ENDORSEMENT

Delivery Room Day 1 - August 8, 2019

In-labor patients:

1. Patient De Luna is 23 years old, single, with an obstetric history of G3P1. Her AOG

is 38 weeks. Cephalic in labor. She has been administered with D5LR @ 120. She

will use IUD as her family planning method after birth. Cervical dilatation is 1 cm.

2. Patient Canoy is 29 years old, single, with an obstetric history of G2P1. Cephalic

in labor. She’s for CS because of CPD (Cephalopelvic Disproportion). She is

currently on NPO and has been administered with D5LR @ 120. She will undergo

Bilateral Tuba Ligation (BTL) as her family planning method.

3. Patient Traya is 17 years old, single, a primigravida mother with an obstetric history

of G1P0. Her AOG is 37 weeks and 3 days. Position of the baby is in transverse

and not in labor. She is for CS. She has been administered with D5LR @ 120.

4. Patient Fontanilla is 33 years old, married, and has an obstetric history of G2P1.

Cephalic in labor. She has already passed urine. She is currently on NPO and has

been administered with D5LR @ 120. Her cervical dilatation is 4 cm.

5. Patient Latrop is a 35 years old, single, with an obstetric history of G2P1. Cephalic

in labor. There is fetal congenital anomaly and she had an appendectomy in the

year 2011. She has been administered with D5LR @ 120. Her cervical dilatation

is 2 cm with 40% effacement.

6. Patient Pilar is a 24-year-old single primigravida with an obstetric history of G1P0.

Cephalic and not in labor. She is diagnosed with oligohydramnios. She’s on NPO

but would change to soft diet. She has been administered with D5LR @ 140.
7. Patient Cundaran is a 38-year-old married Muslim woman who is a primigravida

and has an obstetric history of G1P0. Cephalic in labor. She has gestational

hypertension. Her case is considered as an elderly pregnancy. She’s on NPO and

is on D5LR @ 120 + MgSO4. Her cervical dilatation is 2 cm.

8. Patient Ferones is a 27-year-old married woman who is a primigravida and has an

obstetric history of G1P0. Cephalic and not in labor. She has tachycardia and

hydronephrosis. She is currently on NPO and has been infused with D5LR @ 120.

9. Patient Navarez is a 16-year-old single woman who is a primigravida and has an

obstetric history of G1P0. Cephalic not in labor. Teenage pregnancy. She’s on

NPO and has been infused with D5LR @ 120. Cervical dilatation is 1 cm.

10. Patient Rivera is a 32-year-old married woman who is a primigravida and has an

obstetric history of G1P0. Her AOG is 39 weeks and 1 day. She is currently on

NPO and is infused with D5LR @ 120. Cervical dilatation is 3-4cm with 60%

effacement.

11. Patient Transmonte is a 26-year-old married woman who is a primigravida and has

an obstetric history of G1P0. Cephalic in labor. Has gestational hypertension.

Currently on NPO and has been infused with PNSS @ 140 + MgSO4 drip. Cervical

dilatation is 3-4 cm.

12. Patient Cahayahay is a 31-year-old married woman who is a primigravida and has

an obstetric history of G1P0. She has been infused with D5LR @ 120 and is on

NPO. She is administered with 2g of ampu q 6.

 13. Patient Aking is a 30-year-old single woman with an obstetric history of G5P4. Her

AOG is 39 weeks and 1 day. Cephalic in labor. She also has toxic goiter. She is

on NPO. She has been infused with D5LR @ 120 and was given an epidural.

14. Patient Pelino is a 36-year-old woman who has an obstetric history of G1P0. She

has gestational diabetes. She has been infused with D5LR @ 120 and is for CS.

15. Patient Agawin is a 26-year-old primigravida and has an obstetric history of G1P0.

CBG q 6. She has been infused with D5LR @ 120 and is for CS.

Postpartum Patients:

1. Patient Sotto has delivered a baby girl. She has been administered with carbetocin

and tranex. 1 amp of tramadol was given. Has a foley catheter inserted.

2. Patient Mangadlao has given birth to a baby girl. Oxytocin was given

intramuscularly.

3. Patient Cansancio has given birth to a baby boy. Oxytocin was given

intramuscularly. Tranex 1g was given. MgS04 drip and with foley catheter was

inserted.

4. Patient Pangandaman has given birth to a baby boy. She has an obstetric history

of G3P3. Oxytocin was given intramuscularly.

5. Patient Isugan has given birth to a baby boy with a poor apgar score. Oxytocin IM

+ 30 was given.

6. Patient Elado has given birth and was given Oxytocin IM + 30.

7. Patient Dela Cruz has given birth to a baby girl and has been given Oxytocin IM +

30.

8. Patient Iligan has given birth to a baby girl and is post IUD. Oxytocin IM was given.
