AFFIDAVIT

I, JESSNIEL L. MEDIDAS, of legal age, Filipino Citizen with postal address at Potohan,
Tubigon, Bohol, after having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the operator/owner of the business under the name MILES OF SMILES
TRAVEL AND TOURS with business address at Centro, Tubigon, Bohol;

2. That the business name MILES OF SMILES TRAVEL AND TOURS has already
been registered under the Department of Trade and Industry, however, there was a
requirement to post a Surety or Cash Bond;

3. That when I was about to post a Cash Bond, the DTI personnel in charge referred me to
Malayan Insurance for the latter will be the one to act as my surety upon payment of the
collateral and premium;

4. I have already negotiated and discussed with Malayan Insurance regarding this matter, I
have already paid the collateral amount of P5, 000 and I am just waiting for the approval
of my policy;

5. That upon receipt of such policy, my DTI Clearance will be granted and my application
for registration in the BIR and the procurement of my business permit will be ready for
processing and hopefully will be processed;

6. I am executing this Affidavit to attest the truth of the foregoing and for whatever legal
purpose/s this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 6th day of March,
