AMENDMENT TO JOB ORDER CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This Amended Contract is made and entered this __day of _________ 2018 by
and between:
The _______________________________________________ , a
government agency with principal office located at
______________________________, represented herein by
_________________________, in his capacity as ___________________,
hereinafter referred to as the "FIRST PARTY"
-and-
Mr/s. ________________, of legal age, Filipino and a resident of
_________________________, herein referred to as the "Second Party”

WHEREAS, a Job Order Contract was executed by and between the First and
Second Party on _________________ 2018 for the services of the latter as Project
Assistant II for the Project “_____________________________”;
WHEREAS, it is stated in the said Contract that the Second Party will be hired
as a “job order contractual worker” for a period six (6) months beginning on the date
of notarization of the contract;
WHEREAS, the true intention of the parties is to immediately commence the
services of the second party on 16 July 2018 to catch up with the project timeline which
already began on ____________;
WHEREAS, to manifest the true intent as to the effectivity of the contract, both
parties hereby desire to amend and modify the previous contract;
NOW THEREFORE, for and in consideration of the foregoing premises and
stipulations hereinafter set forth, the parties hereby mutually agreed, as follows:
1. Item 5. a. of Job Order Contract of Service dated _____________ 2018 shall be
amended as follows:

a. “The Second Party is hereby hired for the period of six (6) months beginning
on 16 July 2018 as Project Assistant III with the status as “project-based
job order contractual worker” and to be paid Twenty-six Thousand Five
Hundred Seventy-eight Pesos and Eighty Centavos (P26,578.80) per
month. The said amount is based on the attached Line-Item Budget of a
project entitled
“____________________________________________________”, which
are currently implemented by the First Party”;
2. Except as set forth in this amendment, the agreement is unaffected and shall
continue in full force and effect in accordance with its term.
