COMMISSION ON HIGHER EDUCATION

SELF-ASSESSMENT SURVEY – HORIZONTAL TYPE – UNIVERSITY (SAS-HT-U)


Directions: Listed below are characteristics required of universities as stated in CHED Memorandum Order No. 46, series 2012. Check the box for “Yes” if your institution already
has the required characteristic. If not, check the box for “No.” Then in the column “Details,” cite the statistical data supporting your answer. In the last column, briefly describe
the improvement plans your institution will implement especially for the items you have marked “No”. Attach with this survey form the horizontal instruments which can be
downloaded from the CHED website. Please submit both the survey form and supporting horizontal instruments to the Office of Institutional Quality Assurance and Governance
by December 1, 2016 through the CHED Regional Office. Please submit the SAS form in hard copy and supporting horizontal instruments in soft copies saved in CD. To ensure
the credibility of the answers, this survey form and supporting documents should be duly signed by the president and other appropriate officials.

professional licenses.
4.3 All full-time permanent faculty members teaching in the
doctoral programs have the relevant doctoral degrees
 
as required by CHED.
4.4 All full-time contractual faculty members have the
relevant degrees as required by CHED in the subject
 
areas they handle.
4.5 All full-time contractual faculty members handling core
subjects of licensed programs have the relevant
 
professional licenses.
4.6 All full-time contractual faculty members teaching in the
doctoral programs have the relevant doctoral degrees
 
as required by CHED.

4.7 All part-time faculty members have the relevant


degrees as required by CHED in the subject areas they
 
handle.
4.8 All part-time faculty members handling core subjects of
licensed programs have the relevant professional
 
licenses.

4.9 All part-time faculty members teaching in the doctoral
programs have the relevant doctoral degrees as
 
required by CHED.

5. Faculty Research
5.1 At least thirty (30) full-time faculty members are actively
involved in research and development activities in their
 
respective disciplines as evidenced by refereed
publications, and other scholarly outputs.

5.2 At least 20% of all full-time faculty members actively


involved in research and development activities in their
 
respective disciplines as evidenced by refereed
publications, and other scholarly outputs.

6. Research Program
6.1 The university has a viable research program in
specific (disciplined and multidisciplinary) areas of
 
study that produce new knowledge as evidenced by:

6.1.1 refereed publications

faculty members engaged in research have:
