Problem Presented:
The client is seeking financial assistance for medicine of his son James Pantoja who met
a vehicular accident at Guinsay, Danao City last January 20, 2018 while riding a private car of his
friend from a recreation.
Client Gemma Pantoja is a widow; she has 5 siblings of her late husband Cesario Pantoja
of Taytay, Danao City. She serves as the only breadwinner of the family of being a fish vendor.
Last January 20, 2018 her 4th child James Pantoja had invited a friend for a recreation to
Carmen, Cebu. Unfortunately, while on their way home they meet a vehicular accident at
Guinsay, Danao City. Three of them had died and her son James Pantoja also was immediately
confined at Vicente, Sotto, Memorial, Medical Center, Cebu (VSSMC) for his multiple physical
injury and immediate treatment and medication had done. As a mother she is very much
worried for the financial funding for the laboratories and the prescribe medicines for his son.
The family is financially unstable in terms of income and affirmed that they are one of the
indigent families in their barangay.
Thus, she humbly asked financial assistance for medicine, laboratories and medicine
maintenance for her son. She is so much pleased for a favorable attention imparted in this

separated from her husband for almost four years now.
Cresel Menor has a sister-in law named Imee Dierran a former production worker at
MEPZA, Lapu-lapu City, Cebu. She considers her a generous, thoughtful and a compassionate
person due to the kindness imparted for her family. Till, such time her in-law was diagnosed of
Breast CA, Left S/P MRM. Undergone 10 times chemotherapy sessions since March 2017 to
December 2017 and she’s the one who helps her in inquiring and finding any institution for her
financial sources. Recently, her sister in-law is advised to undergo External Beam Radiation
Therapy via Conventional Radiation Therapy 28 fractions at Cebu Doctors Hospital, Cebu City.
The client at the moment is affirming that they are now financially exhausted from her in –laws
previous chemotherapy. They tried to ask a favor from her friends and relatives still can’t
suffice for all the needed cost. The family’s financial instability hampers their plan for the said
Radiation.
Thus, she decided to ask a favor to avail financial assistance on your good office.
Likewise she is praying and anticipating that it would be positively attended.
