CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This document executed by and between the City of Roxas represented by the City Mayor,
HON. ANGEL ALAN B. CELINO, of legal age, Filipino, married and a resident of Magallanes
St., Brgy. I, Roxas City, hereinafter referred to as the FIRST PARTY:

AND

(NAME OF LSB TEACHER), of legal age, Filipino, and a resident of (Address of LSB
Teacher), hereinafter referred to as the SECOND PARTY;

WITNESSETH:

WHEREAS, the City of Roxas is in need of the services of an LSB Teachers at Schools
Division of Roxas City.

WHEREAS, the SECOND PARTY has expressed willingness to serve

DepEd Schools Division of Roxas City on a contract service basis;

NOW, THEREFORE, for and in consideration of the foregoing, the FIRST PARTY
hereby engages the services of the SECOND PARTY under the following terms and conditions:

1. That the SECOND PARTY shall be paid on a monthly basis at the rate of Eight
Thousand Pesos (P 8,000.00) per month of actual services rendered to DepEd Schools
Division of Roxas City;

2. That the term of this contract shall commence on __________________ and expire on
_______________ ;

3. That, the SECOND PARTY shall be required to report to the assigned school and
expected to perform the following functions:

3.1. Facilitates learning in the elementary/secondary schools through functional lesson

plans, daily log of activities and appropriate, adequate and update instructional
materials.
3.2. Monitors, assesses and evaluates pupils’/students’ progress.
3.3. Undertake activities to improve performance indicators.
3.4. Undertakes updated pupils’ school records.
3.5. Updates parents on pupils’ progress regularly.
3.6. Counsels and guides pupils.
3.7. Conducts action research.
3.8. Maintains cleanliness, orderliness, safety and upkeep of the classroom.
3.9. Participates in staff development activities and utilizes knowledge, skills and
attitudes (KSAs) gained from seminars, trainings and scholarship.
3.10. Maintains harmonious relationship with fellow teachers and other school personnel,
as well as with parents and other stakeholders.
3.11. Supports activities of governmental and non-governmental organizations, and
3.12. Does related work.

4. That the SECOND PARTY’s conduct and performance shall be under the direct control
and supervision of the Department of Education, Schools Division of Roxas City.
 5. That the SECOND PARTY shall not be entitled to the benefits enjoyed by the regular
employees such as ACA, PERA, RATA, Mid-Year Bonus, Productivity Incentive,
Christmas Bonus, Cash Gift, and such other benefits as may be provided for by the
pertinent law, administrative and/or executive orders;

6. That no employer-employee relationship exists between the FIRST PARTY and

SECOND PARTY;

7. That the services rendered by the SECOND PARTY shall not be considered Government
service, and

8. That the FIRST PARTY hereto certifies that the SECOND PARTY has not been
previously dismissed from the service due to commission of an administrative offense;
that she/he is not covered by the prohibition under the rules of nepotism, that she/he is
not being hired to perform the functions pertaining to vacant regular plantilla positions.

IN WITNESS WHEREOF, the parties have hereunto set their hands this ____ day of
