OFFICE OF HEALTH SERVICES

DUTIES AND RESPONSIBILITIES

The health services office aims to make available the much needed health service to
students and personnel of the university right with in the school premises. As such this can be
achieved by:
 Proper orientation to the students of health services offered by the school clinic at
the beginning of the semester. This is to inform the students specially the new
ones on the different services the clinic provides, as well as the extent of services
being offered.

HEALTH SERVICES OFFERED BY THE SCHOOL CLINIC:


A. Medical
 First aide intervention to any emergency situation within the
school premises.
 Administration of emergency medicines to minor ailments such as
menstrual pains, headaches, fever, and LBM.
 Vital signs taking / monitoring
 Weight and height measurement
 Cleansing, treatment and dressing of minor wounds/bruises;
 Removal of wound sutures per Doctor’s order;
 Referral of cases to the hospital or any health agency as needed.
 Follow up of referred cases;
 Health information dissemination or chair side health education to
clients;
 Guidance and counseling services to selected clients in relation to
family planning, responsible parenthood and others as the client’s
needs arise.
B. Dental
 Oral examination of patients to assess their dental needs and to
refer cases which are beyond the capability of the nurse to higher
authorities.
 Prevention through oral prophylaxis if needed.
 Curative measures – performed when the disease is present.

PHILOSOPHY:
All people have a right to quality health care that recognizes the uniqueness of the
individual, respects the individual’s right, and strives to empower individuals to make
appropriate choices about their health regardless of their age, income, education, gender, sexual
orientation or religion.

VISON:

To see each and every student healthy and productive to enable them to attain full
potential for education and likewise ensure a healthy school personnel dedicated and committed
to be of service to the students the whole year round.

MISION:

The health services office shall primarily provide quality health services geared towards
the attainment of a healthy student and personnel and a healthful and drug free school
environment.

Goals:

1. To provide efficient and effective operation of a functional clinic by health personnel.

2. To improve the medical and services of the school thru procurement of the much needed
medicines, supplies and equipment that will ensure the delivery of adequate health
services to the students and school personnel.

3. To ensure continues provision of needed health services through careful use and
maintenance of the medical instruments/equipment for ready use of the clients.

Objectives:
General Objective:
To make available the much needed health services to the students, faculty and staff of
the university right within the school premises with utmost care.

Specific Objectives:
 To procure medicines and other supplies needed in the office.
 To provide emergency treatment and the application of basic nursing skills
in the treatment procedures and conduct of first aide measures.
 To promote health as well as information dissemination of the relevant
current health issues/information to the students, faculty and staff.
 To properly sterilize instruments
 To refer serious cases to higher authorities;
 To make proper recording and filling of patients documents.

POLICIES:
POLICY ON ISSUANCE OF MEDICAL CERTIFICATE

 The clinic will issue medical certificates if, and only if, the employee or student has
undergone proper medical consultation with the School Nurse.
 If the employee or student has been seen by a physician other than the school nurse, the
employee or student in question will have to secure a medical certificate from the
physician who has seen him/her.

EMERGENCY CASES GUIDELINES AND PROCEDURES

 In case of emergency, the patient will be assessed by the clinic personnel on duty and will
be given necessary first aid. The patient will be sent immediately to the nearest hospital
together with the nurse so that medical treatment will be given.
 The nurse on duty will immediately inform the parents/guardian or significant others of
the patient.
 The nurse who accompanies the patient to the hospital will not leave him/her unless the
patient’s parent/guardian or significant others will arrive and/or the patient is in stable
condition and has been properly endorsed to the hospital staff.
 An incident report will be made by the nurse for proper documentation of the incident.

POLICY ON ISSUANCE OF MEDICINES/VACCINES AND OTHER DRUG


ADMINISTRATION
These are the policies on issuance of medicines/vaccines and other drug administration as per
physician’s order:

 Only a starting dose is given to every patient.


 Antibiotics are prescribed only by the physician.
 Topical medication is applied on a daily basis.
 Patients for intramuscular injection are entertained with referral note from Attending
Physician.
 Medicine should be given to students with mild symptoms such as low grade fever, runny
nose, occasional cough except asthma, and diarrhea when bowel movement is less than 3
times in 24 hours.
 Patients who have asthma, hypertension and other serious conditions should bring their
own medicines to school.
 All students with maintenance drugs or special needs must present medical certificate
from their Attending Physician for record purposes.
 Students with contagious illness should be isolated or be confined at home (e.g. chicken
pox, measles, mumps).

OTHER CLINIC POLICIES AND GUIDELINES


On issuance of excuse slip:
· Excuse slip is only given to students if they are seen and checked by the nurse on duty and
assessed to have certain illness. They can be excused from class and will be allowed to go
home.
On being absent from class:
· If a student has been absent from class without informing the nurse-on-duty and was not
able to visit the clinic at the time of his absence, he is required to secure an excuse letter duly
signed by the parent or guardian. The letter must contain information which includes reason
for absence and the date of absence. An excuse slip, as completed by the nurse-on-duty, will
then be attached to the letter and will be presented to their teachers.
On staying inside the clinic:

 Students are only allowed to stay and rest in the clinic if they are sick and may stay in the
clinic until their condition is stable and vital signs are normal.
 Students who are sick are not allowed to bring friends to stay with them.
 Only one person is allowed on each bed.
 Eating, while on bed, is strictly not allowed unless it is necessary.
 Visitors are not allowed to stay inside the clinic. They can visit for only 5 minutes.
 Students are not allowed to standby inside the clinic.
