AFFIDAVIT OF TWO DISINTERESTED PERSONS

KNOW ALL MEN BY THESE PRESENTS:

WE, EMILY L. JERESANO, of legal age, married and presently residing at

Gate 3, SPPVS Barangay Bibincahan, Sorsogon City and RONALDO A. JETAJOBE,
of legal age, single and presently residing at Molave St., Gate 3, SPPVS, Barangay
Bibincahan, both after being duly sworn to in accordance with law, DEPOSE
and STATE:

1. That we personally know DEBBIE DENIEGA OREJO, being

our neighbor and acquaintance for a long period of time;

2. That we know for a fact that she is first born of Mr. Daniel
Lacsinto Orejo and Ma. Leila Sanchez Deniega born on March 26, 2002,
because we even attended her baptism and her birthday celebrations;

3. That perusal to her Certificate of Live Birth, per Registry

No. 2002-3591 issued by Office of the Civil Registrar Sorsogon City, her
name DEBBIE DENIEGA OREJO, was inadvertently entered as
DEBBIE SANCHEZ and the name of her mother Ma. Leila Sanchez
Deniega was inadvertently entered as Ma. Leila Deniega Sanchez;
[Hereto attached as ANNEX “A”].

4. That we certify that she was born at Sorsogon City

Province of Sorsogon, Philippines, on the 26th day of March 2002.
That she has been using the name DEBBIE DENIEGA OREJO
in all her official dealing and scholastic records and transactions;

5. That we hereby attest that the person that we know with the
name DEBBIE DENIEGA OREJO and that with the name DEBBIE
SANCHEZ per Registry No. 2002-3591, is ONE AND THE SAME
PERSON;

6. That we are not related by affinity or consanguinity to

the above mentioned person; and

7. That we are executing this affidavit to attest to the truth

of the foregoing facts for record and in order to apply for its correction.
