AFFIDAVIT OF UNDERTAKING

REPUBLIC OF THE PHILIPPINES

CITY OF BACOLOD

I, RAMY R. JALANDO-ON, of legal age, married, Filipino and residing at Cedar Chesnut,
Villa Angela Subd., Brgy. Estefania, Bacolod City, Negros Occidental, after being duly sworn in
accordance with the law, depose and say:

1. That I am the owner of the motor vehicle which was flagged down and impounded by
elements of the Land Transportation Office (LTO) Region VI for allegedly operating as
a colorum vehicle in May 21, 2019 somewhere in the City of Bacolod;

2. That the said Motor Vehicle is covered by Certificate of Registration No. 318355671
and more particularly described as follows:

SERIES : AC
ENGINE NO. : 4JA1M62402
CHASSIS NO. : PABTBR54FH2094941
MV FILE NO. : 1301-00001111364

3. That I was not able to contest the said apprehension as the five (5) day period within
which to contest the same has already lapsed due to heavy workload and frequent
travels;

4. That our counsel deemed it best to pay the corresponding fine imposed by LTO for the
above-mentioned violation so we can request the release of the said vehicle from the
impounding area;

5. That we are respectfully requesting LTO Bacolod to transfer the impounding area of
the subject vehicle from the LTO grounds to our garage upon payment of the
prescribed fine and fees as the current impounding area is so overcrowded that the
safety and maintenance of the vehicle may be compromised;

6. That we undertake to keep the said vehicle in my garage until the decision and release
order from the Legal Department of LTO Region VI is copy furnished to LTO Bacolod;

7. That we hereby release LTO Bacolod from any liability, whether civil, administrative
or criminal, that may arise from the release of the subject vehicle into our custody, or
from any damage or deterioration of the vehicle by reason of the said impoundment;

8. That I am executing this affidavit to attest the truth of the foregoing and for the purpose
of transferring the impounding area of the subject vehicle in my garage pending the
release order from LTO Region VI.

IN WITNESS WHEREOF, I have hereunto set my hand this _______ day of July 2019,
