I, MARILYN VISORRO DEIPARINE, Filipino, of legal age, married, public school teacher, and with
present postal address at Barangay Sabang, Sibonga, Cebu, after having been duly sworn to in
accordance with law, do hereby depose and state:

AFFIDAVIT OF DISCREPANCY IN NAME

I, MARILYN VISORRO DEIPARINE, Filipino, of legal age, married, public

school teacher, and with present postal address at Barangay Sabang, Sibonga,
Cebu, after having been duly sworn to in accordance with law, do hereby depose
and state:

1. THAT prior to my marriage, my name is MARILYN CARCUEVAS VISORRO,

the last name being that of my late stepfather, GENARO TORRES VISORRO, and
since childhood, my parents, brothers and sister, including all members of my
family, friends and acquaintances call me MARILYN CARCUEVAS VISORRO and
thus since then and until I got lawfully married with my husband, Danilo Llanos
Deiparine, I have been accustomed to using the name MARILYN CARCUEVAS
VISORRO-DEIPARINE in all my personal and school records, transactions and
communications, including my present employment in the government as a public
school teacher.

2. THAT, however, my name as registered and shown in my birth certificate

issued by the office of the Civil Registry of Sibonga, Cebu where I was born is
MARILYN CARCUEVAS. A copy of my Birth Certificate, secured from the National
Statistics Office is hereto attached as ANNEX “A”.

3. THAT my name MARILYN CARCUEVAS as appearing in my said birth

certificate is without a middle name since I am an illegitimate child of my mother,
Juanita Maquiputen Carcuevas (now Visorro) during her maiden days until she
lawfully got married to my stepfather, GENARO TORRES VISORRO.

4. THAT my name MARILYN CARCUEVAS VISORRO-DEIPARINE and my name

MARILYN CARCUEVAS as appearing in my said birth certificate are but one and
the same, and belonging only to one and the same person, being myself.

5. THAT I wish to make all my personal and public records straight by

simply adopting completely my name MARILYN CARCUEVAS as appearing in my
said birth certificate, and thus I am actually and in reality MARILYN CARCUEVAS
DEIPARINE in person.

6. THAT I am executing this Affidavit to attest to the truth of the foregoing

facts and to use the same for whatever legal purpose it may serve.
