the Employee in form of salary,
reimbursements or whatever amount due to be paid to the latter in case there are obligations of the Employee due to be
paid to the Company. I agree that the company therefore has the authority to deduct or set off the amounts due to and from
the Company to, and with, the Employee and vice versa.
c. This agreement shall be binding upon me and my personal representatives and successors in interest, and shall take effect
to the benefit of the Company, its successors and assigns.

E. Amendments

This contract constitute the entire agreement of the parties with respect to the subject matter hereof and there are no other
representations, promises, warranties or agreements, oral or written, by or between the parties with respect to such subject matter.
Unless otherwise provided herein, this Contract shall not be changed, modified, altered, or in any manner amended by acts or by mere
tolerance of the parties. All changes, modifications, alterations, or amendments of this Contract shall have no binding effect unless it is
reduced to writing and signed and agreed by the Company and the Employee.

F. liquidated Damages

You acknowledge that any violation of this Contract shall subject you to a civil liability in favor of the Company in the amount
of Php 100,000.00 which shall first be deducted from the employee’s salary or other benefits from the Company.

The penalty herein shall also include criminal liability, if warranted under circumstances.

We trust you find in this employment terms and conditions the key to a successful career in our company and to as meaningful
relationship with fellow workers as well as customers of the company.

With your signature on every page, you agree with, commit to and abide by the terms and conditions of this Contract of Employment.
